Purpose: The objective of this study is to identify factors associated with perceived health status of married immigrant women. Methods: Data were collected from 65 married immigrant women. The measurements were Symptom checklist-48, Acculturative Stress Scale, Parenting Stress Index(PSI), and Personal Resource Questionnaire(PQR). Descriptive, T-test, ANOVA test, Pearson correlation and multiple regression analyses were used. Results: There were significant differences in perceived health status according to satisfaction with the married life(F=8.819, p=0.000), and help from husband(F=6.308, p=.003). There was also a significant relationship between perceived health status and perceived economic status (F=8.006, p=.001), acculturative stress (r=.560, p<.01), parenting stress (r=.662, p<.01) and social support (r=-.289, p<.05). The result of multiple regression analysis showed a significant association of parenting stress with perceived health status of married immigrant women (${\beta}=0.397$, p=0.037). Conclusion: Comprehensive interventions designed to enhance the health of married immigrant women, including prevention of acculturative stress, parenting stress and improving social support may be helpful in mitigating health in married immigrant women.

스트레스 상황이 개인의 대처능력을 초과할 때 임상적 우울, 불안과 같은 심각한 건강 문제가 나타날 수 있다(Anisman, 2015). 그러므로 본 연구에서는 결혼이주 여성의 인지된 건강정도를 확인하고 이들이 경험하는 스트레스 중 기존 연구에서 중요한 요인으로 언급되어 온 문화적응 스트레스, 양육 스트레스와 스트레스 완충 작용으로 중요한 사회적 지지를 중심으로 인지된 건강정도에 영향을 주는 요인을 파악하여 이들의 건강에 도움이 될 수 있는 중재 방안을 마련하는데 기초자료를 제공하고자 한다.
제안 방법
대상자의 일반적 특성은 예비조사와 문헌고찰 결과 어머니의 건강에 영향을 줄 수 있는 변수를 포함하여 문항을 구성하였다. 어머니 연령, 학력, 직업, 종교, 지각된 경제 상태, 동거 가족과의 관계, 결혼 만족도, 남편도움, 출신국, 자녀수가 포함되었다.
대상 데이터
본 연구는 서울시 N병원 다문화가정 진료센터와 경기도 G군 다문화가족 지원센터를 방문한 결혼이주 여성을 유한모집단으로 하여 한국인 남성과 결혼하였고, 자녀가 있으며 질문의 내용을 이해하고 응답할 수 있으며, 본 연구에 참여를 동의한 결혼이주여성으로 하였다. 표본 산출을 위해 G power 3.
설문에 사용된 시간은 20분 정도 소요되었고 자료수집기간은 2013년 10월 1일부터 2014년 2월 28일까지 하였다. 설문지는 모두 70부를 배부하여 70부를 회수하였으며 회수한 설문지 중에서 응답이 충분히 되지 않은 설문지 5부를 제외한 65부를 분석하였다.
데이터처리
대상자의 일반적 특성은 실수와 백분율로 분석하였고 주요 변수의 점수는 평균과 표준편차로 분석하였다. 결혼이주 여성의 인지된 건강정도와 각 요인과의 상관관계는 Pearson correlation coefficient로 분석하였으며, 인지된 건강정도에 영향을 미치는 요인을 분석하기 위해 입력변수 법(enter)으로 다중회귀분석을 하였다.
PASW software (version 19, SPSS Inc, Chicago, IL, USA)를 사용하여 자료를 통계 분석하였다. 대상자의 일반적 특성은 실수와 백분율로 분석하였고 주요 변수의 점수는 평균과 표준편차로 분석하였다. 결혼이주 여성의 인지된 건강정도와 각 요인과의 상관관계는 Pearson correlation coefficient로 분석하였으며, 인지된 건강정도에 영향을 미치는 요인을 분석하기 위해 입력변수 법(enter)으로 다중회귀분석을 하였다.
이론/모형
결혼이주 여성의 인지된 건강정도를 측정하기 위한 연구도구는 Derogatis (1997)가 개발한 증상체크리스트-90을 기초로 하여 Seo와 Oh (1993)가 개발한 건강증상조사서-48을 사용하였다. 건강증상조사서는 신체, 정신, 사회적 건강증상에 대한 48개 문항으로 구성되어 있으며 개발 당시의 Cronbach's α는 .
결혼이주민 여성이 자녀를 양육하면서 경험하는 스트레스를 측정하기 위하여 PSI (Parenting Stress Index) (Abidin, 1990)를 Kim (1997)이 번역한 도구를 사용하였다. PSI는 3가지 하위 척도로 구성 되는데, 부모가 자녀 양육에서 경험하는 스트레스를 그 근원에 따라 부모의 스트레스(부모영역), 부모-자녀의 역기능적인 상호작용(부모-자녀관계 영역), 아동의 까다로운 기질(아동영역)로 구분되어 있다.
문화적응 스트레스는 Sandhud와 Asrabedi (1994)가 개발한 Acculturative Stress Scale for International Student를 Hong (2009)이 번역한 도구를 사용하여 측정하였다. 하위 영역은 지각된 차별감 8문항, 향수병 4문항, 지각된 적대감 5문항, 두려움 2문항, 문화충격 3문항, 죄책감 2문항, 기타(사회적 고립감, 열등감, 불신) 11문항이며, ‘전혀 그렇지 않다’ 에서 ‘매우 그렇다’까지 5점 리커트 척도에 따라 구성되어 있고 최저 35점에서 최고 175점의 범위를 가진다.

성능/효과
문화적응 스트레스, 양육 스트레스, 사회적 지지를 중심으로 파악하고자 하였다. 분석 결과 결혼이주 여성의 인지된 건강정도는 결혼생활 만족도, 남편의 도움, 문화적응 스트레스, 양육 스트레스, 사회적 지지와 관계있는 것으로 나타났으며 유의한 모든 변수 통제한 상태에서는 양육 스트레스가 결혼이주민의 인지된 건강정도에 영향을 주는 요인으로 나타났다.
후속연구
결혼이주여성은 새로이 이주한 나라의 새로운 문화에 적응하며 자녀를 양육하면서 경험할 수 있는 다양한 스트레스로 인해 이들의 건강상태에 부정적인 영향을 받게 되고 상대적으로 취약한 사회적 지지는 이러한 열약한 상태를 가속화시킬 수 있다. 그러므로 결혼이주여성의 건강에 영향을 줄 수 있는 문화적응, 사회적 지지 특히 양육 스트레스를 중재할 수 있는 프로그램을 개발하고 중재할 것을 제언한다. 또한 본 연구 결과를 토대로 결혼이주여성의 건강문제를 감소시키고 건강한 삶을 살 수 있도록 돕기 위한 다각도의 체계적인 지원전략을 개발해야 할 것이다.
그러므로 결혼이주여성의 건강에 영향을 줄 수 있는 문화적응, 사회적 지지 특히 양육 스트레스를 중재할 수 있는 프로그램을 개발하고 중재할 것을 제언한다. 또한 본 연구 결과를 토대로 결혼이주여성의 건강문제를 감소시키고 건강한 삶을 살 수 있도록 돕기 위한 다각도의 체계적인 지원전략을 개발해야 할 것이다.
