A Study on the application of an integrative arts therapy program to improve self-regulation in children with oppositional defiant disorder원문보기

This study is a qualitative case study that designed and implemented an integrative arts therapy program to enhance self-regulation abilities in young child with oppositional defiant disorder. The research subject is a 6-year-old boy who meets the diagnostic criteria for oppositional defiant disorder according to DSM-5 and exhibits oppositional behavior problems within the clinical range on the Child Behavior Checklist (CBCL). Oppositional defiant disorder is a prominent emotional and behavioral disorder in early childhood, characterized by three core symptoms: frequent anger and irritability, argumentative and defiant behavior, and vindictiveness, which result in maladaptive functioning in daily life. In Chapter 1, the diagnostic criteria, causes, and symptoms of oppositional defiant disorder were examined as the theoretical background. In Chapter 2, the concept of self-regulation, a significant factor for improving oppositional defiant disorder, was divided into three sub-factors: physiological regulation, emotional regulation, and cognitive regulation. Specific behaviors related to age-appropriate self-regulation abilities for the research subject were described. In Chapter 3, the concept of integrative arts therapy, therapeutic factors that could influence oppositional defiant disorder and self-regulation were identified, and based on this, an integrative arts therapy program was designed. The design and analysis process proceeded as follows. Firstly, through initial interviews and pre-assessments, the primary concerns of the research subject were identified. Based on this information, the three sub-factors of self-regulation and symptom improvement goals were designed according to the CERAT stages. Secondly, distinct models and techniques were selected to achieve the symptom improvement goals for each session, with a total of 16 sessions in the program. Thirdly, the program was conducted twice a week for 50 minutes per session. During this process, qualitative analysis was performed to examine the behavioral changes in the research subject, and pre- and post-assessments were conducted to obtain reliability Based on post-assessments and interviews, the analysis of changes in the research subject revealed a decrease in explosive anger expressions, excessive behaviors, blaming others, and passive vindictiveness within the home environment. In terms of behavioral regulation, there was a reduction in emotionally reactive and aggressive behaviors, which were initially within the clinical range according to the Child Behavior Checklist (K-CBCL), and a significant increase in emotional regulation. This indicates that the integrative arts therapy program had a positive impact on behavioral regulation within the home environment and emotional regulation in daily life for children with oppositional defiant disorder. However, this study yielded limited results regarding changes in behavior and cognitive regulation within the kindergarten setting.
