본 연구의 과정은 정신적 고통의 발현에서부터 약물 복용, 그리고 약물 중단과 그 이후까지 광범위한 시간적 흐름에 따른 경험의 궤적들을 연구참여자들과 함께 따라가는 일이었다. 연구 결과 도출된 주제에 따르면 정신장애인의 약물중단 경험은, ‘현실을 잠식시켰던 혼돈의 경계를 긋고, 다시 현실을 찾아가는 여정’이었다. 이 여정은 총 4개의 단계로 이루어진다. (1) 정신적 고통으로 혼돈이 출현하면서 현실이 잠식되어 (2) 약물로 혼돈이 뒤덮이게 되지만 당사자들은 현실의 문턱을 넘지 못한다. (3) 약물을 떼어내고 혼돈에 직면하자 현실이라는 목적지가 선명해지고 (4) 혼돈 속에서 발견한 나침반으로 혼돈에 경계를 긋고 현실을 찾아갈 수 있게 되었다. 정신장애 당사자들은 오래된 과거부터 주변에 조성되었던 심리사회적 환경과 경험들로 인해 스트레스 상황에 대한 ‘약함’을 형성하였다. 정신적 지형이 약해진 상태에서 마주한 ‘압도적인 스트레스 상황’은 정신적 고통을 촉발하는 ‘트리거’가 된다. 그렇게 시작된 정신적 고통을 통제할 수 없고 그 고통에 매몰됨으로써 ‘혼돈’의 상태에 빠지게 되는데, 이 ‘혼돈’의 영역은 정신장애인이 기존에 위치해있던 ‘현실’의 영역을 점차 침범하고, 결국 당사자들의 ‘현실’은 점차 혼돈에 잠식되어 간다. 정신장애 당사자들을 옆에서 지켜보는 타자들은 혼돈을 억제하기 위해 정신적 고통을 ‘소거’할 것을 강요한다. 이러한 강요의 방식은, 사회에서 당사자를 격리시켜버리는 ‘물리적 억제’와 사회 안에서도 약물을 통해 당사자들의 행위를 통제하려 하는 ‘화학적 억제’로 나뉜다. 그 중 ‘화학적 억제’에 해당되는 정신약물은 당사자들의 정신적 고통을 ‘위압’하는 동시에, 정신장애 당사자 자체를 ‘위압’하기도 한다. 때문에 정신장애인들은 약물 복용과 함께 정신적 고통이 둔감해지는 효과를 경험하지만, 그들의 신체와 기능, 감각과 감정도 함께 둔감해지는 것을 느낀다. 하지만 약물을 복용하여도 증상은 잔류해있고 재발도 수차례 발생하는 것을 경험하면서 점차 약물을 통해 혼돈에서 벗어날 수 없다는 사실을 인지하기 시작한다. 약물은 혼돈을 없애주는 것이 아니라 그저 혼돈을 뒤덮으며 억누르고 있었으며, 당사자들은 약물 복용에 대한 의구심을 갖게 된다. 이에 따라 정신장애인들은 약물이 그들의 삶에서 어떤 의미인지, 복용 또는 중단했을 때의 득과 실은 무엇인지에 대해 생각하게 된다. 이러한 ‘사유’는 약물의 위압 작용에서 벗어나기 위한 시도로 이어졌다. 한편 약물의 위압 작용으로부터 벗어난다는 것은 당사자의 정신적 고통이 다시 증가할 수 있다는 가능성과 당사자가 본연의 모습을 되찾을 수 있다는 사실을 동시에 내포하고 있기에, 정신장애인은 약물의 복용 여부에 대한 선택의 기로에 서게 된다. 이 때, 정신장애인은 ‘약물에 취한 모습’이 아닌 자기 본연의 모습으로 정신적 고통을 견뎌내는 길을 선택하고, ‘고유의 시선’으로 그 혼돈을 새롭게 바라보게 된다. 이처럼 정신장애인은 약물의 작용에서 벗어나 본인의 혼돈에 기꺼이 직면하였다. 혼돈에 직면한다는 것은 ‘약물의 위압으로부터 벗어나는 것’뿐만 아니라, 정신장애인이 본인 ‘고유의 시선’에서 그 혼돈을 새로운 관점으로 바라보게 되었음을 뜻한다. 정신장애인은 본인의 시선을 통해 정신적 고통을 ‘알게’되고 그 ‘정체’를 규명해내면서, 현실과 혼돈을 분간하는 능력도 갖추게 된다. 때문에 더 이상 정신적 고통에 괴로워하거나 두려워하지 않아도 된다는 것을 함께 깨닫게 되고 정신적 고통을 대수롭지 않게 여길 수 있다. 또한 혼돈과 현실 간의 ‘간극’을 인식하게 되면서 본인이 현실로부터 얼마나 떨어져있는지를 직시하고, 현실이라는 목적지를 선명하게 바라볼 수 있게 된다. 그러나 정신적 고통으로부터 완전하게 자유로울 수 없는 정신장애 당사자들은 다시 혼돈에 잠식될 수 있다는 가능성을 품고 살아가야 한다. 대신, 당사자들은 혼돈 속에서도 언제든지 현실이라는 N극을 가리키는 ‘나침반’을 마련한다. 그 중에서도 ‘인지적 나침반’에는 당사자들이 스스로 정신적 고통을 알아차릴 수 있는 인지적인 단서, 정신적 고통에 매몰되지 않을 수 있는 행위, 그 고통을 견딜 수 있는 대처법 등이 해당된다. ‘정서적 나침반’은 현실을 찾아가는 여정을 옆에서 지원해주는 정서적 지지체계를 말한다. 그리고 이 두 개의 나침반을 손에 쥠으로써, 언젠가 밀려올 수도 있는 혼돈의 경계를 명확히 긋는다. 혼돈과 현실 간의 경계가 분명해지자, 연구참여자들은 혼돈 속에서도 길을 잃지 않고 언제든지 ‘내 힘으로’ 다시 현실을 찾아갈 수 있게 되었다.
본 연구의 함의는 다음과 같다. 첫째, ‘정신적 고통’을 정신의학의 자연과학적 방식이 아닌 정신장애인의 실존적 측면에서 바라봄으로써 정신건강 문제에 대한 다각적인 시각을 제공한다. 둘째, 정신의료의 권력이 의료실천 현장에서 어떻게 구현되는지를 정신장애인의 경험에 비추어 보여준다. 셋째, 지속적으로 약물이 강요되는 분위기 속에서도 정신장애인이 약물을 중단하고자하는 맥락을 보여준다. 넷째, 정신적 고통을 직접 경험하는 정신장애 당사자의 ‘전문성’ 형성 과정을 보여준다. 다섯째, 대안적 치료방식에 대한 도입가능성을 입증한다. 여섯째, 정신장애 당사자가 원하는 바는 정신적 고통의 소거만이 아니라 다시 현실을 되찾아가는 것임을 알려주며, 고통이 완전히 소거되지 않아도 현실을 되찾아가는 것에 큰 장애가 되지 않음을 보여준다.
이러한 결과와 함의에 따라 본 연구는 다음과 같이 제언한다. 첫째, 치료라는 명목에서 행해지는 강제적이고 폭력적인 의료적 처치가 정신장애인에게 ‘트라우마’를 제공할 수 있는 상황에서 이러한 의료적 관행을 ‘치료’라고 볼 수 있는지 다시 검토해보아야 하며, ‘수단’으로써 사용되어야 할 약물이 오히려 정신장애인의 삶을 좌지우지하는 주객전도의 현상을 인식하여 정신약물의 작용과 약물 실천에 대해서도 재검토해야 한다. 둘째, 정신약물은 의료의 전유물이 아니라 정신장애 당사자와 정신보건체계에 속한 인력 모두가 함께 사유하고 논할 수 있는 ‘수단’이 되어야 한다. 셋째, 정신장애인의 전문성을 인정하여 정신보건체계에 동료지원서비스를 제도화하고 정신의료 실천에 ‘치료적 파트너쉽’을 적극 도입해야 한다. 넷째, 약물 복용 여부 혹은 그 효과에 대한 자각 정도와는 상관없이, 정신적 고통에 대한 통찰과 그 고통을 대하는 당사자의 태도를 중심으로 하는 새로운 ‘병식(insight)’ 개념을 검토하여야 한다. 다섯째, 정신보건체계는 정신약물에 대한 대안적 치료방법들을 다양하게 도입 및 개발하고, 그에 따라 ‘치료’ 개념을 재검토해야 한다. 마지막으로, 본 연구는 해외의 일각에서 ‘약물 이행 담론’에 맞서 조금씩 형성되고 있는 ‘약물 대안 담론’ 속에 위치해있으며, 종합적으로 ‘지지적 환경 내에서 당사자 중심으로 이루어지는 비약물치료’를 제언하는 바이다.

The purpose of this study is to examine the context in which people with mental disabilities discontinue their medication and what they experience accordingly by looking into their real life. In other words, in the society where the discourse of medication adherence is prevalent, this study finds out the context in which the people with mental disabilities go through with mendication discontinuation and the meaning behind the experience of discontinuation. To achieve this, it adopted a qualitative research method. For data collection, research participants were selected using the snowball sampling method among those who had been given psychiatric diagnosis due to mental distress and disabilities, and who had been taking psychiatric medications for more than 6 months and discontinued for more than 2 years. Data were collected through in-depth interviews with them. The collected data were analyzed through the constant comparative method, and process and method of inductive thematic analysis.
The process of this study was to follow the trajectories of the experience of the participants over a wide range of time with them, from the occurrence of mental distress, use of medications, to discontinuation of medications and beyond. According to the theme derived, the experience of medication discontinuation is that ‘a journey to draw a boundary with the chaos that encroached on reality and to go back to reality.’ The journey consists of four stages. (1) The chaos emerges due to the mental distress and encroach on the reality; (2) the chaos becomes covered by medications, but the people with mental distress cannot cross the threshold of the reality; (3) Detaching themselves from medications and confronting to the chaos make their destination of the reality clearer; (4) with the compass found in the chaos, they draw the boundaries with the chaos and go back to their reality. The people with mental distress have had a ‘weakness’ in stressful events because of psychosocial environments that had been created around them since the old past. With the weakened mental terrain, an ‘overwhelmingly stressful event’ touches off the mental distress as a ‘trigger.’ Then they sink to chaos with the mental distress that is uncontrollable and makes them buried under the distress. This region of ‘chaos’ intrudes on the region of ‘reality’ that the people with mental distress gradually, and finally their ‘reality’ becomes encroached by the chaos. Others who stand over the people with mental distress force them to ‘eliminate’ the mental distress to suppress the chaos. This kind of coercion is divided into ‘physical suppression’ that isolate the people with mental distress from society, and ‘chemical suppression’ that attempts to control the behaviors of them by medications within the society. The psychiatry medications, which belong to chemical suppression, not only ‘overbear’ the mental distress, but also ‘overbear’ the people with mental distress themselves. As a result of using medications, they experience the effects that become insensitive to their mental distress, but their bodies, function, sense, and feeling also become insensitive. However, even after using the medications, as the symptoms remain and they often experience relapse several times, they gradually start to realize that they cannot escape from the chaos through medication treatment. The fact that medications could not get rid of the chaos, and just overbear and suppress it causes the people with mental distress to doubt about using the medications. Therefore the people with mental distress start to think about what the medications mean in their lives and what the benefits and harms of using or stopping the medication. These ‘thought’ leads to an attempt to escape from being overborne by the medications. On the other hand, escaping from the overbearing effects of medications implies that the mental distress can be increased and also that the people with mental distress can re-gain their true selves so that they reach crossroads between using or stopping their medications. At this time, they choose a way to endure the mental distress in their true selves, not ‘intoxicated with medications’, and face to the chaos with their own ‘true eyes.’ Facing to the chaos means not only ‘escaping from the suppression of medications’, but also means that they look at the chaos with a new perspective in their own ‘true eyes.’ As people with mental distress become ‘aware of’ their distress through their eyes and ‘investigate’ the identity of the distress, they develop the ability to distinguish the chaos from reality. Therefore they realize that they don’t have to be bothered or afraid of the mental distress anymore, and they feel that the distress is not significant for their lives. Also as they become realized the ‘gap’ between chaos and reality, they see how far they are from reality and clearly see the destination towards reality. However, as people with mental distress cannot be completely free from the distress, they have to live with the possibility of being encroached by the chaos again at any time. Instead, they get ‘compasses’ that always point to the N-pole that means the reality even in chaos. The ‘cognitive compass’ includes cognitive clues that allow them to recognize the mental distress on their own, actions not to be buried in the distress and alternative ways to cope with the distress. The ‘emotional compass’ refers to the emotional supporters that back up their journey to go back to their reality. With these two compasses in their hands, they clearly draw the boundaries with the chaos that may be flooded into their lives. As the boundaries between the chaos and reality become clear, they can go back to their reality ‘on their own’ at any time without getting lost amid chaos.
The implications of this study are as follows. First, it provides various perspectives on mental health issues as looking at the ‘mental distress’ not in a way of natural science of psychiatry but in an aspect of the existence of the people with mental distress. Second, it shows how the power of psychiatry is embodied in the field of mental health in light of the experience of people with mental distress. Third, in recognition of the professionalism of the people with mental distress, it is necessary to legislate for peer support services in the mental health system and actively introduce ‘therapeutic partnerships’ in mental health practices. Forth, the new concept of ‘insight’ should be reconsidered focusing on the actual insight on the mental distress and their attitude toward the distress, regardless of whether they take medications or not, and how much they perceive the effects of medication. Fifth, the mental health system should introduce and develop various alternative treatments to medication treatments, and accordingly, review the concept of ‘treatment’. Lastly, this study is located in the ‘discourse of alternatives to medications’ which is being formed gradually against ‘discourse of medication adherence.’ Overall, it suggests ‘non-medication treatment centered on the people with mental distress within supportive environments.’
The purpose of this study is to examine the context in which people with mental disabilities discontinue their medication and what they experience accordingly by looking into their real life. In other words, in the society where the discourse of medication adherence is prevalent, this study finds out the context in which the people with mental disabilities go through with mendication discontinuation and the meaning behind the experience of discontinuation. To achieve this, it adopted a qualitative research method. For data collection, research participants were selected using the snowball sampling method among those who had been given psychiatric diagnosis due to mental distress and disabilities, and who had been taking psychiatric medications for more than 6 months and discontinued for more than 2 years. Data were collected through in-depth interviews with them. The collected data were analyzed through the constant comparative method, and process and method of inductive thematic analysis.
The process of this study was to follow the trajectories of the experience of the participants over a wide range of time with them, from the occurrence of mental distress, use of medications, to discontinuation of medications and beyond. According to the theme derived, the experience of medication discontinuation is that ‘a journey to draw a boundary with the chaos that encroached on reality and to go back to reality.’ The journey consists of four stages. (1) The chaos emerges due to the mental distress and encroach on the reality; (2) the chaos becomes covered by medications, but the people with mental distress cannot cross the threshold of the reality; (3) Detaching themselves from medications and confronting to the chaos make their destination of the reality clearer; (4) with the compass found in the chaos, they draw the boundaries with the chaos and go back to their reality. The people with mental distress have had a ‘weakness’ in stressful events because of psychosocial environments that had been created around them since the old past. With the weakened mental terrain, an ‘overwhelmingly stressful event’ touches off the mental distress as a ‘trigger.’ Then they sink to chaos with the mental distress that is uncontrollable and makes them buried under the distress. This region of ‘chaos’ intrudes on the region of ‘reality’ that the people with mental distress gradually, and finally their ‘reality’ becomes encroached by the chaos. Others who stand over the people with mental distress force them to ‘eliminate’ the mental distress to suppress the chaos. This kind of coercion is divided into ‘physical suppression’ that isolate the people with mental distress from society, and ‘chemical suppression’ that attempts to control the behaviors of them by medications within the society. The psychiatry medications, which belong to chemical suppression, not only ‘overbear’ the mental distress, but also ‘overbear’ the people with mental distress themselves. As a result of using medications, they experience the effects that become insensitive to their mental distress, but their bodies, function, sense, and feeling also become insensitive. However, even after using the medications, as the symptoms remain and they often experience relapse several times, they gradually start to realize that they cannot escape from the chaos through medication treatment. The fact that medications could not get rid of the chaos, and just overbear and suppress it causes the people with mental distress to doubt about using the medications. Therefore the people with mental distress start to think about what the medications mean in their lives and what the benefits and harms of using or stopping the medication. These ‘thought’ leads to an attempt to escape from being overborne by the medications. On the other hand, escaping from the overbearing effects of medications implies that the mental distress can be increased and also that the people with mental distress can re-gain their true selves so that they reach crossroads between using or stopping their medications. At this time, they choose a way to endure the mental distress in their true selves, not ‘intoxicated with medications’, and face to the chaos with their own ‘true eyes.’ Facing to the chaos means not only ‘escaping from the suppression of medications’, but also means that they look at the chaos with a new perspective in their own ‘true eyes.’ As people with mental distress become ‘aware of’ their distress through their eyes and ‘investigate’ the identity of the distress, they develop the ability to distinguish the chaos from reality. Therefore they realize that they don’t have to be bothered or afraid of the mental distress anymore, and they feel that the distress is not significant for their lives. Also as they become realized the ‘gap’ between chaos and reality, they see how far they are from reality and clearly see the destination towards reality. However, as people with mental distress cannot be completely free from the distress, they have to live with the possibility of being encroached by the chaos again at any time. Instead, they get ‘compasses’ that always point to the N-pole that means the reality even in chaos. The ‘cognitive compass’ includes cognitive clues that allow them to recognize the mental distress on their own, actions not to be buried in the distress and alternative ways to cope with the distress. The ‘emotional compass’ refers to the emotional supporters that back up their journey to go back to their reality. With these two compasses in their hands, they clearly draw the boundaries with the chaos that may be flooded into their lives. As the boundaries between the chaos and reality become clear, they can go back to their reality ‘on their own’ at any time without getting lost amid chaos.
The implications of this study are as follows. First, it provides various perspectives on mental health issues as looking at the ‘mental distress’ not in a way of natural science of psychiatry but in an aspect of the existence of the people with mental distress. Second, it shows how the power of psychiatry is embodied in the field of mental health in light of the experience of people with mental distress. Third, it shows the context in which the people with mental distress discontinue the medications even in the environment that force them to take the medications constantly.
