결과: 치매와 MCI 환자를 대상으로 병합중재가 인지기능에 미치는 효과를 확인한 국내·외 총 5047편의 문헌이 검색되었으며 최종적으로 22편의 문헌이 선정되었다. 무처치 및 플라시보군(n=18)에 비교하여 분석한 결과 병합중재가 치매와 MCI 환자의 인지기능에 미치는 효과 크기는 0.49(95% CI: 0.25~0.73)로 Cohen(1988)이 제시한 기준에 따라 중간 효과 크기를 의미하였고, 이는 병합중재가 치매와 MCI 환자의 인지기능 개선에 효과적이라는 것을 알 수 있었다. 조절효과분석 결과 중재 기간이 길수록 효과 크기가 통계적으로 유의하게 감소하는 것을 확인하였다. 이 외 연령, 진단명, 중재 횟수, 중재 수행순서, 중재 접근방식에 따라 통계적으로 유의한 차이는 없었다. 또한, 병합중재군을 신체활동군(n=4)과 비교 분석한 결과 효과크기는 0.09(95% CI: -0.19~0.36), 인지중재군(n=5)과 비교하여 분석한 결과 효과크기는 0.25(95% CI: -0.38~0.88)로 통계적으로 유의한 효과크기의 차이가 없어 병합중재가 단일중재와 동일한 효과를 나타냄을 확인하였다.

Pharmacological and non-pharmacological approaches have been used to prevent and treat dementia and MCI. Since pharmacological approaches are expensive and have limited treatment and other adverse effects, there has been an increasing attention on non-pharmacological interventions. This study meta-analyzed the domestic and international literature on the effects of a combined intervention on the cognitive function of patients with dementia or mild cognitive impairment, verified the effects of a combined intervention, and confirmed the additional effects of combined intervention by a comparison between physical activity and cognitive intervention.
Method: Without setting a limitation on the year, 5 electronic databases(RISS, KISS, NDSL, Ovid MEDLINE, and Ovid EMBASE) were searched for randomized controlled trials published before September 2020 that investigated the effects of combined intervention on cognitive function in patients with dementia and MCI. Additional data were collected by searching existing literature reviews on the relevant topic and references found in the primary search. R—version 3.6.3—was used for data analysis, and Risk of Bias (RoB) Summary in RevMan—version 5.4—was used to assess the risk of bias.
Result: A total of 5047 domestic and international studies on the effects of combined intervention on cognitive function in patients with dementia and MCI were searched, and 22 studies were selected for this study. A comparison with the non-treatment and placebo group (n=18) showed that the effect size of combined intervention on the cognitive function of patients with dementia and MCI was 0.49(95% CI: 0.25~0.73), which represented a medium level according to Cohen (1988). This showed that combined intervention is effective in improving the cognitive function of patients with dementia and MCI. A moderation effect analysis showed that the effect size significantly decreased with a longer intervention period. No statistically significant difference was found on the factors of age, diagnosis, intervention setting, mode of combination, total sessions. In addition, when comparing the combined intervention group with the physical activity group (n=4) and the cognitive intervention group (n=5), the effect size was 0.09 (95% CI: -0.19~0.36) and 0.25 (95% CI: -0.38~0.88), respectively. No statistically significant difference was found, confirming that the combined intervention and single intervention were equally effective.
Conclusion: The results of this study confirmed that the combined intervention is as effective as the single intervention in improving the cognitive function of patients with dementia and MCI, but had no additional effects. However, considering the additional benefits of combined intervention for issues other than cognitive impairment in patients with dementia and MCI, it is necessary to design and perform them appropriately. As the importance of non-pharmacological intervention for these patients is rising, further multi-arm randomized controlled trials based on this study are needed, as well as a subsequent meta-analysis that integrates them.
