A microneedle(MN) patch can more efficiently deliver therapeutic agents over the stratum corneum, which is a key component of the skin barrier, than conventional transdermal drug delivery. Recently, many studies are trying to use MN patch in various purpose. Two prior studies have suggested that biodegradable MN patches are useful for managing prurigo nodularis and psoriatic plaques. However, clinical trials of MN patches for atopic dermatitis(AD) have not been reported. Thus, we aimed to evaluate the efficacy of a biodegradable hyaluronic acid MN(BHMN) patch combined with topical medication for the treatment of dry skin in AD patients. In this single-center, investigator-initiated, assessor-blinded, split-body, randomized controlled parallel trial, patients with dry skin and AD diagnosed according to the Hanifin and Rajka criteria were included. Twenty participants were recruited, and two experimental skin lesions were chosen from each participant. The participants applied an aroma cream to the both lesions and attached a BHMN patch on the randomly allocated skin lesion daily for 2 weeks(BHMN patch group). On the other lesion, only topical medication was applied for 2 weeks(control group). After 2 weeks of the trial, intragroup and intergroup comparisons were conducted. As the result, significant improvement was noted in the local scoring AD(L-SCORAD) index, which was the primary outcome of this study. Furthermore, the BHMN patch group significantly improved some secondary outcomes, including visual analog scale(VAS) scores for skin dryness and skin hydration. Other secondary outcomes including Investigator’s Global Assessment scale scores, VAS scores for pruritus, and transepidermal water loss also significantly decreased after 2 weeks in both groups. However, superiority of the BHMN patch group over the control group was not noted. The BHMN patch improved the L-SCORAD index, VAS scores for skin dryness, and skin hydration by facilitating topical medication absorption. In addition, the moisturizing effect of hyaluronic acid MNs may also have contributed to the aforementioned results. The absorption-facilitating activity of the BHMN patch may be resulted from penetrating the skin barrier. Moreover, hyaluronic acid in the MN may resulted in moisturizing effect of the BHMN patch. Clinically, these results suggest that the BHMN patch can be used with topical medication for the treatment of dry skin in AD patients.
