The purpose of this study is to verify whether ankle instability is improved through implementing 6 weeks of exercise program treatment by comparing three groups: first, a proprioceptive exercise group using ballet movements with BOSU ball; second, a remedial exercise group; and third, a non-exercise control group. In order for the verification, the ankle instability improvement of ballet dancers was comparatively evaluated using CAIT (Cumberland Ankle Instability Tool) program. Two-way ANOVA with Repeated Measures was conducted to verify the difference among dependent variables measured at different time points (i.e. pre-exercise and post-exercise) across three groups, and a Wilcoxon signed-rank test was performed to compare the differences in pre- and post-exercise among each group. Here, the LSD (Least Significant Difference) test is used to examine the statistically significant differences in interactions. The significance level of all tests was set at ⍺=.05. The research results are as follows.
1. CAIT test result of the pre- and post-treatment among three groups illustrates that all groups showed statistically significant differences between pre- and post-exercise in the principal ankle joint (p<.05), and all groups showed improvements in the post-exercise. 2. Regarding the ankle joint position sense, there was no significant difference between the groups nor the time points (p<.05), with only minimal improvements in post-exercise scores of all groups. As a result of analyzing the difference in the ankle proprioceptive sensation for the dorsiflexion, the active joint position sense in the principal ankle was different between pre- and post-exercise (p<.05). The analysis of the difference in the ankle proprioceptive sensation for the plantarflexion indicates the difference between pre- and post-exercise scores of both active and passive joint position sense in the principal ankle (p<.01). Specifically, the ballet movement group and control group showed an increase in the post-exercise scores of active joint position sense when compared to that of pre-exercise (p<.035). In passive joint position sense, on the other hand, the remedial exercise group showed an increase in scores (p=.036). 3. In the case of the center of gravity control (COG), the overall, before and after results of 6th and 1st balance phases depicted difference in pre- and post-exercise (p<.01), and 6th balance phase of demonstrated an increase in post-exercise scores in ballet movement and remedial groups (p<.05). The 1st balance phase of COG also indicated increase in post-exercise scores in ballet movement group (p<.05). The before and after balance scores increased in the post-exercise period in ballet movement and control groups (p<.05). Again, the interior and exterior balancing scores also increased in the post-exercise period in both ballet movement and control groups (p<.05). 4. The Y-Balance test of non-principal ankle had no significant differences between pre- and post-exercises (p<.05), while the interaction effect between pre- and post-exercise among groups between pre- and post-exercise scores (p<.05). There was a statistically significant difference among groups (p<.01), and the post-hoc result illustrated a statistically significant difference between ballet movement and remedial groups (p<.05), and between ballet movement and control groups (P<.05). 5. The inversion and eversion of the ankle at axial velocity 30° of principal ankle had a statistically significant difference between pre-exercise and post-exercise scores (p<.05). In the case of the remedial group, the eversion of axial velocity 120° of the principal ankle showed an increase in post-exercise scores compared to that of pre-exercise (p<.05). The remedial group also showed an increase in post-exercise scores of both inversion and eversion ratio (p<.05). The post-exercise scores of plantar flexor at a non-principal angular velocity of 120° increased in ballet movement group (p<.05), while the post-exercise scores of plantar flexor/muscle strength ratio also increased in ballet movement and remedial groups (p<.05).
Therefore, ballet dancers with chronic ankle instability showed significant differences in joint position senses. balancing strength, isokinetic muscle movements, while the joint position sense that connotes awareness of proprioceptive sensations had a minimal increase with no significant difference. Further, the proprioceptive exercise with ballet movement was more effective than remedial exercise when it comes to balancing strength and isokinetic muscle performances. To conclude, the implementation of effective proprioceptive exercises for ballet dancers with chronic ankle instability to prevent and recover from injuries is expected to elevate the techniques of ballet dancers.
