Purpose: This study was conducted to investigate the relationship between cultural competency and the importance of nurses' qualities perceived by undergraduate nursing students. Methods: Researchers developed two tools for this study after reviewing the related literature and conducting research team workshops: questionnaire of cultural competency and the importance of nurses' qualities. 200 nursing students were recruited in convenient sampling to respond to these questionnaires. Of nurses' 10 qualities, major affecting factors on the cultural competency were identified by stepwise multiple regression analysis. Results: The nursing students perceived technical nursing skills and professional nursing knowledge as nurses' most important qualities. However, 'having a passion for patient care', 'demonstrating strong nursing profession's code of ethics' and 'teaching and research ability' were found as significant influencing factors on the variance of the cultural competency. These three factors explained 16% of the total variances of the cultural competency (F=13.98, p<.001). Conclusion: The educational strategies to improve cultural nursing competency need to incorporate students' expectations for the professional nurses' roles. Also, further studies need to develop reliable and valid measurement tools for cultural competency.

간호대학생이 지각하는 간호사 자질 중요도를 측정하기 위하여, 본 연구진은 전문직 간호 업무를 수행하는 데 필수적인 10가지 자질을 선정하고 이에 대한 중요도를 평가하기 위한 설문지를 제작하였다. 본 도구의 10가지 전문직 간호사의 자질은 Medigovich[23]의 ‘훌륭한 간호사(good nurse)’ 자질과 Cowan 등[17]의 전문직 간호사에게 필요한 역량을 검토한 뒤본 연구진의 논의를 거쳐 ‘간호 기술 발휘’, ‘간호학 전문지식’, ‘환자 및 보호자와의 커뮤니케이션 능력’, ‘동료와의 협동적 관계형성’, ‘환자에 대한 관심과 사랑’, ‘건강한 신체 유지’, ‘간호 전문직 윤리구현’, ‘간호학 발전에 기여하려는 열정’, ‘교육과 연구능력’ 및 ‘유머감각’을 주요 간호사 자질로 선정하였다.
연구 초기에는 23개로 구성된 측정도구를 개발하였다. 이때 Campinha-Bacote[12]의 건강 전문가를 위한 문화능력 측정도구(Inventory for Assessing the Process of Cultural Competence among health care professionals-Revised, IAPCC-R)와 Rew 등[22]의 간호대학생을 위한 문화인식 측정 도구(measurement of Cultural Awareness for nursing Student, CAS)를 검토한 뒤, 임상 시나리오를 사용한 설문도구를 제작하였다.

대상 데이터

본 연구대상은 국내 지방 대도시에 소재한 4년제 간호대학의 1학년 학생 100명과 4학년 100명을 편의 표출하였다. 자료 수집은 2014년 9월~2014년 10월 기간 동안 실시하였다.
본 연구에 참여한 대상자는 총 218명이었으나, 불충분한 자료를 제외한 200명의 자료만을 본 연구의 분석대상으로 하였다. G*Power 3.

데이터처리

연구대상자의 일반적 특성에 따른 문화역량의 차이는 Student’s t-test와 일원배치분산분석을 실시하였고, 연령, 지각된 간호사의 자질 및 문화역량과의 관계는 상관관계 분석을, 문화 역량에 주요 영향을 미치는 간호사의 자질을 확인하기 위한 단계적 회귀분석(stepwise regression analysis)을 각각 실시하였다. 이와 같은 분석은 PASW Statistics 18 프로그램을 사용하였다.

성능/효과

본 연구는 200명의 간호대학생을 대상으로 하여 본 연구진이 개발한 ‘문화역량’ 측정도구와 ‘간호사 자질 중요도’ 측정도구를 사용하여 주요 간호사 자질 및 일반적 특성에 따른 문화역량 점수의 차이를 확인하였다. 연구결과 환자에 대한 관심과 사랑, 간호 전문직 윤리 구현 및 ‘교육 및 연구 능력’을 중요시하는 간호대학생일수록 문화역량이 높은 것으로 나타났다.

후속연구

특히 본 연구에서 1학년의 문화 인식과 갈망, 문화 만남 영역의 점수가 4학년에 비하여 유의하게 높았던 점은 본 연구가 실시된 기간 쇼셜 미디어[27]와 미디어와 대학 교육환경[28]이 크게 작용하였다고 해석한다. 본 연구는 이와 같은 사회 환경이나 대학 환경 변수를 포함하여 조사하지 않았기 때문에 1학년과 4학년 사이의 문화역량 차이를 정확히 설명하기 어려웠다. 이는 추후 연구를 통하여 좀 더 확인할 필요가 있다고 본다.
한편 국내 다문화에 민감하고 신뢰도 높은 전문직 자질 측정도구와 문화역량 도구 개발에 대한 추후 연구가 꾸준히 지속될 필요가 있다. 특히 본 연구에서 개발된 문화역량 측정도구를 임상실무자에게도 적용하여 신뢰도와 타당도를 재확인하여 안정된 측정도구로 발전되도록 추가적인 수정 및 보완이 필요하다.
간호대학생의 문화역량 증진 프로그램 운영 시 지식이나 기술 보다는 간호에 대한 열정이나 전문직 윤리를 강조하고 탐구 능력 강화에 초점을 맞출 필요가 있다. 한편 국내 다문화에 민감하고 신뢰도 높은 전문직 자질 측정도구와 문화역량 도구 개발에 대한 추후 연구가 꾸준히 지속될 필요가 있다. 특히 본 연구에서 개발된 문화역량 측정도구를 임상실무자에게도 적용하여 신뢰도와 타당도를 재확인하여 안정된 측정도구로 발전되도록 추가적인 수정 및 보완이 필요하다.
