This study is to evaluate the clinical efficacy of contact lens coated with PVP on the surface of the hydrogel lens for the purpose of maintaining wettability, improving fit and visual acuity. Twenty-one people (42 eyes) in their 20's with normal tears were treated with two hydrogel lenses with the same parameters except PVP coating. And 15 minutes, 8 hours and one month after wearing the lenses, non-invasive tear breakdown time (NIBUT) and degree of hyperemia were evaluated. Contrast sensitivity was measured under the condition of photopic vision and mesopic vision, and was observed for hyperemia of bulbar and limbal. OSDI questionnaires were used at weekly intervals to evaluate subjective symptoms, and CLDEQ-8 surveys were conducted at two-week intervals. Also, assessed daily changes by extracting from OSDI questionnaires such as 'glare', 'gritty', 'pain', 'blur', and 'poor vision' that correspond to subjective symptoms. After wearing the lens in a daily wear cycle for 4 weeks, the lens was returned and the parameters measured. The lens parameters were measured for power, total diameter, base curve, center thickness, water content, visible light and UV-A, B transmissibility. The difference between uncoated and PVP-coated lenses after wearing for 4 weeks was compared with based on the new lens. The amount of protein attached was confirmed by protein quantification, and the surface change of the lens was confirmed by scanning electron microscopy. Experimental results showed that the contrast sensitivity decreased significantly as the wearing time increased in both the photopic vision and mesopic vision compared to before wearing, but no difference was observed between the uncoated lens group and PVP-coated lens group. The grading scale for bulbar and limbal increased with the increase of lens wearing period. Especially, the uncoated lens showed a statistically significant increase according to the wearing period. NIBUT showed a significant decrease according to the increase in wearing time compared to before wearing, and after one month of wearing, there was a statistically significant difference depending on the presence or absence of coating. Subjective symptoms of the OSDI questionnaire showed a significant increase in the uncoated lens and PVP-coated lens with the time of wearing, but there was no significant increase between 3 and 4 weeks of wearing the coated lens. In the CLDEQ-8 survey, discomfort tended to be the higher while wearing the uncoated lens in the 2nd and 4th-week, but there was no statistical significance in the wearing time and between the lens group. In the analysis of the 5 subjective symptoms in the OSDI questionnaire, discomfort tended to increase regardless of coating, but when the uncoated lens was worn, scores of all symptoms except 'poor vision' tended to higher compared to PVP-coated lens, but there was no significant difference between the lenses. There was a significant difference in all parameters except power and water content between the new and the worn in the uncoated lens, while the PVP-coated lens showed differences only a few parameters in base curve, visible light and UV-A, B transmissibility. That is, the base curve of the lens was increased compared to the new lens (8.6mm), but the diameter did not show a significant difference compared to the new lens (14.2mm). In contrast, the center thickness decreased and the difference between the uncoated lens group and PVP-coated group was statistically significant. Visible light and transmissibility decreased and UV-A transmissibility increased, while UV-B transmissibility decreased compared to the new lens (5%). In addition, the amount of protein adhesion was relatively higher than the uncoated lens compared to the PVP-coated lens, but the difference was not so large that there was no significant difference. In the case of lens surface observation by scanning electron microscopy, both lenses showed no significant difference before wearing, whereas surface cracking was observed after wearing and coating lens was more cracked. In this study, but the tear film stability was confirmed in comparison with the uncoated lens, even if the wear period of the water-soluble coating hydrogel lens was increased, in the normal tear people. The questionnaire analysis showed both lenses showed that discomfort increased with the wearing time, that the degree of discomfort confirmed by the uncoated lens was larger than that of the PVP-coated lens. It was found to be clinically effective in subjective symptoms. In addition, even if the PVP-coated lens is worn every day for 4 weeks, it was found that the change compared to the uncoated lens on the same parameter is less. Therefore, wearing the PVP-coated lens may help to improve wearability by maintaining the tear film stability of the wearer, and may help in determining the wearing cycle during the daily wearing of the PVP-coated lens.
