Purpose: This study aims to identify the influential factors of nursing home nurses that impact fall prevention behaviors, thereby providing basic data to prepare effective measures that can increase the fall prevention behaviors of nurses.
Method: This study, which is a descriptive survey study, recruited a total of 136 nurses from 4 nursing homes having more than 200 beds located in B metropolitan city who understand the study objectives and signed the written consent form to participate. The relevant data were collected through the self-report questionnaires from August 2 to August 31, 2021. The tools used include perception of patient safety culture, fall prevention behavior health beliefs, and accidental fall prevention behavior tools. The collected data were analyzed using the SPSS 27.0 program in the mean and standard deviation, independent t-test, one-way ANOVA, Pearson's Correlation Coefficient, and Multiple Regression.
Result: The results of this study are summarized as follows: 1. For the general characteristics of the participants, in the case of gender, 91.9% (125 nurses) accounted for females and 8.1% (11 nurses) for males. The average age of the participants was 46.41±13.07 years old, 31.6% (43 nurses) accounting for 55 or older, 24.3% (33 nurses) accounting for under the age of 35, 23.5% (32 nurses) accounting for the age between 45∼55, and 20.6% (28 nurses) accounting for the age between 35∼45. In addition, 69.1% (94 nurses) and 30.9% (42 nurses) accounted for the cases with or without spouses, respectively. The average working career of participants in the nursing homes was 6.92±5.02 years and the over 9 years of career accounted for 31.6% (43 nurses), which is the highest. It was revealed that 95.6% (130 nurses) had experience in patient safety education, 81.6% (111 nurses) had experience in accidental falls, and 98.5% (134 nurses) exhibited experience in fall prevention behaviors.

Regarding the scores for the sub-factors, the highest was the degree of falls prevention behaviors with 4.58±0.50 on average out of a 5-point scale, and then followed by the perceived benefits with 4.10±0.55, the perceived seriousness with 3.99±0.65, the motivation of action with 3.91±0.50, the perceived sensitivity with 3.60±0.57, and the perceived obstructive with 2.62±0.64, respectively.
3. No variable was entailed due to the difference in perception of patient safety culture according to the general characteristics of the participants. For the fall prevention behavior health beliefs, a significant difference was exhibited in the presence or absence of a spouse (t=-2.70, p=.008), the experience in patient safety education (t=1.99, p=.049), and the experience in accidental falls (t=2.80, p=.006). For the accidental fall prevention behavior, a significant difference was exhibited in the age (F=8.04, p<.001) and the presence or absence of a spouse (t=-2.91, p=.005).

In addition, the perceived seriousness, the perceived obstructive, and the motivation of action among the sub-domains of fall prevention behavior health beliefs were revealed to be the significant influential factors. Therefore, it is necessary not only to recognize the importance of fall prevention behavior health beliefs by highly reminding of the seriousness of the prevalence and the mortality due to accidental falls but also to devise education programs related to accidental fall prevention for decreasing the perceived obstructive. Moreover, the accidental falls should be prevented by increasing the opportunity for actions that can reduce the risk of occurring the accidental falls. Furthermore, measures to improve fall prevention behaviors should be sought through continuous education on fall prevention for nurses under the age of 55, which is one of the general characteristics. This study has significance in that the results helped identify the influential factors on the fall prevention behaviors of nursing home nurses, thereby providing fundamental data for effective fall prevention behaviors.
