Objectives : The aim of this study was to investigate the effect of Omibaekchul-san on postpartum edema in women hospitalized at korean medical postpartum care center in comparison with Saenghwa-tang. Methods : The body composition data was collected from 102 women, who took postpartum care in postpartum care center attached to Won-kwang University Jeonju Korean Medical Hospital, from March 2012 to July 2013. The data was analyzed by Body Composition Analyzer (Inbody 720). Specifically the data was composed of the Extra Cellular Fluid/Total Body Fluid (ECF/TBF), Total Body Water (TBW), Intra Cellular Water (ICW), Extra Cellular Water (ECW), Body Weight, Body Mass Index (BMI), Waist Hip Ratio (WHR), and the ECF/TBF of each body parts such as the upperlimb, lowerlimb and trunk. The body composition was measured twice during 2~8.5 days from childbirth and once again after 7~13.5 days from childbirth by Body Composition Analyzer. Results : The Omibaekchul-san -treated group showed a significant decrease of ECF/TBF, ECW, TBF, Body weight, BMI, ECF/TBF of each body parts and increase of WHR in comparison with the data of the Saenghwa-tang-treated group. In two groups, only the change of ICW was unsignificant. Conclusions : Statistically this study suggests that the Omibaekchul-san will help the quality of life of women who are in postpartum edema.

이에 저자는 산후부종에 대한 오미백 출산의 효과를 검증하고자 ○○대학교 ○○한방병원 부속 산후조리원 입원 환자 중, 오미백출산 또는 생화탕을 복용하고 체성분이 측정된 산모의 자료를 선별하여 연구를 진행하고, 유의한 결과를 얻었기에 이를 보고하는 바이다.
제안 방법
Extra Cellular Fluid/Total Body Fluid (ECF/TBF), Total Body Water(TBW), Intra Cellular Water(ICW), Extra Cellular Water(ECW), Body Weight, Body Mass Index(BMI), Waist Hip Ratio(WHR), Trunk & Both limbs ECF/TBF의 변화량을 관찰하였다.
산후조리원 입원 1일차 또는 2일차에 1차 체성분 검사가 시행되고, 입원 5일차에서 7일차 사이에 2차 체성분 검사가 실시된 환자의 자료를 대상으로 하였다. 모든 환자는 금속류를 부착하지 않은 가벼운 옷차림 상태에서 오전 10~12시 사이에 체성분 측정이 이루어졌다.
대상 데이터
다음에 제시된 1)의 조건을 모두 만족시키고, 2)의 배제 조건(Fig. 1) 중 한 항목이라도 해당하는 산모의 기록은 제외하여 오미백출산 복용군 50명, 생화탕 복용군 52명 총 102명의 자료를 선정하였다.
조사 대상자 884명 중 오미백출산과 생화탕을 복용한 산모 523명의 자료를 1차 선별하였고, 추가적으로 배제 기준(Fig. 1)에 따라 나머지 421명을 제외한 오미백출산을 복용한 52명, 생화탕을 복용한 50명총 102명의 자료를 최종 선별하여 연구를 진행하였고 그 결과는 다음과 같다.

이론/모형
본 연구에서는 전기저항을 이용한 다주파수 부위별 생체전기저항측정법(Bioelectrical Impedance Analysis, BIA)을 적용한 Biospace사 제품인 체성분 분석기(Body Composition Analyzer) Inbody720를 통한 데이터를 기반으로 하였다.
성능/효과
생화탕 복용군 52명에서 질식분만을 시행한 산모는 37명(71.2%)이며, 제왕절개술을 시행한 산모는 15명(28.8%)이었다.
최근의 연구에서 산욕기 산모가 호소하는 증상은 계통별로는 근골격계 39.3%, 생식기계 20.5%, 순환기계 14.

따라서 분만 후 정상적인 이뇨작용은 분만 후 2~5일이 지난 후 나타나고 보통 이러한 이뇨작용을 통해 출산 후 2~3 kg의 수분배출이 이루어진다29). 위 사실로 미루어 봤을 때 출산 직후 체내 수분저류를 적게 하고 부적절한 이뇨작용을 돕는 작용이 오미백출산이 생화탕보다 더 우수하며, 체내수분량 감소를 통한 체중 감량도 오미백출산군이 평균적으로 더 큰 폭의 감소를 보인 것으로 사료된다.
자료 분석 결과 ECF/TBF 항목은 오미백출산 복용군에서 0.0133±0.0059 감소하였고, 생화탕 복용군에서는 0.0078±0.0067 감소하여 두 군은 통계적으로 유의한 차이를 보였다(p=0.000). TBW는 오미백출산 복용군에서 2.
후속연구
또한 문헌적으로 나와 있는 탕약의 효과를 실제 산욕기 산모의 자료를 통해 입증하였기 때문에 앞으로 탕약을 처방하는 데 있어서 일정부분 그 근거를 뒷받침할 수 있을 것이라고 사료된다. 그러나 본 연구는 산후부종 외에도 산욕기에 관심 있게 치료하고 관리해야 할 자궁 회복 상태나 회복 정도, 오로 상태, 전반적인 몸 회복속도 등에 대한 평가가 동반되지 않아서 이 부분에 대한 평가도 추가적으로 필요할 것으로 여겨지며, 탕약을 복용하지 않은 산모군을 추가로 연구하면 산후 부종에 대한 오미백출산의 효과를 조금 더 객관적으로 파악할 수 있으리라 사료된다.
