본 연구의 목적은 임상실습 경험에 따른 물리치료 대학생들의 손 씻기에 대한 인식도와 실천도를 조사하여 물리치료학과 학생의 손 씻기에 대한 기초 자료를 제공하고자 하였다. 본 연구는 광주광역시 소재의 물리치료학과 대학생을 대상으로 총 536부의 설문조사하여 자료를 수집하였다. 대상자의 일반적 특성을 빈도분석으로 산출하였고, 임상실습 유·무의 손 씻기에 대한 인식도와 실천도를 독립표본 t-test로 분석하였다. 임상실습 유·무에 따른 손 씻기 인식도를 분석한 결과, 손목까지 손 씻기에서 유의한 차이를 보였고(p<0.05) 올바른 손 씻기 6단계에서 유의한 차이를 보였다(p<0.01). 임상실습 유·무에 따른 손 씻기 실천도를 분석한 결과, 식전과 임상실습 전 손 씻기에서 유의한 차이를 보였고(p<0.05) 실습기구 접촉 후 손 씻기, 엄지손가락부터 손가락 돌리며 손 씻기에서 유의한 차이를 보였다(p<0.01). 인식도와 실천도 사이의 상관관계를 분석한 결과 대부분 양의 상관관계를 보였다. 이는 손 씻기에 대한 인식도를 높여서 실천도를 높아야 함을 의미한다. 따라서, 앞으로 학교 내와 임상실습 내에서 자체적으로 손 씻기 교육을 반복적으로 실시하여 인식도와 실천도를 높여야 할 것으로 사료된다.

The purpose of this study was to provide basic data on hand washing of physical therapists by examining their awareness and practice of hand washing according to proper clinical practice. A total of 536 questionnaires were collected from physical therapy students in Gwangju metropolitan city. The general characteristics of the subjects were calculated by frequency analysis, and the awareness and practice of hand washing with and without clinical practice were analyzed by independent t-tests. As a result of analyzing hand washing awareness according to the clinical practice, there was a significant difference in hand washing up to the wrist (p<0.05), and a significant difference in 6 stages of proper hand washing (p<0.01). As a result of analyzing hand washing practice according to clinical practice, there was a significant difference in hand washing before eating and starting clinical practice (p<0.05), a significant difference in washing hands after contacting clinical practice equipment and washing your fingers from your thumb down (p<0.01). As a result of analyzing the correlation between awareness and practice, most showed positive correlation. This means it is important to raise the awareness of proper washing hands. Therefore, it is believed that the training school should repeatedly conduct its own hand washing training within the school and in clinical practice to increase awareness and improve handwashing in clinical practice.

본 연구의 대상자는 광주 지역 대학에서 재학 중이고 학교 내에서 손 씻기의 전문적인 교육을 듣지 않은 물리치료학과 학생 629명으로 대상자 모두 연구목적을 들은 후 동의하였고 학생들의 윤리적인 측면을 고려하여 연구의 목적, 설문지의 내용, 연구철회가 가능다는 부분을 명시한 서면동의서에 서명을 한 경우에 한해 설문지를 작성하도록 하였다. 표본 추출은 G*power 3.

데이터처리

둘째, 대상자의 임상실습 경험 유·무에 따른 손 씻기 인식도와 실천도는 독립표본 t-검정으로 분석하였다.
셋째, 손 씻기 인식도와 실천도와의 상관관계를 검증하기 위해 Pearson's 상관계수를 사용하여 분석하였다.

성능/효과

(P<0.05) ‘식전 손 씻기’는 임상실습을 다녀온 학생이 오히려 더 낮았으며 이는 선행연구로 보건계열 대학생들의 '식사 전' 손 씻기에 대해 낮은 실천율을 보인 결과와 일치하였고[22], 군 병원에 입원한 환자의 경우 ‘식사 전 손 씻기를 한다’의 점수가 가장 낮았다는 연구결과와 비슷하게 나타났다[29]. ‘화장실이용 후 손 씻기‘의 경우로 임상실습 전과 후에서 유의한 차이는 나타나지 않았지만 비교적 높은 점수를 보였는데 이는 선행연구에서 군 병원 입원 환자의 ’화장실 사용 후‘[29], '공중화장실 이용 후'[30], '화장실에 다녀온 후'에서 가장 실천율이 높게 나타난 결과와[31] 유사하게 나타났다.
6%에 지나지 않았다고 하였다[28]. 본 연구도 선행연구와 같이 저조하였으나 본 연구에서 ‘올바른 손 씻기 6단계를 알고 있다’ 문항에서는 임상실습을 다녀오기 이전보다 이후에서 더 좋은 결과가 나타났다. 이는 학교와 병원에서 손 씻기에 대한 교육은 따로 하지 않지만 물리치료 임상실습을 통해 물리치료사의 손 씻기를 직접 관찰함으로써 이러한 결과가 나온 것으로 보인다.
손 씻기 실천에 대한 임상실습 유·무의 관계를 비교한 결과, 식전 손 씻기에서 통계학적으로 유의한 차이를 보였고(p<0.05), 실습 전 손 씻기에서도 통계적으로 유의한 차이를 보였으며(p<0.05), 실습기구 접촉 후 손 씻기, 엄지손가락부터 손가락을 돌리며 손 씻기에서 통계학적으로 유의한 차이를 보였다(p<0.01).

후속연구

첫째, 연구결과의 일반화 가능성 제고를 위하여 대표성이 높은 확률적인 표집방법을 통해 여러 지역을 대상으로 연구가 진행되어야 할 것이다. 둘째, 일정 기간을 두고 정기적으로 손 위생 관련 교육을 진행하였을 때 인식도와실천도의 변화에 대하여 파악함으로써, 변수들 간의 인과관계 규명을 명확히 할 수 있는 전향적인 연구가 추가로 진행되어야 할 것이다.
본 연구의 결과를 바탕으로 다음과 같이 제언 한다. 첫째, 연구결과의 일반화 가능성 제고를 위하여 대표성이 높은 확률적인 표집방법을 통해 여러 지역을 대상으로 연구가 진행되어야 할 것이다. 둘째, 일정 기간을 두고 정기적으로 손 위생 관련 교육을 진행하였을 때 인식도와실천도의 변화에 대하여 파악함으로써, 변수들 간의 인과관계 규명을 명확히 할 수 있는 전향적인 연구가 추가로 진행되어야 할 것이다.

	의료종사자의 감염 노출예방에도 관심이 높아지게 된 계기는?	2010년부터는 보건복지부가 시행하는 의료기관 인증평가에서 중요사항으로써 감염관리를 다루면서 환자를 위한 감염 노출 예방만이 아닌 의료종사자의 감염 노출예방에도 관심이 높아지고 있다[3]. 선행연구에서는 감염예방 지침만 충분히 실천하여도 병원감염의 1/3 이상이감소한다고 보고하였다[5].
	가장 쉬운 감염 예방법으로 손 씻기가 꼽히는 이유는?	손 씻기는 손을 통해 인체로 각종 세균과 바이러스가 전파되는 경로를 막을 수 있는 가장 쉬운 감염 예방법이다[10]. 인체에서 손은 각종 유해 세균에 가장 많이 접촉을 하는 부위로 손 씻기를 통하여 70% 예방이 가능하다. 그러나 2016년 발표에 의하면 올바른 손 씻기를 하는 국민은 10명 중 4명뿐이라고 하였다[11].
