본 연구는 서로 다른 의료제도와 정책을 가지고 있는 미국과 한국의 의료정책과 의료시설정책의 변화들이‘의료시설’라는 구체적인 매체안에서 어떠한 방식으로 공간을 구축하고 유형화되는 지를 살펴보고 각 의료영역의 공간적 특성과 사회적 의미를 고찰하였다. 본 연구의 목적은 미국과 한국의 의료시설 공간구조의 연대별 변화분석을 통해 의료정책의 변화간의 상관관계를 분석하고 이를 통해 의료시설계획 변화의 흐름과 특성을 파악하여 현재 우리나라 의료시설계획이 가지고 있는 문제점을 도출하고 그 개선방안을 제안하였다. 연구방법 및 범위는 문헌조사, 통계조사, 인터넷조사 등을 통해 의료제도와 의료정책의 특성을 고찰하여 그 개념을 정립하고, 미국과 한국의 의료제도와 의료정책의 변화를 그 특성에 따라 연대기적으로 분류하여 공간구조의 변화와의 상관관계를 분석하여 의료시설의 변화원인을 규명하였다. 또한 미국과 한국 의료시설의 병동부, 영상의학과, 수술부, 응급부, 외래진료부의 사례분석을 통해 연대별 공간구조체계와 특성을 분석하였다.
연구결과는 다음과 같다.
첫 번째, 주요 국가별 의료시스템과 의료정책에 대한 고찰을 통해 현재 많은 국가들의 의료정책방향은 공공의료지불을 억제하면서 양질의 의료서비스를 제공하는 데 초점을 두고 있으며 의료시설정책의 방향도 역시 이러한 경향에 영향을 받아 의료 효율성과 양질의 의료서비스를 제공하는데 초점을 두고 있음을 알 수 있다.
두 번째, 앞서 다루어진 의료시스템과 의료정책 고찰에 기초하여 미국과 한국의 의료제도와 의료정책을 주요 변화 시기중심으로 발전과정을 살펴보면 메르스와 같은 전염성질환의 창궐, 의료기술의 발달, 공공의료지불억제 등의 변화요인들이 의료효율성과 양질의 의료제공과 같은 의료정책시행, 미국의 관리의료(HMOs)와 포괄수가제(DRGs)도입 등의 의료수가제도의 변화, 의료시설관련 규정 및 체계에 중요한 요소로 작용했음을 알 수 있다. 우리나라의 경우, 미국의 경우와 마찬가지로 위와 같은 의료정책들이 시행되고 의료시설의 공간구조에 직간접적으로 영향을 미치고 있으나 의료시설관련법령 및 의료시설기준에 관련된 정책이나 규정체계는 미비함에 따라 신속하고 유연한 대처가 어려운 실정이다. 따라서 이에 대한 보완이 필요하다.
세 번째, 미국과 한국의 의료시설의 변천과정을 연대별로 비교분석하여 살펴보면, 미국 의료시설은 1973년 관리의료(HMOs)와 1980년대 포괄수가제(DRGs)도입으로 미국의 의료정책의 방향이 병원입원기간을 단축시키는 등의 비용절감과 더불어 의료의 효율성을 높이기 위해 양질의 의료서비스에 초점을 두는 의료정책에 의해 병동부전체를 1인 실화하고 이에 따라 병동부 계획도 이에 대응하는 1인실(Private room)구성에 적합한 새로운 병동부 디자인이 나타나게 된다. 의료시설관련 연구 분야에서도 안정성, 의료과실, 병원 내 감염 등의 의료의 질 개선에 초점을 둔 근거중심의 디자인(Evidence-based Design)연구가 활발하게 일어나면서 병동 내 정원 설치, 각 병실의 조망권 확보, 유니버설 병실, Decentralized Nursing Station, 백열등사용, 가족지원공간제공, 린 디자인 등이 나타나고 있다. 이러한 연구결과들을 토대로 치유환경과 의료서비스의 효율성에 초점을 둔 의료시설계획이 실제 의료 환경을 평가하는 중요한 요소로서 미국 내에서 자리 잡게 되었고 실제 시설기준으로 명문화됨을 알 수 있다. 그러나 우리나라의 경우, 메르스와 같은 전염성질환의 창궐, 의료기술의 발달, 환자중심의 의료시설 등의 변화요인들에 의해 의료시설의 공간구조도 역시 발전해오고 있으나 아직까지는 미국, 일본과 유럽 주요 국가들에 비해 뒤따라가는 양상을 보여준다. 이러한 배경에는 최소기준만을 제시하는 의료시설관련 규정과 의료시설기준의 부재가 주요한 요인으로 보인다.
네 번째, 연대별 미국과 한국 의료시설의 공간구조를 살펴보기 위해 병동부, 영상의학과, 수술부의 공간구조를 비교분석하여 살펴보면 다음과 같다.
미국 병동부의 공간구조를 살펴보면, 1980년대 포괄수가제 도입이후 병동부의 공간구조 내에서 의료진의 업무 효율성을 높이고 환자에 대한 양질의 의료서비스를 제공에 초점을 둔 정책들이 시행되면서 사생활과 안전성이 병동부의 공간구조 특성으로 나타나고 있다. 그러나 우리나라 병동부는 2000년대로 들어서면서부터 환자중심의 계획이 수행되고 있긴 하나 미약한 편이며, 아직까지는 ‘환자중심의 의료시설’이라기보다는 ‘환자에 초점을 둔 의료시설’ 로서 환자와 환자가족을 고려한 좀 더 실질적인 계획이 필요하다고 본다. 미국과 한국의 영상의학과의 공간구조를 살펴보면 의학기술의 발전에 의한 새로운 영상기기의 도입, 포괄수가제나 영상기기관련 수가제도의 변화가 일어난 시기를 기점으로 지속적으로 성장해왔다. 미국의 경우, 포과수가제가 시작된 1980년대를 기점으로, 우리나라의 경우도 포괄수가제도 시행, 국민 건강검진제도 시행, 외래진료센터의 등장 CT & MRI의 의료보험 적용 등의 의료정책이나 의료보험수가제도의 변화가 일어났던 2000년대 이후에 폭발적인 성장세에 있다. 미국의 경우, 계획 시 영상의학과 내에서의 의료장비 중복투자방지, 개인 사생활과 업무 효율성증대에 초점을 둔 계획이 나타나고 있다. 우리나라의 경우도 비슷하긴 하나 업무 효율성과 개인 사생활측면에 대한 고려가 좀 더 필요하다. 미국과 한국의 수술부의 공간구조를 살펴보면, 외래수술실, 유니버설 수술실과 수술부 내에서의 의료진공간에 대한 고려가 나타나고 있으며 미국의 수술부계획시 향후 새로운 시설의 확충 등에 대비한 증설계획도 같이 고려한 4개이나 6개의 수술실로 구성된 소규모 Surgery cluster suite가 주로 적용되고 있다. 우리나라의 경우, 몇몇 병원들은 미국의 경우 매우 보편적인 계획방법으로 적용되고 있는 ‘shell’공간을 수술부 내에 미리 배치시키는 계획이 나타나고 있으나 아직 증축에 대한 고려는 미흡한 실정이다. 실제 증축이 어려운 선형유형과 대규모 단일 ‘perimeter’유형이 가장 보편적인 유형으로 사용되고 있다. 미국과 한국의 응급부를 살펴보면, 미국 응급부의 공간구조는 응급진료실을 1인실로 계획하고 Trauma room과 심폐소생실의 위치는 구급차진출입구에 근접하여 배치시키는 유형이 일반적이다. 이러한 공간구조는 응급환자들을 triage room, fast-track, emergency, trauma 등등의 치료영역으로 신속하게 이동시켜 치료받게 하며 환자의 체류시간을 줄일 수 있다. 이와 아울러 Shell space나 perimeter layout과 같이 향후 응급부 확장에 대한 고려가 초기 계획단계에서부터 반영된 유형이 나타나고 있다. 의료진영역을 살펴보면, 린 디자인(Lean Design)에 의한 지원영역이 한곳에 집중 배치되고 있으며 업무효율성을 극대화하는 inner-core계획이 나타났다. 최근경향은 미국 내의 약물남용과 정신건강에 대한 인식이 높아지면서 인지장애가 있는 환자들을 위한 응급영역이 계획되고 있다. 한국의 응급부도 역시 계속적인 성장세에 있으며 이에 따른 공간구조의 변화도 나타나고 있다. 예를 들어, 미국의 1990년대 응급부평면도와 유사한 유형이 사용되고 있다. 그러나 세분화된 중증도에 따른 영역분류유형은 환자의 대기시간과 체류시간을 늘어 치료의 효율성을 저하시키는 단점이 있음을 인지할 필요가 있다. 또한 앞으로 감염 및 신속한 외상환자와 응급환자들의 치료를 위해 별도의 구급차진출입구 설치를 고려해야 할 필요성 있다. 또한 우리나라의 경우에도 응급부내에 정신질환환자영역을 따로 분리하여 계획할 필요가 있다. 미국과 한국의 외래진료부를 살펴보면, 미국 외래진료부는 1973년 HMO와 1983년 DRG, 2010년 The Affordable Care Act 등에 의해 팽창과 변화가 지속되고 있다. 1990년대에는 검사영역과 진료영역간의 관계에 초점이 맞추어졌다면, 2000년대 이후는 환자와 업무의 효율성에 초점을 두고 계획됨을 알 수 있다. 우리나라도 역시 포괄수가제의 도입과 종합병원 내에서의 외래진료부의 수익률증가로 인해 향후 10년 동안에도 외래부가 지속적으로 발전해 나갈 것으로 판단되며 이에 따라 몇 가지 고려사항들을 살펴보면, 시대적인 변화와 경제성을 고려하여 외래부문 공간사용의 유연성에 대한 검토가 필요하며 환자와 의료진들에 대한 공간적 고려가 나타나고 있긴 하나 아직 의료기관적인 측면에서만 고려되고 있어 환경적인 측면에서의 고려가 더 필요하다.
미국과 한국의 의료시설의 가장 두드러진 차이점은 미국의 의료시설은 의료시장에 계속적으로 새로운 유형을 개발해서 판매하는 신상품에 비유한다면, 우리나라의 의료시설은 기존상품에 새로운 기능을 추가해서 판매하는 업그레이드상품에 비유할 수 있다. 미국의 의료제도는 앞서 언급했던 것처럼 전체적으로 고비용 저효율의 구조를 가지고 있다. 그러나 이를 개선하기 위한 미국정부의 의료개혁과정들을 통하여 미국의 정책방향이 비용절감과 양질의 의료제공에 중점을 두게 되면서 정부와 의료공급자들이 의료 환경개선의 중요성을 많은 연구들을 통하여 인식되었다. 이러한 의료시설정책들이 가능하기 위해서는 첫 번째, 의료시설개선효과가 의료비용절감과 환자들에게 더 좋은 의료효과를 제공함을 증명해주는 연구들이 필요하다. 두 번째, 의료공급자들이 먼저 양질의 의료서비스 제공과 수익률 증대를 위하여 의료 환경의 중요성을 인식하고 시설분야에 대한 과감한 투자가 필요하다. 마지막으로, 의료 환경의 개선효과가 의료비용을 증대시킨다는 부정적인 인식에서 개선효과를 통하여 의료비용절감효과와 환자들에게 양질의 치료결과를 얻을 수 있음을 정부의료관련기관들에게 인식시키고 의료시설개선에 대한 대중적인 관심이 필요하다. 본 연구의 의의는 앞으로 우리나라에서 시행되는 의료정책이나 의료시설정책 등이 의료시설에 어떠한 영향을 주는 지를 예측할 수 있는 기초자료로 활용할 수 있고 또한 새로운 의료시설정책을 우리나라에 적용되었을 때 예상되는 결과를 예측하여 이에 대비할 수 있다고 본다. 본 연구에서는 다루어지지 못하였으나 의료부문의 공용면적비율이나 전체공간구조에 대한 미국과 한국 간의비교분석을 통해 좀 더 정확한 기초자료연구가 진행되어야 할 것으로 사료된다.

The purpose of this study is to analyze the correlation between medical policy changes through analysis of changes in the spatial structure of medical facilities in the United States and Korea, and to suggest improvement of problems were presented.
The research methods and scope are to review the characteristics of medical system and medical policy through literature survey, statistical survey, internet survey, and to analyze the correlation with the changes of spatial structure to identify the cause of change in the medical facility. In addition, This Study analyzed the spatial structure and the characteristics by analyzing case studies of Inpatient Care department, Radiology department, Surgery department, Emergency department, and Outpatient department of Korean and US medical facilities.
The results of the study are as follows:
