본 연구는 상급종합병원 5개 중환자실의 중심정맥관 관련 혈류감염 위험요인을 확인하기 위한 후향적 사례-대조군 연구이다. 1) CLABSI군과 비 CLABSI군의 특성 비교시 재원일수(p<.001), 만성신부전(p=.007), APACHEⅡ 점수(p<.001), 입실 경로(p<.001)가 통계적으로 유의미하였다. 다제내성균 집락화로는 MRSA (p=.002), MRAB (p<.001), VRE (p<.001), CRE (p<.001), 미집락(p<.001)인 경우 차이를 나타냈으며 항생제 및 투약의 종류로는 Carbapenem계(p<.001), Glycopeptide계(p=.002), 그 외 다른 항생제를 사용한 경우(p=.003) 차이를 확인할 수 있었다. 침습적 기구와 행위에서는 수술, 인공호흡기 적용, 기관내 삽관 튜브, 비위관, 투석 도관(p<.001), 다른 침습적 기구 삽입(p=.016)시에도 유의한 차이가 있었다. 그 외 삽입 부위(p=.001), 삽입 장소(p<.001)가 유의한 차이를 보였다. 병원체 요인에서도 중환자실 종류(p=.038)가 차이를 보였으며 환경요인에서는 3-way hub 조작 횟수(p=.007), MBP 수행(p=.040), 다제내성균 밀도(p=.017), 다제내성균 압력도(p=.005), 간호 인력(p=.002)이 유의미한 차이를 보였다. 2) 중심정맥관 관련 혈류감염 원인 균주로는 Gram positive spp. 41.9%, Gram- negative spp. 38.5%, Fungus가 19.7%의 비율을 보였다.
3) Cox proportional hazards model을 이용하여 단변량 분석시 유의미하게 확인(p<.05)된 변수들을 다변량 분석하였다. 재원일수는 1일 증가할 때 마다 위험도가 감소(HR=0.95; 95% CI=0.93–0.97)하였고 APACHEⅡ 점수의 경우 1점 증가할 때 마다 1.04배 위험도가 증가(HR=1.04; 95% CI=1.00–1.07)하였다. 입실 경로는 중환자실 간의 전실시 위험도가 증가(HR=2.26; 95% CI=1.03–4.96)하였으며 다제내성균이 하나 이상 집락시 미집락할 경우보다 2.43배 위험도가 증가(HR=2.43; 95% CI=1.40–4.21)하였다. 삽입 부위로는 대퇴 부위(HR=1.90; 95% CI=1.13-3.22), 팔오금 부위(HR=7.15; 95% CI=1.40–36.50)에 삽입시 위험도가 상승하였으며 삽입 장소로는 수술실에서 삽입한 경우 제일 안전한 것으로 확인되었다(HR=0.12; 95% CI=0.03–0.58). 환경요인 중 3-way hub 조작 횟수는 1회 증가시 마다 위험도는 1.03배 증가하였다(HR=1.03; 95% CI=1.01–1.04).
본 연구를 통해 CLABSI 발생에는 삼각모형에서 확인된 환자, 병원체, 환경요인을 통해서 나타남을 확인하였다. 이는 국내/외 대표적인 CLABSI 위험사정도구가 마련되지 않은 상황에 기초자료로 활용될 수 있으며, 생존분석을 통해 조기에 CLABSI 발생을 감시할 수 있는 이론적 근거를 제시하였다는 점에서 의의가 있다. 특히 환경요인 중에서 감염발생의 주요 위험요인으로 확인된 결과를 통해서 간호실무의 감염관리 교육과 지침 수립에 효과적인 방향을 제시하였다는 점에서 의의가 있다.

This study is a retrospective case-control study to identify risk factors for central line-associated bloodstream infection in five intensive care units of a tertiary care hospital. The subject of research were patients who entered the adult intensive care unit of a tertiary hospital with 860 beds in Korea for 24 months from 2018.02 to 2020.01, and risk factors were identified by dividing 101 case groups and 303 control groups into patients, pathogens, and environmental factors. Data were collected through identified risk factors confirmed from expert feasibility study, and the data analysis was conducted using the R version 4.2.3, Web-based Analysis with R 4.2 version, IBM® SPSS® Statistics GradPack 28 for windows statistics program, and the Wilcoxon rank-sum test, chi-square, and cox proportional hazards model. The results of this study are summarized as follows.
1) When comparing the characteristics of the CLABSI group and the non-CLABSI group, hospital length of days (p<.001), chronic kidney disease (p=.007), APACHEII score (p<.001) and route of admission (p<.001) were statistically significant. As multidrug-resistant organisms colonies, MRSA (p=.002) and MRAB (p<.001), VRE (p<.001), CRE (p<.001), no colonization (p<.001) showed differences, and the types of antibiotics and medication were carbapenems (p<.001), glycopeptides (p=.002), and other antibiotics (p=.003) I could see the difference. In terms of invasive device and acts, there were also significant differences in surgery, ventilator, endotracheal tube, levin tube, hemodialysis catheter (p<.001), and insertion of other invasive device (p=.016). As for other factors, insertion site (p=.001), place of insertion (p<.001) showed a significant difference. In pathogen factors, the type of intensive care unit (p=.038) also showed significant statistical values. In environmental factors, the number of 3-way hub manipulations (p=.007), MBP (p=.040), density of multidrug-resistant organisms (p=.017), and pressure of multidrug-resistant organisms (p=.005), nursing staff (p=.002) showed a meaningful difference.
2) The strains that caused central line associated bloodstream infection were Gram positive spp., Gram negative spp., and Fungus and the ratio were 41.9%, 38.5%, and 19.7% respectively.
3) A multivariate analysis was performed with the variables significantly identified (p<.05) during univariate analysis using the cox proportional hazards model. As the number of hospital stays increased by 1 day, the risk decreased (HR=0.95; 95% CI=0.93–0.97), and for the APACHEII score, the risk increased 1.04 times every 1 point increase (HR=1.04; 95% CI=1.00–1.07). The risk increased in case of anterior chamber between intensive care units (HR=2.26; 95% CI=1.03–4.96), and the risk increased 2.43 times when one or more multidrug-resistant organisms colonized than in the case of non-colonization. As insertion site, the risk increased when implanted in the femoral are (HR=1.90; 95% CI=1.13-3.22) and the cubital fossa area (HR=7.15; 95% CI=1.40–36.50) and the operating room (HR=2.26; 95% CI=1.03–4.96) was found as the safest place to insertion. Among environmental factors, the risk of 3-way hub manipulations increased by 1.03 times per each increase (HR=1.03; 95% CI=1.01–1.04). Through this study, it was confirmed that incidence of CLABSI appears through patients, pathogens, and environmental factors identified in the triangle model. This is significant in that it can be used as basic data in situations where representative CLABSI risk assessment tools are not prepared at home and abroad, and it provides a theoretical basis for early monitoring of CLABSI outbreaks through survival analysis. In particular, it is meaningful in that it presented an effective direction for infection control education and guidelines in nursing practice through the results identified as a major risk factor for infection among environmental factors.
