Purpose: Psychological well-being, that is happiness index (HI) thought to be a determinant of health and well-being. The purpose of this study was to observe the level and associating factors of HI among Korean adults by life cycle age
Methods: The subjects were total 226,545 from the secondary data source, 2015 community health survey (CHS). HI was checked using the single question with 10 Likert scale (1 to 10), and the subjective happiness cognition rate (SHCR) was defined as with the higher score of eight, that corresponded the fourth percentile, the top 25 percent or higher when classified by quartile. As relating factors, informations about demographic characteristics, health behavior, morbidity and health service utilization, socio-environmental satisfaction, frequency of contacts with neighborhoods, and participations to the social activities were collected. The life cycle age brackets organized by life cycle of 19-44, 45-64, 65-74, and over 75 years. Data analysis performed using SPSS program (ver 25.0). In univariate analysis, the SHCR compared by the selected characteristics using chi-square test, and influencing factors to the SHCR in total and by the life cycle age observed using multiple logistic regression analysis.
Results: Statistics of the HI score on the whole subjects were as follows: 1st quartile (Q1) 5, 3rd quartile (Q3) 8, median 7, mean±S.D. 6.68±1.82, and the SHCR was 34.7%. As results of the univariate analysis, SHCR was lower when aged, instead, higher when higher education and income, and currently with spouse (p<0.001). When subjected the whole subjects, the hazard ratio (HR) of influencing to SHCR were as follows: current smoking (0.70), high risk drinking (0.89), breakfast missing (0.88), physical activity (1.25), regular walking (1.24), sleep over 8 hours (1.07) in health behavior, good self rated health (2.49), hypertension (0.72), diabetes (0.70), hyperlipidemia (0.82), arthritis (0.57), unmet health need (0.55), accident (0.82) in morbidity and health service utilization, neighborhoods trust (1.42), satisfaction to safety (1.36), social environments (1.36), natural environments (1.27), health service (1.21), public transport (1.18) in satisfaction to neighborhoods and environments, family (1.34), friends (1.30), neighborhoods (1.03) in social contact over two times per week, public service (2.16), leisure (1.98), friendship (1.42), religion (1.41) in social activities participation. When subjected the differentiated group with life cycle age, HR of the biggest influencing to SHCR were as follows: as health behavior, in men, current smoking at 19-44 years (0.62) and 45-64 years (0.57), breakfast missing at 64-74 years (0.56) and over 75 years (0.59), in women, current smoking at 19-44 years (0.37), 45-64 years (0.39), 64-74 years (0.45), breakfast missing at over 75 years (0.49), as morbidity and health service utilization, unmet health need in both gender at all the four life cycle age brackets, in men (0.62, 0.54, 0.46, 0.35), in women (0.59, 0.56, 0.42, 0.36), as socio-environmental satisfaction, in men, neighborhoods trust at 19-44 years (1.75), 45-64 years (1.60), 64-74 years (1.40), satisfaction to health service at over 75 years (1.55), in women, neighborhoods trust at 19-44 years (1.85), 45-64 years (1.57), satisfaction to health service at 65-74 years (1.46), satisfaction to social environment at over 75 years (1.60), as social contact, in men, family at 19-44 years (1.32), 45-64 years (1.41), 65-74 years (1.49), friend at over 75 years (1.62), in women, family at 19-44 years (1.35), over 75 years (1.55), friend at 45-64 years (1.42), 65-74 years (1.49), as social activities participation, in men, public service at 19-44 years (1.78), 45-64 years (2.07), 65-74 years (2.63), leisure at over 75 years (2.24), in women, public service at all the four life cycle age brackets (1.91, 2.28, 2.38, 2.74). The results of multiple logistic regression analysis were adjusted with gender, education, income and spouse status. When subjected the whole subjects, the OR (95% CI) of significantly affects SHCR were as follows: social activities participation (3 or more) 2.07 (1.99-2.15), socio-environmental satisfaction (4 or more) 1.68 (1.56-1.81), contact with neighborhoods (2 or more) 1.50 (1.46-1.54), breakfast not missing 1.26 (1.23-1.30), sleep over 8 hours 1.19 (1.16-1.22) let SHCR increased, instead, unmet health need 0.63 (0.61-0.66), current smoking 0.71 (0.68-0.73), prevalent NCD (3 or more) 0.72 (0.69-0.76), high risk drinking 0.87 (0.84-0.89) let SHCR decreased. When subjected the differentiated group with life cycle age, the most affective factor to SHCR at all the four life cycle age brackets were also social activities participation as positive, but, unmet health need as negative with the highest OR (95% CI) as 2.48 (2.22-2.77) at 65-74 years and 0.45 (0.38-0.53) at over 75 years, respectively.
