The Relationship between Injury Mechanism, Trauma Site and Hospitalization of Elderly Trauma Patients Visited through Emergency Room원문보기

Objectives: The purpose of this study is to examine associated factors affecting length of hospitalization based on the injury mechanisms and trauma sites in elderly trauma patient. And these analyses may help to establish the prevention planning to reduce elderly trauma patients.
Methods: This study was conducted based on NEDIS data. Through NEDIS data analysis, 55,837 trauma patients aged 65 or older who visited emergency rooms nationwide for one year in 2017 were investigated. Data such as general characteristics of elderly trauma patients, a situation in which trauma occurs, injury mechanism and trauma site were analyzed to study the association between damage caused by trauma among elderly people and the length of hospitalization. The criteria for selecting the target are patients who have been diagnosed with one or more S codes among main diagnosis or additional diagnosis using the Korean Standard Classification of Diseases. SPSS 25.0 and SAS 9.4 were used for the analysis.
Results: Elderly trauma patients have a higher number of female patients than males, and the proportion of female patients has increased with age. The season when elderly trauma occurs the most is autumn, and the time of occurrence was the highest from 12:00 to 18:00 hrs. The distribution differences between genders and age groups over seasons and time of occurrence were statistically significant. The most of elderly trauma patients has a health insurance rather than other insurances or medical benefits, and most patients were transferred from the accident site to emergency room directly. The frequency of hospital visits through 119 ambulance was the highest among all the visitors. Injury mechanism was shown in the order of slip down, traffic accidents and falls. In the area of Injury, the head was most damaged between the ages of 65 and 84, while those aged 85 and older suffered the most damage to their hips and thighs. There were more single-damaged patients than multiple-damaged patients. When the elderly trauma patients visited the emergency room, they were evaluated. Most patients showed the level of consciousness as alert with the range of systolic blood pressure from 90 to 140 mmHg, heart rate from 60 to 100 times per minute, and respiratory rate from 12 to 20 times. The largest number of patients were classified as the Level 3 patients. Most of the initial hospitalization sites were admitted to general hospital rooms, and as the age increased, the rate of hospitalization in general hospital room was higher than that in intensive care units. The average length of hospitalization among all elderly trauma patients was 19.07 days. In-patients with a single part injury had the longest hospitalization period when the trauma was attributed to slipping, damaged hips and thighs, or when the severity was Level 2. In the case of short-term hospitalization among multiple injuries, the patients who were damaged to hips or thighs showed longer hospitalization and those who were damaged other reason also showed longer hospitalization. And the patients who showed level 1 damage showed shortest hospitalization period. In the case of long-term hospitalization involving multiple injuries, the length of hospitalization was the longest in patients who were hospitalized due to traffic accidents or who suffered damage, including areas of knees and lower legs. In addition, the hospitalization period in patients with Level 1 trauma was longer than in other level.
Conclusion: Age, gender, insurance type, injury mechanism, mental, SBP, HR, severity, and trauma site affect the hospitalization period. These results suggested important factors associated with long hospitalization of elderly patients caused by trauma and are intended to contribute to develop the prevention methods to reduce the number of elderly trauma patients.
