Objectives: Despite the well-known beneficial health effects of serum bilirubin through the alleviation of oxidative stress, the association between serum bilirubin and oxidative stress-related diseases has been confined to observational studies. Therefore, this study investigated the causal relationship between serum bilirubin and oxidative stress-related diseases and mortality. Methods: This study was conducted with 9,229 participants from the Dong-gu cohort study. The 2 SNPs (rs11891311, rs4148323) in the UGT1A1 gene were used as instrumental variables for serum bilirubin. Observational approach analyses were conducted to evaluate the observed association between serum bilirubin and oxidative stress-related disease and mortality. The association between genetically predicted serum bilirubin and oxidative stress related disease and mortality was evaluated with a mendelian randomization (MR) approach using two-stage least squared regression. In each approach, different regression models were applied according to the type of outcome variables. Linear regression was used for continuous variables, logistic regression was utilized for binary variables, and Cox proportional hazard regression models were applied for mortality. Results: In the observational approach, this study found a positive association between total bilirubin and total cholesterol, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, appendicular muscle mass index, handgrip strength, estimated glomerular filtration rate, bone mineral density, and MMSE-K scores. Conversely, there was an inverse association between total bilirubin and systolic blood pressure, pulse rate, fasting glucose, HbA1c, triglyceride, CRP, brachial-ankle pulse wave velocity, serum creatinine, cystatin C, UACR, hypertension, diabetes, peripheral arterial diseases, sarcopenia, CKD, and cognitive impairment. Significant inverse associations were found between total bilirubin and overall mortality, cancer mortality, and mortality from other causes. In the MR approach, genetically predicted serum bilirubin did not exhibit significant association with oxidative stress-related diseases, their risk factors, and mortality. Conclusion: There was no evidence of an association between serum bilirubin and oxidative stress-related diseases and mortality in this MR study. The results suggest that the beneficial association of serum bilirubin with oxidative stress-related diseases found in observational approaches could potentially be attributed to unmeasured confounding or reverse causation. Further analysis with a larger sample size might be needed. In addition, conducting bidirectional MR analysis to evaluate the direction of causal association, could be beneficial for expanding our knowledge.
