Purpose: Alopecia is a common medical problem affecting both males and females. Dermal papilla (DP) cells and Outer root sheath (ORS) cells are the ultimate reservoir of cells with the potential of hair regeneration in alopecia patients. Defect in any one of cells can lead to hair loss. Biovesicle are nanosized vesicles consistent of extracellular vesicles (EVs) and extracellular vesicles mimetics (EMs). EVs are naturally releasing vesicles and EMs are generated vesicles. Here, the role of mesenchymal stem cell (MSC), macrophage (MAC) and fibroblast (FB) derived EVs and macrophage generated EMs were analyzed in promoting hair growth.
Materials and Methods The EVs were isolated from the cell culture supernatant and EMs were generated from the cells. The characterization was performed by transmission electron microscope, nanoparticle tracking analysis and western blotting (WB). The WB and flowcytometry was performed the find the cargos of EVs. The proliferation, migration and the expression of growth factors and hair follicle inducer proteins and genes were studied in human-DP cells in the presence or absence of EVs and EMs. The retention of EVs and EMs were performed in the mice by in vivo fluorescent imaging. The effect of MSC-EVs, MAC-EVs and MAC-EMs treatment were tested on hair regeneration in a mouse model. Additionally, the effect of MAC-EVs, FB-EVs and MAC-EMs were tested on human derived hair follicles.
Results: MSC-EV and MAC-EV treatment increased DP cell proliferation and migration, and elevated the levels of Bcl-2, phosphorylated Akt and phosphorylated ERK. The DP cells treated with MSC-EVs, MAC-EVs, MAC-EMs showed increased expression of growth factors. MAC-EVs, FB-EVs and MAC-EMs increased the expression of β-catenin related transcription factors in cultured DP cells. The results suggest that Wnt proteins in MAC-EVs, human FB-EVs and MAC-EMs activate the Wnt/ β-catenin signaling pathways, which leads to activation of transcription factors (Axin2 and Lef1). MSC-EVs, MAC-EVs, MAC-EMs increased the phosphorylated AKT, phosphorylated ERK, Bcl-2 and PCNA. FB-EVs promoted the migration, proliferation, and differentiation of outer root sheath cells. FB-EVs elevated differentiation markers in ORS cells. In vivo fluorescent imaging of MSC-EVs, MAC-EVs and MAC-EMs showed that they were stable in mice skin for more than 72 hours. Intradermal injection of MSC-EVs, MAC-EVs and MAC-EMs into C57BL/6 or Balb/C mice promoted the hair regeneration in mice. Finally, the MAC-EVs, FB-EVs and MAC-EMs promoted the human hair shaft elongation.
Conclusions: The current study results suggest that MSC-EVs, MAC-EVs, FB-EVs and MAC-EMs have the potential to activate DP or ORS cells, and promote hair growth. These findings revealed a novel mechanism by which EVs and EMs influenced the hair growth and highlighted the importance of recognizing their unique properties. These findings suggest that EVs and EMs treatment could be clinically used as a promising novel treatment of alopecia.
