Results: Analyses on each tumor marker indicated that squamous cell carcinoma in the head and neck region had statistically significant correlation with p53, SCC-Ag(TA-4), Cyfra 21-1 and Ferritin. p53 demonstrated the highest sensitivity. Especially, 4 cases among 18 cases which Ferritin was measured exhibited metastasis. In all those 4 cases, Ferritin values were higher than the standards (15 - 332ng/ml). Therefore, Ferritin is considered to have a close relation with metastasis of squamous cell carcinoma. 제안 방법

데이터처리
각각의 종양표지자들과 나이, 성별, 분화도, 병기 등의 요소들 사이의 연관성에 대해서는 Pearson correlation coefficient analysis를 이용하여 분석하였다.

이론/모형
각 환자들은 혈액을 채취하여 이를 핵의학과에서 각각의 kit를 이용하여 방사면역측정법(Radioimmunoassay; RIA)과 면역방사계수측정법(Immunoradiometric Assay; IRMA)을 이용하여 측정하고 p53은 효소면역검정법 (Enzyme Immunoassay; EIA)을 이용하여 측정하였다. 각각의 kit 및 종양표지자들의 정상 범위는 다음과 같다 (Table 2).
성능/효과
1. Ferritin은 성별에 따른 차이가 비교적 뚜렷한 종양표지자이며, 두경부에 발생한 편평세포암종에 대해서는 33.3%의 민감도를 나타내었다.
2. Ferritin을 종양표지자로 채취한 18례 중 4례에서 전이가 있었으며, 4례 모두에서 Ferritin은 기준치를 초과하는 측정치를 나타내었다. 이로 볼 때, Ferritin과 종양의 전이성과는 밀접한 연관성이 있음을 알 수 있다.

본 연구에서 각각의 종양표지자들은 그 자체로 진단적 도구로 이용되기에는 정확성이 낮음을 알 수 있다. 따라서 아직까지 종양의 진단에 있어서는 영상학적 자료와 조직검사를 통한 진단이 더욱 유용하다 할 수 있다.

이상의 결과를 종합하여 볼 때, 여러 종양표지자들 중 Ferritin은 구강내에 발생한 편평세포암종의 전이 유무를 예측하는 데 유용할 것으로 생각되며, 이를 임상에 적용하면 치료의 방법 결정 및 술 후 계속적인 치료에 많은 도움이 될 것으로 생각된다.
본 연구에서는 p53이 민감도는 높았지만 종양의 전이성과는 유의할 만한 연관성을 나타내지는 않았다. 하지만 p53 표본수가 6례로 작기 때문에 좀 더 많은 연구가 필요하다.
