Objective : The purpose of this study is to understand aging and skin aging with both western and Korean medicine. Methods : We investigated the comprehension of general aging and skin aging in both western and Korean medicine through literature review. Result : The results are as follows. 1. Aging in western is a complex process influenced by telomere shortening and damage to cellular DNA. In Korean medicine, it is supposed that decline of interaction between yin(陰) and yang(陽) makes aging. 2. Skin aging in western medicine consists of intrinsic aging and extrinsic aging. In Korean medicine, skin aging goes with the aging of whole body and distinctive features of geroderma notice the unbalance of the whole body function. 3. Generally geroderma has decreased development of cells and low immunity, which is the function of kidney(腎) in Korean medicine. Conclusion : Further studies are needed to apply comprehension of skin aging in Korean medicine to clinical stage.

대상 데이터

서양의학 서적으로는 《노화방지의학》¹⁸⁾, 《피부노화학》¹⁹⁾을 중심으로 노화 및 피부노화에 대해 기술한 서적의 전체 혹은 일부를 발췌하여 재료로 삼았다. 또한 학술 데이터베이스 검색사이트(KSI KISS)를 통해 ‘aging’, ‘skin aging’ 등을 검색하여 얻은 다수의 논문 중 노화와 피부노화의 정의, 기전, 임상양상, 조직학적 변화 등을 기술한 논문을 조사, 연구하였다.
한의학 서적으로는 현존하는 最古書 중 하나이며 한의학 이론의 근간이 되는 《黃帝內經》^3,16)을 위주로 고찰하였으며, 필요에 따라 《東醫寶鑑》¹⁷⁾을 참조하였다. 또한 전통의학정보포털시스템(OASIS)에 ‘aging’, ‘skin aging’, ‘노화’, ‘피부노화’ 등을 검색하여 얻은 다수의 논문 중 노화와 피부노화의 원인, 기전, 양상 등에 대한 고찰을 위주로 한 논문을 조사, 연구하였다.

성능/효과

1. 서양의학에서는 노화의 원인과 기전을 설명하기 위해 다양한 가설이 제시되었는데, 가장 대표적인 것은 텔로미어 단축설과 자유라디칼설이다. 한편 한의학에서는 陰陽교류를 생명의 원천으로 보고, 생명활동 중 陰精의 소모에 따라 陽氣가 虛火로 변해 上逆함으로써 陰陽의 균형이 실조되고 氣血이 문란해지기 때문에 노화가 발생한다고 보았다.
2. 서양의학에서는 피부노화를 내인성 노화와 광노화로 나누어 설명하였다. 한의학에서는 피부노화를 전신노화에 수반되는 현상으로 보고, 안색 및 모발의 변화 등을 腎氣衰, 陽明脈衰 등의 전신 노화 상태를 드러내는 표지로 파악하였다.
3. 노화피부의 전반적 기능저하를 유발하는 세포의 분열과 재생, 멜라닌 색소나 비타민 D 전구물질의 합성저하, 전신 및 피부면역의 저하는 한의학적으로 발육, 면역을 담당하는 腎의 기능저하와 유관하다고 생각되며, 이는 피부노화가 전신노화에 수반 하여 발생하며 藏精을 主하는 腎臟의 기능저하가 전신노화에 주요한 원인이 되기 때문으로 생각된다.

	피부의 노화에 관한 특징은?	근래에는 노화에 의한 병리적 질환을 치료하는 것을 넘어 생리적 기능저하를 지연 혹은 회복시킴으로써 건강하고 젊은 노년을 보내고자하는 대중의 욕구가 높아지고 있는데, 그 대표적인 관심분야가 피부노화이다2). 피부는 외부환경에 지속적으로 노출되어 시간의 흐름에 따른 손상이 인체 다른 계통에 비해 확연한 특징이 있다. 또한 자각적·타각적으로 노화의 정도를 가늠하기 쉬운 척도가 된다.
