Purpose: The purpose of this study was to provide basic data about agitation in patients with dementia by surveying the literature. Method: Key words used for search through hand-search and electronic database (CINHAL, Pubmed, Google scholar, Riss, Kiss, DBpia) included 'dementia', 'Alzheimer disease', 'agitation', 'aggression or aggressive behavior', 'problem or disruptive behavior', and 'abnormal behavior.' Seventeen studies met the inclusion criteria for the Meta-analysis and 'R' version 3.2.2 was used to analyze the correlated effect size. Results: Study results showed that variables related to agitation were identified as the demographic (age, gender), dementia-related (cognition, medication uses), physical (Activity of Daily Living [ADL], pain), psychological (depression, psychotic symptom, caregiver burden) and environmental (psychosocial environment) factors. The effect size between the correlated variables and agitation were low to moderate (caregiver burden .36; ADL -.24; psychotic symptom and depression .21; pain .19; cognition -.15; medication uses .12; and psychosocial environment -.12). Conclusion: Based on the findings of this study, strategies to improve patients' depressive and psychotic symptoms and ADL and to reduce caregivers' burden are needed for prevention and management of agitation in patients with dementia.

이러한 일관성이 부족한 개별 연구들로부터 메타분석의 연구방법을 통해 각 연구들을 심도 있게 분석하고 종합적인 결론을 도출할 필요가 있다. 그러므로 본 연구에서는 다양한 상태와 거주 장소에서 수행된 치매 환자의 초조행동 관련변인에 대한 연구결과를 종합하기 위해 상관계수를 이용한 메타분석을 수행하고자하며, 초조행동의 상관계수 효과크기를 파악함으로써 근거기반 간호 수행의 기초자료를 제공하고자 한다.
제안 방법
Algase 등[10]은 초조행동 유발요인으로 배경요인과 근위요인으로 구분하여 인구학적 특성, 신체적 특성, 일반적 특성, 심리·사회적 특성, 환경적 특성의 세부영역을 제시하였다. 본 연구에서는 이러한 기존문헌을 고려하고 자료분석 대상 논문에서 확인된 하위변인들을 고려하여 초조행동의 관련변인 영역을 인구학적 특성, 질병 관련 특성, 신체적 특성, 심리·사회적 특성, 환경적 특성의 5개 군으로 분류하였다. 변인군에 포함되는 관련 변인들과 효과크기 등을 코딩하였고, 효과크기는 상관계수 값(r)을 활용하였으며, 총 68개의 상관효과크기가 산출되었다.
본 연구는 치매 환자의 초조행동 관련변인들의 효과크기를 산출하기 위해 2015년 10월까지 국내·외에서 발표된 학위논문 및 학술지에 게재된 논문을 검색하였다. 검색과정은PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analysis) 가이드라인에 따라 실시하였고, 분석대상 논문검색은 CINAHL, Pubmed, Google scholar, RISS (Research Information Sharing Service), KISS (Korean studies Information Service System) and DBPIA (Database Periodical Information Academic) 등의 데이터베이스를 활용하였으며, 데이터베이스에 포함되지 않는 논문을 검색하기 위해 관련 참고문헌을 수기로 검색하였다.
최종적으로 각 ‘r’값은 표준화된 효과크기로 변환하기 위해 일반적으로 활용되고 있는 Fisher's Z를 사용하여 산출하였고, 95% 신뢰구간을 계산하였으며, 각 효과크기의 가중치(weight)는 분산의 역수(inverse of variances)를 이용하여 계산하였다. 또한, 본 연구에 포함된 개별연구의 대상자, 연구방법, 측정 도구 측면이 다양함을 근거로 하여, 분석모형 중 랜덤효과모형 (random-effects model)을 사용하여 전체적인 초조행동 관련 변인의 효과크기를 측정하였다.
대상 데이터
검색을 위한 주요어는 치매(dementia), 알쯔하이머(Alzheimer disease), 초조행동(agitation), 공격행동(aggression or aggressive behavior), 문제행동(problem or disruptive behavior), 이상행동(abnormal behavior)을 포함하였고, 검색은 1) 치매 or 알쯔하이머 (dementia or Alzheimer*) and 2) 초조 or 공격 or 문제 or 이상 or 행동(agitation or aggress* or problem* or disruptive or abnormal or behavior) 등을 병합하여 이루어졌다. 문헌을 검색한 결과, 총 17,013개의 논문이 검색되었고, 서지 프로그램 (Endnote X7)을 통해 1,469개의 중복논문을 제외한 결과 일차적으로 15,544개의 논문이 선정되었다.
치매 환자의 초조행동 관련변인들의 효과크기를 평가하기 위해 메타분석에 포함될 논문의 선정기준은 1) 치매 환자를 대상으로 한 연구, 2) 초조행동과 그 관련변인들의 관계를 분석한 양적 연구, 3) 치매 환자의 초조행동 관련요인 변수들 간의 값이 통계치 상관관계 r값에 해당하거나 r 값으로 변환가능한 값(t, F, OR, Mean, SD, n)이 있는 연구, 4) 원문제공이 되는 연구, 5) 언어가 영어 또는 한국어로 기술된 연구이었다. 제외기준은 1) 실험연구, 질적 연구, 리뷰 연구, 2) 초조행동에 망상이나 환각과 같은 정신증상을 포함하고 있는 연구, 3) 초조행동의 총합점수와 관련변인과의 관계를 보고하지 않고 하위영역별 점수와의 관계 만을 보고한 연구, 4) 초조행동과 관련변인의 상관계수 사례수(k)가 3개 미만인 변인만을 포함한 연구, 5) 초록과 포스터만을 발표한 연구 중 초조행동과 관련변인의 통계값을 보고하지 않은 연구이었다. 1차 검색된 논문 중 연구제목과 관련이 없는 15,457개의 논문을 제외하고, 87개 논문의 원문을 검토하였다.
원문검토 결과, 초조행동에 정신적 증상(망상, 환각)이 포함된 연구(20개), 초조행동의 총합점수가 제시되지 않은 연구(15개), 상관관계 값이 제시되지 않은 연구(13개), 상관관계 조사 연구가 아닌 경우(11개), 초조행동과 관련변인의 상관계수 사례수가 3개 미만인 경우(4개), 연구대상자가 치매 환자가 아닌 연구(3개), 초록만 제시한 연구(3개), 중국어로 발행된 연구(1개)를 제외하고 총 17개의 연구가 최종 분석에 포함되었다(Figure 1).

데이터처리
본 연구에서는 메타ANOVA와 메타회귀분석을 사용하여 인지기능의 조절효과 분석을 실시하였다.
이론/모형
