Phase-contrast X-ray imaging facilitates spatial resolution at the microscopic level and allows the observation of multiple structures. Unlike conventional mechanistic methods, sample tissue can be analyzed in a non-destructive way. Additionally, it is possible to irradiate extremely small objects in the order of tens of nanometers and to simultaneously observe the real surface and internal microstructure, enabling real-time image analysis. It is a very useful imaging technique that can visualize and distinguish internal microstructures and enable three-dimensional (3D) analysis. This study included eight female ICR mouse strains. Four randomly selected mice underwent ovariectomy (OVX group) that can lead to osteoporosis. Sham procedure (SHAM group) does not performed in the remaining four mice resection of the ovary. After 6 weeks, all mice were sacrificed to obtain the left and right femur bones. The number of femur samples was as follows: OVX group (n = 8) and SHAM group (n = 8). Thereafter, we performed analysis using hematoxylin and eosin (H&E) staining, micro-computed tomography, and phase-contrast radiography. Cross-sectional structures in the sample that underwent H&E staining were obtained. However, there were limitations in this technique that included a risk of damage to the bone, which became hard tissue during slide formation and obtaining a two-dimensional tomographic image at a limited position. The limitations of the staining method can be overcome by a non-destructive method using micro-computed tomography. The latter was also insufficient for the accurate diagnosis of osteoporosis due to the weak spatial resolution and difficulty in identifying the microdamage mechanism. Due to phase-contrast X-ray imaging used in this study, we were able to observe more blood vessels and spongy bones. In addition, we were able to diagnose spongy bone microvariation and analyze the geometric properties of the bone. Through this technique, it was possible to obtain important medical images that could make objective 3D diagnosis, and quantitative pathological mechanism of osteoporosis and the framework for medical rehabilitation. Bone Volume (BV) measured on performing micro-computed tomography was significantly different between the OVX and SHAM groups (p<0.05). This is an important factor as it can cause secondary complications. As BV decreases, the probability of fracture increases. In this study, only BV, which is the most important aspect for osteoporosis analysis, was analyzed for the quantitative three-dimensional diagnosis of spongy bone microvariation. The results of the phase-contrast X-ray analysis proved that there was a significant difference between the OVX and SHAM groups (p0.05). Moreover, It was not significant in SHAM (p>0.05). In previous studies, quantitative osteoporosis analysis was performed by phase-contrast X-ray imaging. Through this study, a new osteoporosis imaging analysis method was constructed, which can complement the limitations of existing medical imaging methods. Via improvements in medical technology, it is possible to develop 3D medical images of blood vessels and spongy bones effectively by expanding and developing an artificial intelligence diagnosis system for musculoskeletal diseases. These techniques can be useful for accurate evaluation through the establishment of an evidence-based automated diagnostic system. Even in the absence of basic research on bio-materials, it is possible to create an objective pathological mechanism and a framework for therapeutic foundation, which can prove to be sufficiently competitive. Therefore, by complementing the limitations of previous medical imaging methods, it is possible to apply phase-contrast X-ray imaging to perform qualitative image analysis of spongy bone microvariation.
