Purpose: To propose the baseline data necessary for the establishment of tuberculosis prevention and control policies through observing the regional differences and influencing factors on the tuberculosis relapse related indices (TRRI).
Method: An ecological study conducted around the tuberculosis relapse rate and the related indices collected using the secondary data sources. Subjects of this study were the nation–wide 241 regions excluding five cities and provinces which missed the corresponding data. Data sources were the local governments’ statistical yearbooks, the 2016 Community Health Survey, the local government statistics and the National Statistical Office statistics. The collected data were sociodemographic factors, health behaviors, socioeconomic factors and the two public-private mix (PPM) indices. Data analysis performed using SPSS (ver. 25.0). The differences and associations of the TRRI were analyzed with correlation analysis and multiple linear regression analysis.
Results: The TRRI showed big difference across the regions, with the maximum gap as 45 persons per 100,000 people between the lowest and the highest area. As result of correlation analysis, the TRRI was significantly higher when higher regional deprivation index (RDI) (r=0.478), higher rate of single-person household (r=0.442), the aged (r=0.436), living alone elderly (r=0.418), poverty (r=0.346), current smoking (r=0.326) and high-risk drinking (r=0.278), and in contrast, it was significantly lower when higher rate of social welfare budget (r=-0.378), population density (r=-0.219) and health screening (r=-0.185). As result of the regression analysis, the factors significantly affecting the TRRI were the weight of social welfare budget (β=-0.297), RDI (β=0.272) and current smoking rate (CSR) (β=0.215). And by the region, BCG vaccination (β=-0.384) and CSR (β=0.337) in the metropolitan cities, and CSR (β=0.243) and CSR (β=0.307) in the cities and the counties, respectively significantly affecting to the TRRI.
Conclusion: I observed the TRRI and related factors showed the regional disparities, and these factors also affect to the TRRI. Therefore, when establish the new era tuberculosis control policy, it would be necessary to prioritize the differentiated and customized strategy through considering not only the currently used biomedical interventions for the individual patients but also the vulnerabilities and socioeconomic determinants of the region.
