최근 의료기관은 진료기록 증가에 따른 지속적인 용량증설, 신속한 성능개선의 어려움 그리고 정부의 의료정책 등으로 인하여 새로운 정보시스템 도입 및 유지비용이 지속적으로 증가되고 있다. 또한 급변하는 의료정책 적용과 효과적인 지표관리 등을 위해 수시로 정보시스템을 변경하고 있어 안정적인 운영과 관리에 어려움이 발생하고 있다. 이러한 의료기관의 현안뿐 아니라 최근 의료산업 분야에서 부각되고 있는 의료정보 교류사업의 활성화, 의료정보의 빅데이터 분석 및 AI를 이용한 새로운 의료정보 기술개발 등에 대한 적절한 대안으로 클라우드 정보시스템에 대한 관심과 필요성이 대두되고 있다. 본 연구는 대부분의 국내 의료기관에서 운영되는 온프레미즈 환경의 정보시스템을 클라우드 정보시스템으로 전환하는데 미치는 요인을 도출하고 실증적으로 분석한 연구이다. 이를 위해 본 연구는 우선 의료기관 정보시스템의 특징들을 살펴보고, 조직 정보기술의 수용모형인 혁신확산이론과 TOE프레임워크를 근간으로 한 조직에서의 정보시스템 도입과 관련 연구들에 대하여 문헌연구를 수행하였다. 이후 이주이론과 PPM모델을 근간이론으로 하여 의료기관이 클라우드 정보시스템으로 전환하는데 미치는 요인들에 대한 연구모형을 설정하고 이를 실증적으로 분석하였다. 본 연구모형은 PPM모델을 근간으로 밀침효과는 신뢰성 저하, 경직성, 확장성 제한 그리고 적합성 미비를 요인들로 채택하고, 당김효과는 비용절감, 서비스 전문성, 유지보수 용이성, 제도적 지원 그리고 지표관리 용이성을 채택했으며, 정박효과로는 관성, 보안우려, 매몰비용 그리고 변화에 대한 저항을 채택하였다. 그리고 이러한 요인들이 불만족과 계류 그리고 가치를 매개로 하여 의료기관의 클라우드 정보시스템 전환의도에 영향을 미치는 것으로 설정하였다. 본 연구모형에 대한 실증분석을 위해 의료기관 종사자들을 대상으로 온·오프라인 설문조사를 실시하여 총 212부의 유효한 설문지를 회수하였다. 이를 PLS 구조방정식모델링을 이용한 확인적 요인분석과 경로분석을 실시하여 실증적 분석을 수행하였다. 연구결과 경직성, 적합성 미비는 불만족에 영향을 주고, 관성, 매몰비용, 변화에 대한 저항은 계류에 영향을 주며, 비용절감, 유지보수 용이성, 지표관리 용이성은 가치에 영향을 주고, 계류와 가치는 전환의도에 영향을 주는 것으로 분석되었다. 그러나 신뢰성 저하, 확장성 제한은 불만족에 영향이 없고, 보안우려는 계류에 영향이 없으며, 서비스의 전문성, 제도적 지원은 가치에 영향이 없는 것으로 나타났다. 또한 불만족도 전환의도에 영향이 없는 것으로 나타났다. 본 연구는 클라우드 정보시스템으로 전환에 미치는 영향을 의료기관을 대상으로 실증하였다는 점과 의료기관용 클라우드 정보시스템이 본격적으로 출시를 앞두고 있는 상황에서 시의적절한 연구라는 점 그리고 클라우드 정보시스템 전환의도에 가치가 당김효과로 중요한 요인임을 밝혔다는 점 등이 본 연구의 이론적 측면의 의의라고 할 수 있다. 또한 실무적 관점에서 본 연구는 의료기관의 효과적인 클라우드 정보시스템으로의 전환을 위하여 클라우드 정보시스템의 가치를 높이는 방안을 제시하고 정박효과를 축소시키기 위한 변화관리차원의 사전교육 필요성과 강력한 리더십으로 변화를 이끌어 한다는 점을 강조하였으며, 아울러 클라우드 컴퓨팅 활성화를 위하여 정부의 보다 적극적인 제도적 지원이 필요하다는 점과 기존 정보시스템의 문제를 해결하는 클라우드 컴퓨팅의 특성 기능이 반드시 구현되어야 한다는 점 등의 실천적 방안을 제시하였다.

In recent years, medical institutions’ introduction of new information systems and their maintenance costs are steadily increasing due to continued storage capacity expansion and difficulties in fast performance improvement followed by the increase of their medical records, and medical policy changes by the government. In addition, medical institutions change their information systems frequently for the application of rapidly changing medical policies and effective indicator management, which causes a difficulty in stable operation and management of the systems. There is a growing interest and necessity for cloud information systems as an appropriate alternative not only for such current issues of medical institutions, but for activation of medical information exchange service emerging in the area of medical industry in recent years, big data analysis of medical information, technical development for new medical information with AI. This study is to draw the factors which influence on the switching of the information systems of on-premise environment being operated in most of the domestic medical institutions to cloud ones and to analyze them empirically. To this end, I examined the characteristics of the information systems of medical institutions at first and conducted a literature review on their introduction based on innovation diffusion theory and TOE framework as an acceptance model of organizational information technologies and on the study related with it. Afterwards, I set up a study model for the factors which influence on the switching to cloud information systems by medical institutions and analyzed it empirically based on migration theory and PPM model. Based on PPM model, this study model chose reliability degradation, inflexibility, limited scalability and lack of compatibility as factors of push effect, cost reduction, service expertise, serviceability, institutional support and indicator manageability as factors of pull effect, and inertia, security concerns, sunk cost and resistance to change as factors of mooring effect. Moreover, I assumed these factors influenced on the intention of switching to cloud information systems at medical institutions through dissatisfaction, moorage and value as parameters. In order to conduct the empirical analysis on this study model, a total of 212 valid questionnaires were collected by conducting an online and offline survey with workers of medical institutions. Empirical analysis on the data was conducted through confirmatory factor analysis and path analysis using PLS structural equation modeling. According to the study results, inflexibility and lack of compatibility influenced on dissatisfaction, inertia, sunk cost and resistance to change did on moorage, cost reduction, serviceability and indicator manageability did on value, and moorage and value did on the switching intention. However, it was shown that reliability degradation and limited scalability did not have an influence on dissatisfaction, security concerns did not on moorage, service expertise and institutional support did not on value. Additionally, it was shown that dissatisfaction also did not have an influence on the switching intention. It can be said that significances of this study in its theoretical aspect are that it has demonstrated the influence on the switching to cloud information systems in medical institutions and it is a timely study in a situation where cloud information systems for medical institutions are about to be released in earnest, and value is an important factor of pull effect in the switching intention of cloud information systems. In addition, this study highlights from a business perspective that it proposes how to increase the value of cloud information systems for its effective switching in medical institutions and emphasizes that it is necessary to take lead in change through strong leadership and the necessity of prior education in change management level to decrease mooring effect and also proposes a practical plan including more aggressive institutional support by the government for the activation of cloud information systems and solid implementation of characteristics and functions in cloud computing which solves the problems of the existing information systems.
