Comparison of acute effect of ankle bathing versus aerobic exercise on vascular function in young adults
The favorable therapeutic effects of habitual hot water or sauna bathing on vascular function have been established, and akle bathing in hot water is an alternative thermal therapy that could potentially yield similar positive outcomes on vascular function. Although thermal therapy may serve as an alternative therapeutic strategy to improve vascular function, it remains unclear whether ankle bathing in hot water may be as effective as aerobic exercise in improving vascular health. Therefore, we tested the hypothesis that an acute bout of ankle bathing in hot water would be as effective as aerobic exercise in transiently improving vascular health in healthy young adults. Forty-two participants (25 male and 17 female; age: 22±2 years) were randomized to 40 min of either: (1)an acute bout of moderate-intensity aerobic exercise (AE)(n=13, 40-60% of heart rate reserve, walking on a treadmill), (2)ankle bathing in hot water (AB)(n=15, 43℃), or (3) ankle bathing in lukewarm water as a control (CON)(n=14, 36℃). Brachial artery flow-mediated dilation (FMD) and carotid-femoral pulse wave velocity (c-f PWV) as indices of vascular function were measured before and at 30 min and 90 min after interventions using Doppler ultrasound and applanation tonometry, respectively. The AE (baseline: 7.8 ± 2.9%, post 30 min: 8.9 ± 2.1%, post 90 min: 9.2 ± 2.9%, p≤0.05) and AB (baseline: 7.2 ± 1.7%, post 30 min: 8.9 ± 2.4%, post 90 min: 9.4 ± 3.0%, p≤0.05) trials increased FMD significantly, compared with baseline. However, there were no significant difference among three trials. c-f PWV decreased in AE (basline: 6.7 ± 0.7m/s, post 30 min: 6.0 ± 0.7m/s, post 90 min: 6.0 ± 0.6m/s, p≤0.05) and AB (basline: 7.0 ± 0.8m/s, post 30 min: 6.6 ± 0.7m/s, post 90 min: 6.7 ± 0.8m/s, p≤0.05) trials significantly, compared with baseline but there were no significant difference between AB trial and CON trial. These findings suggest that ankle bathing in hot water were not as effective as aerobic exercise in improving vascular health in young adults. Therefore ankle bathing in hot water may not be served as an alternative exercise modality to improve vascular function in healthy individuals.
