Background and Objectives Type 2 diabetes mellitus causes various complications, such as cardiovascular complications and diabetic nephropathy, and its prevalence is increasing. Glucagon-like peptide-1 (GLP-1) is an incretin hormone that acts on the pancreas to increase insulin and decrease glucagon secretion. The serum GLP-1 concentration is decreased in patients with type 2 diabetes. This study aimed to confirm the relationship between serum GLP-1 levels and various metabolic indicators in patients with type 2 diabetes mellitus and investigate the association between serum GLP-1 levels and albuminuria in patients with type 2 diabetes mellitus. Methods This was a retrospective single-center cross-sectional study. This study was conducted on outpatients and inpatients with type 2 diabetes mellitus at Wonkwang University Hospital, and blood tests, physical measurements, and vital signs taking were conducted in 68 patients. Logistic regression analysis was performed according to the serum GLP-1 level, with the presence or absence of moderately increased albuminuria as a dependent variable. Results Of 68 patients, those with moderate or severe albuminuria had significantly higher serum GLP-1 levels, body mass index, and waist circumference (p=0.040, p=0.018, and p=0.023, respectively). The glomerular filtration rate (GFR) was higher in patients with moderate or severe albuminuria, but the difference was not statistically significant (p=0.104), and serum creatinine level was significantly decreased (p=0.039). According to the serum GLP-1 levels, the lower third was assigned to the high-risk group (Group H), and the remaining patients were assigned to the low-risk group (Group N). Group H showed a significantly lower GFR (p=0.022). The urine albumin-to-creatinine ratio was higher in Group H (p=0.002). Conclusions In patients with type2 diabetes mellitus, the higher the serum GLP-1 level, the lower the incidence of moderate to severe albuminuria, which was statistically related.
