연구 방법 : 연구 대상자는 2017년 근로환경조사 대상자 50,205명 중 결측값을 제외한 주당 30시간 이상 근무하는 임금근로자 34,316명이었다. 불편한 자세, 사람을 들거나 이동시키는 작업, 중량물 취급 작업, 오래 서있기, 손과 팔의 반복적 작업 총 5가지의 인간공학적 위험요인과 장시간 노동(주당 52시간 초과노동)을 노출변수로 허리, 상지, 하지의 통증 여부를 종속변수로 하여 다중 로지스틱 회귀분석을 수행하였다. 장시간 노동과 인간공학적 위험요인의 동시노출이 근골격계 증상 유병률에 미치는 영향을 조사하기 위해 교호작용을 분석하였다. RERI(Relative Excess risk due to interaction)는 덧셈 교호작용, Ratio of ORs(Odd ratios)는 곱셈 교호작용 분석을 위해 계산하였다.
연구 결과 : 근골격계 증상 유병률에 대한 장시간 노동의 오즈비는 1.51(95%CI: 1.36-1.67), 인간공학적 위험요인의 오즈비는 3.37(95%CI: 2.99-3.80)이었다. 장시간 노동과 인간공학적 위험요인의 교호작용분석에서 RERI 0.82(95%CI: 0.11-1.53), Ratio of ORs은 0.83(95%CI: 0.50-1.14)이었다.
연구 결론 : 본 연구에서 장시간 노동과 인간공학적 위험요인이 독립적으로 근골격계 증상 유병률을 높이는 결과를 확인하였다. 두 요인이 동시에 노출될 경우 각각 노출 될 때보다 근골격계 증상 유병률에 상승효과가 있음을 확인하였다. 근골격계 증상을 관리하기 위해서 장시간 노동에 대한 법적 제한 및 인간공학적 위험요인 개선 프로그램의 도입이 필요하다.

METHODS : This study analyzed 34,316 wage workers from the 5th Korean working condition survey (KWCS). The effects of long working hours and 5 ergonomic risk factors (awkward posture, moving or lifting people, handle heavy objects, prolonged standing, repetitive movement) on musculoskeletal symptoms (pain of low back, upper limb and lower limb) were evaluated by using survey-weighted multiple logistic regression analysis. To assess the combined effect of ergonomic risk factors and long working hours on musculoskeletal symptoms, the RERI (Relative Excess risk due to interaction) and Ratio of ORs (Odds Ratios) were calculated by using the survey-weighted multiple logistic regression analysis and post-estimation commands.
RESULTS : The workers exposed to long working hours and ergonomic risk factors showed ORs of 1.51 (95% CI: 1.36-1.67) and 3.37 (95% CI: 2.99-3.80), respectively, on musculoskeletal symptoms. The RERI was 0.82 (95% CI: 0.11 to 1.53) and the ratio of ORs was 0.83 (95% CI: 0.50 to 1.14).
CONCLUSION : Long working hours and ergonomic risk factors were associated with musculoskeletal symptoms among Korean wage-workers. Also, there was a synergism between long working hours and ergonomic risk factors on musculoskeletal symptoms. Hence, a strict regulation on the working hours and ergonomic intervention program could reduce the prevalence of musculoskeletal symptoms.
