It is known that more than 50% of the world's population is infected with gastritis, and more than 80% are infected in Korea. This is because early detection is difficult because there are no or insignificant initial symptoms. However, if detected early, it can be recovered with simple treatment. Periodic medical checkups are required to diagnose gastritis early. Currently, the most accurate test method for diagnosing gastritis is histological test through gastroendoscopy. There are various types of biopsy methods, but this study attempted to examine in detail Hematoxylin and eosin (H&E) stain, which is the basic staining of all biopsy. H&E stain, the most widely used basic staining to identify tissue pathological findings, has two methods: progressive staining and regressive staining. Progressive staining is a method of gradually staining the components of the tissue in a predetermined order, while regressive staining is performed by overstaining the entire tissue structure to unwanted parts and then removing unnecessary parts through a bleaching process. Until now, most medical institutions have customarily used regressive staining using Harris hematoxylin. However, some institutions have begun to use progressive staining through equipment using Mayer hematoxylin. However, there is no direct comparison of these two staining methods yet, so the usefulness is not well known. Therefore, by comparing these two representative techniques of H&E stain, this study was conducted to understand the advantages of staining using Mayer hematoxylin currently used. The study was conducted using samples of 83 patients who underwent endoscopic biopsy to evaluate gastritis after gastro endoscopy at Seoul National University Hospital. All staining methods were performed using gastrointestinal mucosal samples. A total of three methods were implemented by adding Mayer H&E stain currently being implemented by this institution, Harris H&E stain, and Mayer H&E stain, which increased Mayer hematoxlin time to five minutes. In addition, Wright-Giemsa stain, a test method with high sensitivity and sensitivity in H. pylori detection, was performed, and the value was used as the distinction standard. On each slide where the staining was performed, two pathologists read H. pylori, nucleus, capillary, neutrophils, and internal metaplasia. H. pylori compared and analyzed the results of W-G stain and each Harris H&E stain, Mayer H&E stain, and 5' Mayer H&E stain. The results were analyzed by dividing them into four stages: negative, mild, moderate, and severe or positive and negative. As a result of reading by both readers, both showed the highest detection rate in 5' Mayer H&E staining. In addition, when comparing the reading results between the two readers, 5' Mayer H&E stain showed the highest matching rate in the H. pylori detection rate. In addition, for comparative evaluation of the stainability of gastro endoscopy tissue, stainability was evaluated for nucleus, capillary, neutrophils, and intestinal metaplasia, respectively. The most significant results were found in 5' Mayer H&E in nucleus, capillary, and neutrophils. As a result, among the three methods, 5' Mayer H&E stain showed excellent results in gastritis evaluation, including H. pylori detection during relatively short test time and reading. Based on these results, it is judged that it would be helpful to use Mayer he staining as a basic histological staining.
