The purpose of this study was to compare a sample of patients who had condylar fractures treated with open reduction using different plating techniques, to evaluate which plating technique is useful for stable fixation for fractures of the mandibular condyle and to evaluate effectiveness of resorbable miniplate. There were 60 patients (41 males, 19 females) whose condylar fractures were treated with open reduction. Rigid fixation was performed with a single miniplate, double miniplate ot one miniplate & one microplate and single resorbable plate. All patients remained intermaxillary fixation for 1 week postoperatively. Active physiotherapy was started after 2 weeks postoperatively. Radiographic evaluation (plate fracture, plate bending, screw loosening, displacement of condyle etc.) was performed at pre-operative, immediate, 2 weeks, 1 month, 3 months, 6 months after surgery. Clinical evaluation included degree of mouth opening, occlusion, mandibular lateral excursion, infection and facial nerve paralysis. In radiographic evaluation, displacement of fractured condylar segment associated with plate bending or screw loosening were showed 6 cases and 3 cases for single miniplate system and resorbable miniplate system. There was no patients who have this problem on double miniplate system. The results revealed that the application of two miniplates were more recommendable than single miniplates. When we select resorbable miniplate system, we should consider the type of fractures, post-operative treatment protocol and surgical technique.

제안 방법
Group A에서 고정판의 휨이 나타난 경우와 Group C에서 골절편의 변위가 나타난 경우 중 각각 1명은 지속적인 동통과 개구 장애로 재수술을 시행하였다(Table 4).
Type II 골절이 38 명으로 가장 많았으며, Type III 골절이 9 명, Type IV 골절이 3 명, Type V 골절이 10 명으로 분류되었다(Table 2). 모든 하악 과두 골절 수술시 악하(submandibular) 접근법을 사용하였고, 골절편의 정복 후 33 명은 4-hole miniplate(Martin®, Zimmer, USA) 1개, 19 명은 4-hole miniplate (Martin®, Zimmer, USA) 2개 혹은 4-hole miniplate(Martin®, Zimmer, USA) 1개와 4-hole microplate(Martin®, Zimmer, USA) 1개, 8 명은 흡수성 고정판 1개(BioSorbFXTM®, Linvatec Biomaterials Ltd, Finland)를 이용한 견고 고정을 시행하였으며, 각각을 Group A, B, C로 분류하였다(Fig. 1). 모든 환자는 술후 1주간 악간 고정을 유지하였으며, 술후 2주째부터는 일반식사가 허용되었으며, 6개월 이상 경과 관찰을 시행하였고, 자료는 술전, 수술 직후, 술후 1 개월, 3 개월, 6 개월경에 촬영된 panoramic & reverse Towne’s position radiograph와 임상검사 결과를 이용하였다.

임상적 검사로는 수술 직후, 술후 1 개월, 3 개월, 6 개월경에 교합상태, 개구량, 개구시 하악 편위, 안면신경 손상유무, 감염 등이 평가되었으며, 교합상태는 좌, 우 조기 접촉 유무를 관찰하였으며, 개구량은 상, 하악 중절치의 절단부 사이의 거리를 측정하였고, 개폐구시 하악이 이환측으로 변위가 일어나는지 검사하였다. 안면신경의 경우“우”, “이” 등을 발음할 때, 좌, 우 비교시 하순과 구각부의 운동이 유약한 경우 손상이 있는 것으로 간주했다.
대상 데이터
본 연구는 하악 과두 골절로 2002년 1월부터 2003년 12월까지 경북대학교 병원 구강악안면외과에서 관혈적 정복술을 시행한 환자 중 6 개월 이상 방사선 및 임상적 평가가 가능했던 60 명의 환자(남 41, 여 49)를 대상으로 하였다. 수술시 평균연령은 28.
성능/효과
1. 하악 과두 골절의 관혈적 정복술 후 임상 검사에서 개구 장애가 한 개의 miniplate로 고정한 군과 흡수성 miniplate로 고정한 군에서 각각 1 명씩 나타났으며, 두 경우 모두 회복이 불가능하였고 재수술이 시행되었다.

또한 각각의 군에서 신경 손상 정도는 큰 차이가 나타나지 않아 신경 손상 위험도 면에서 하나의 miniplate를 고집할 이유는 없는 것으로 사료된다. 본 연구에서 나타난 신경 손상의 경우 11 명은 3 개월 내에, 3명은 6 개월 내에 완전히 회복되었다. 수술 시간은 하악 과두 골절만 있는 경우와 정중부 골절이 동반된 경우 모두 각군간에 크게 차이가 없었다.
수술 방법에서 골절선으로의 접근 방법은 악하(submandibular) 접근법을 사용하였는데 이와 연관된 일시적인 안면신경 손상은 Group A, B, C에서 각각 8명(24%), 4 명(21%), 2 명(25%)으로 나타났다. 이 접근법을 사용할 때 주로 안면신경의 하악 연지(marginal mandibular branch)가 견인에 의한 손상을 입게 되는데, 다른 구외 접근법 사용 후 나타난 신경 손상을 보면 Chossegros 등18)은 후하악(retromandibular) 접근법으로 수술한 후 11%의 안면신경 손상을 보고하였고, Eckelt와 Rasse19)는 lagscrew를 이용한 후 13%의 신경 손상을 보고하였고, Tasanene과 Lamberg20)는 악하(submandibular) 접근법을 이용하여 하악 과두 골절의 관혈적 정복술을 시행한 후 37%의 일시적인 안면신경 손상을 보고하여 본 연구와 크게 차이가 나지 않았다.
수술 시간은 하악 과두 골절만 있는 경우 Group A에서는 2.50±0.74시간, Group B에서는 2.90±0.27시간이었고, 하악 과두와 정중부 골절이 동반된 경우 Group A에서는 3.20±0.79시간, Group B에서는 3.10±0.57시간, Group C에서는 3.10±0.69시간으로 나타나 각 군간의 차이가 크게 나타나지 않는 것으로 관찰되었다(Table 5).
방사선 사진상의 고정판의 휨이나 나사 풀린 등도 double miniplate 군에서는 나타나지 않았다. 이와 같은 결과로 하악 과두 골절의 관혈적 정복시 골절편을 한 개의 금속 miniplate나 흡수성 miniplate로 고정하면 악간 고정 제거 후 가해지는 부하를 견딜 수 없다는 것을 알 수 있었다. 보다 안정적인 고정을 위해서는 double miniplate로 고정하는 것이 추천된다고 할 수 있다.
후속연구
최근 Suzuki 등24)은 12명의 환자에 흡수성 miniplate를 사용하여 3 년간 관찰한 연구에서 일시적인 안면 신경 손상 외에 어떤 합병증이나 문제점도 나타나지 않았음을 보고하였다. 그러나 본 연구에서는 8 명 중 3명에서 하악 과두의 변위 및 개구시 하악의 이환측으로의 편위가 나타나 지속적인 물리 치료와 검사가 요구되었다. 본 연구에서는 술후 악간 고정이 1 주동안 유지되었고 술후 2 주째부터는 일반 식사가 허용되었으나 앞선 다른 연구에서는 악간 고정을 2 주 동안 유지하고 2주 더 탄성 고무를 이용한 악간 고정을 유지하였다.
또한 Suzuki 등24)의 연구에서는 사용된 흡수성 miniplate의 종류가 4-hole miniplate 1 개 (6명), 4-hole miniplate 2개 (2명), T-shape miniplate (6명)으로 다양하였으며, 골절의 양상을 살펴보면 본 연구에서는 8 명 중 5 명이 변위가 있는 낮은 골절이었는데 반해 Suzuki 등24)의 연구에서는 14 명 중 8 명이 변위가 없는 골절이었다. 따라서, 하악과두 골절에 흡수성 miniplate를 사용할 때는 골절선의 양상 등 환자 선택에 있어서 신중을 기해야 하며, 술후 악간 고정 기간 등도 금속 miniplate와 다른 술 후 프로토콜이 요구되며, 흡수성 나사에 필요한“tapping”과정에서 나사 구멍을 과도하게 크게 만들거나, 나사 삽입시 잘못된 방향으로 과도한 힘을 주어 수술 중에 나사에 손상을 주는 등의 기술적인 문제가 고려되어야 할 것으로 생각되며, 추후 흡수성 miniplate 2 개를 적용하는 임상 연구가 추가되어야할 것으로 사료된다.
