Stroke is the leading cause of dysphagia. Swallowing disorders cause serious problems such as increased mortality, prolonged hospital stay, aspiration pneumonia, malnutrition, and dehydration. There is a need for a systematic protocol that occupational therapists can apply in the clinical field. The purpose of this study is to develop a protocol for swallowing rehabilitation therapy that allows occupational therapists to perform evidence - based swallowing rehabilitation therapy for stroke patients with swallowing disorders. Search was performed using computerized databases(Pubmed, Medline Complete(EBSCOhost), National assembly library of Korea) and citation search from January 1979 to December 2018. For the keywords, “(dysphagia OR “deglutition disorder” OR “swallowing disorder”) AND (stroke OR Apoplexy OR CVA OR “cerebrovascular accident” OR “cerebrovascular apoplexy” OR “vascular accident”) AND (treatment OR therapy OR rehabilitation)” were used to identify systematic review studies. After reviewing the collected studies, the draft protocol for swallowing rehabilitation treatment was developed. To develop the preliminary protocol, the content validity of the protocol draft was verified by 10 expert groups. As a result, the content validity of the total items was 0.81. After that, 10 occupational therapists performing real swallowing rehabilitation were verified the user validity to apply the preliminary protocol to 50 patients, 5 patients each. The questionnaires were conducted on the appropriateness of the preliminary protocol to the participating experts. Based on this, we developed a final protocol through further interviews. The completed final protocol lists the symptoms based on the most commonly used test items for swallowing disorders. We segmented them into specific problems and linked them with evidence-based therapies to treat each problem. In addition, categories of evidence and consensus were presented to reflect the opinions of the rehabilitation therapists. The swallowing rehabilitation protocol developed in this study will provide important information for occupational therapists to perform effective evidence - based treatment. In conclusion, the swallow rehabilitation treatment protocol will be an important data to maintain and manage the quality of the swallow rehabilitation treatment provided by occupational therapist above a certain level.
