제안 방법
본 연구는 IMAR의 유용성을 알아보기 위하여 임상에서 치료목적으로 사용하는 재료 중 고밀도 물질로 정형외과적 치환술 시 이용하는 고밀도(L-spine : Titanium, Hip joint: Stainless steel)물질, 뇌혈관 동맥류의 중재적 시술을 위해 사용하는 Platinum성분의 GDC(Guglielmi Detachable)Coil, 치아 충전재로 사용되는 Gold를 연구를 위한 대상으로 선정 후 Somatom Definition Flash(Siemens, Enlarge, Germany.)와 GE 64-MDCT (Discovery 750 HD, GE HEALTHCARE, Milwaukee, USA.)를 사용하였으며 실험으로 획득한 팬텀영상과 이미 촬영된 Raw 데이터를 이용하여 IMAR 알고리즘과 MAR 알고리즘의 소프트웨어를 적용하여 재구성된 데이터의 CT value를 측정하여 CT Nnumber 의 변화율을 비교 분석 하였다. 정량적 분석을 위해 적용한 통계 프로그램은 SPSS 18.
또한 금속물질의 종류에 따라서 최대 6번의 IMAR의 특징인 Iteration loop를 적용시킬 수 있어 다양한 밀도의 금속 물질에 의한 인공물의 발생을 감소시킬 수 있는 금속 인공물 감소 소프트웨어이다[7]. 이러한 IMAR 알고리즘이 CT영상 에서 금속인공물질을 감소시키는데 있어 매우 유용성이 있다는 가설하에 우리는 IMAR라는 금속인공물 감소를 위한 영상 재구성 소프트웨어를 적용하여 실험을 진행 하였다. 기존의 Weighted Filtered Back Projection(WFBP) 이라는 재구성을 적용한 MAR 소프트웨어를 적용한 영상과 비교를 실험을 통해 Iterative Metallic Artifact Reduction 알고리즘 적용 CT 재구성영상의 CT Number 변화율 분석비교해 보고자 한다.
IMAR는 검사에서 금속으로 생기는 영상 왜곡 현상을 보정하는 기능으로 현존하는 유일한 4세대 방법으로 기존에 나와 있던 장점을 모두 반영 및 반복적인 보정 루프 적용이 가능하다. 이번연구에서는 CT value값을 이용하여 감약계수로 환산 후 각 장비별 인공물 감소 정도를 백분율로 계산 후 비교 분석하였다. 팬텀연구의 경우Atteuation deviation(%)이 모든 고밀도 물질에서 IMAR는 WFBP 영상의 Black·White streak type artifact Image를 최소14% ∼ 최대 100%까지 감소되었으며, MAR의 Black·White streak type artifact Image를 최소6% ∼ 최대 94%까지 감소되었다.
지금 까지 영상의 화질을 저하 시키는 금속 인공물을 감소시키기 위해 다양한 소프트웨어를 개발하고 적용해 왔지만 원래 데이터의 왜곡과 주변조직의 화질 저하 등 다양한 단점들이 여전히 문제화 되고 있는 것이 현실이다. 이에 우리는 Iterative Metallic Artifact Reduced(IMAR)라는 금속인공물 감소를 위한 영상 재구성 소프트웨어를 적용하여 실험을 진행 하였다. IMAR란 기존의 MAR의 Iterative correction loop를 통해서 최대한 금속물질 주변의 Soft tissue 및 Bone structure를 원본 CT value와 유사하게 만들어낼 수 있는 장점을 가지고 있다.
데이터처리
팬텀연구는 실험을 위해 준비한 의료용 고밀도 물질을 수조에 넣은 후 조직등가물질인 파라핀(Paraffin)을채워 넣어 조직에 삽입된 조건과 유사한 환경이 되도록 하였다. 스캔 후 획득한 Raw 데이터를 Advantage Workstation(Version 4.6, GE, USA)프로그램으로 전송후 Black hole type artifact Image와 White streak type artifact Image의 평균 CT value값을 측정하였으며 측정된 CT Nnumber를 이용하여 감약계수(Attenuation deviation)로 환산 후 각 장비별 인공물 감소 정도를 백분율로 비교 분석 후 통계처리 하였다. 적용한 수식 (1),(2),(3)은 아래와 같다[Fig.

Gold(Dental fillng)재질에서 WFBP Black hole type artifact Image는 -557.4 HU, White streak type artifact Image는 718.6 HU으로 측정되어 Attenuation deviation이 62%, 49%, MAR Black streak artifact는 -188.63 HU, White streak type artifact Image는 -100.64 HU, IMAR Black hole type artifact Image는 -122HU, White streak type artifact Image는 –29.6 HU 으로 측정되어 Attenuation deviation이 MAR 알고리즘 에서 30%, 22%와 IMAR 알고리즘 적용 시 24%, 16% 로 상대적으로 적지만 변화가 있는 것으로 분석되었다.
MAR Black hole type artifact Image는 –960.8 HU, White streak type artifact Image는 221 HU로 측정되었으며 IMAR Black hole type artifact Image는 –197.6 HU, White streak type artifact Image는 108 HU로 측정되어 Attenuation deviation이 MAR 알고리즘에서 94%, 51%와 IMAR 알고리즘 1%, 38%로 MAR 알고리즘에서보다 IMAR 알고리즘 적용시 CT Number의 변화가 있는 것으로 분석 되었다.

Raw 데이터 영상에서 IMAR는 WFBP 영상의 Black hole type artifact Image와 White streak type artifact Image를 최소7% ∼ 최대54%까지 감소되는 것을 확인 할 수 있었다.
각 장비별 획득된 영상 데이터와 WFBP 영상에서 Black hole type artifact Image와 White streak type artifact Image에서 동일 부위에 직경 2mm의 ROI를 그려 CT value값을 측정하여 평균 CT value값을 분석한 결과 Platinum(Neuro Coil)재질은 WFBP영상 Black hole type artifact Image는 -1019.8 HU, White streak type artifact Image는 219 HU로 측정되어 Attenuation deviation이 101%, 52%로 나타났다.
