As an on-site diagnostic method, the rapid antigen test (RAT) is mainly used, but its sensitivity is low in the early stage of infection. As a laboratory-based diagnostic method, PCR is widely used due to its high sensitivity, specificity, and accuracy. However, since it requires a nucleic acid extraction process, expensive thermocycling equipment and specialized personnel, there is a limit to its use in field. In order to overcome these limitations, recently, a nucleic acid extraction process has been simplified or an isothermal nucleic acid amplification method such as LAMP has been developed. In this paper, a nucleic acid extraction and nucleic acid detection method were developed for on-site diagnosis of infectious diseases. First, a technology for on-site diagnosis of candidiasis caused by Candida blood infection was developed. As a method for extracting nucleic acids from blood samples infected with Candida, the Chelex-100/boiling method was developed. In addition, a Candida/Pan LAMP assay was developed that can diagnose all species of Candida. The developed technology can be diagnosed within 1 hour and shows 100% sensitivity and specificity for clinical samples. Second, we developed a rapid SARS CoV-2 reverse transcription loop-mediated isothermal amplification (RT-LAMP)-lateral flow assay (LFA). This kit includes a Chelex-100/boiling nucleic acid extraction device and one-step amplification detection apparatus, which can perform the entire process from RNA extraction to detection and can diagnose SARS CoV-2 infection without contamination. The developed technology can diagnose within 40 minutes, and showed 97.8% sensitivity and 100% specificity for clinical samples. Last, a LAMP assay that can simultaneously diagnose coronavirus and influenza, which have similar symptoms was developed. In this study, influenza A, influenza B and SARS-CoV-2 were diagnosed in real time using the LAMP assimilating probe. The developed LAMP assay was compared with commercial qPCR kits in detection performance, and it was confirmed that the detection performance of the two methods was similar with a kappa index of 0.7 or higher for all viruses. In conclusion, developed the nucleic acid extraction and molecular diagnosis method for on-site diagnosis of infectious diseases is valuable as a low-cost, rapid, simple, and accurate POCT platform.
