This is a descriptive study investigating the relationships between sexual function, depression, and sleep quality in male hemodialysis patients. The participants of this study consisted of 134 outpatients who were diagnosed with end stage chronic kidney disease and who had undergone hemodialysis. The data were collected from June 10, 2019, to October 30, 2019. The collected data were then analyzed using descriptive statistics, the mean from independent sample t-tests, one-way analysis of variance (ANOVA), the Scheffe test, and Pearson's correlation coefficient using SPSS version 21.0. The lower the overall satisfaction, the lower the sexual function and the higher the degree of depression (r=.-19, p=.028). The greater the depression, the lower the quality of sleep (r=.60, p<.001). This study shows it is necessary to develop a program including symptom management to reduce depression in male hemodialysis patients. When developing nursing interventions to improve sexual function and quality of sleep, it will be necessary to develop a program that includes people who help the subjects.

제안 방법

대상자의 일반적 특성으로는 연령, 결혼상태, 현재의 직업상태, 월평균 수입, 보험형태, 도움을 주는 사람을 함하였고, 질병관련 특성으로는 투석기간, 투석횟수, 기저질환 및 약물복용 여부, 음주여부, 흡연여부, 혈액투석 이후로 성상담 여부 및 사유, 앞으로 성상담의 참여의향, 성상담을 누구에게 받을 것인가, 성기능 개선 약물 복용 여부, 한 달 동안 힘들었던 증상을 조사하였다.

대상 데이터

본 연구는 만성 신부전을 진단 받고 서울시 소재 종합 병원의 인공신장실에서 혈액투석을 받는 대상자를 편의 표집하였다. 만 19세 이상의 성인 남성으로, 신장내과 전문의에게 만성신부전을 진단 받고, 혈액투석 치료를 주 2회 이상 정기적으로 시행한지 2개월 이상 된 자를 대상으로 했으며, 과거 척수 손상에 의한 하반신 마비 상태인 자는 제외하였다[8,10,24]. 본 연구를 위한 예상 표본 수는 G-power 3.
본 연구는 2019년 6월 10일부터 10월 30일까지 진행했으며 S시 소재 1개 종합병원의 인공신장실에서 혈액 투석을 받는 환자를 대상으로 시행했다. 대상자에게 연구 목적을 설명하고, 동의를 구한 뒤 대상자가 직접 설문지를 작성하도록 했다.
95로 양측 검정 결과 134명이었다[19]. 탈락률을 10%로 고려하여, 147명에게 설문을 의뢰했고 수집된 자료 중 선정기준에 맞지 않거나 불충분하게 응답한 자료 등을 제외하여 최종분석 대상자는 134명이었다.

데이터처리

1) 대상자의 일반적 특성 및 질병관련 특성은 실수와 백분율 및 평균과 표준편차로 분석했다.
2) 대상자의 성기능, 우울 및 수면의 질은 실수와 백분율 및 평균과 표준편차로 분석했다.
3) 대상자의 일반적 특성 및 질병관련 특성에 따른 성기능, 우울 및 수면의 질 차이는 t-tset, ANOVA로 분석했고, 사후 검정은 Scheffe test로 분석했다.
4) 대상자의 성기능, 우울 및 수면의 질 간의 상관관계는 Pearson's correlation coefficients로 분석했다.

이론/모형

본 연구에서는 Radloff [27]가 개발한 Center for Epidemiologic Studies – Depression Scale (CES-D)를 Chon 등[28]이 번역한 한국어판 도구를 번역한 저자에게 이메일로 사용 승인을 받아 사용하였다. 총 20문항으로 우울 감정, 긍정적 감정, 신체 행동 둔화, 대인관계의 4가지 요인으로 구성되어 있다.
본 연구에서는 Rosen 등[25]이 개발한 International Index of Erectile Function (IIEF)를 Chung 등[26]이 번역한 한국어판 도구를 번역한 저자에게 이메일로 사용 승인을 받아 사용하였다. IIEF는 지난 한달 간의 성기능을 측정하기 위해 개발된 도구로 총 15개 문항으로 발기 능력, 성생활에 대한 전반적인 만족도, 성적 욕구, 성교만족도, 절정감으로 구성되어 있다.
수면의 질은 Buysse 등[29]이 피츠버그 수면센터에서 개발한 Pittsburgh Sleep Quality Index (PSQI)를 Sohn 등[30]이 번역한 한국어판 도구를 원저자 및 번역한 저자에게 각각 이메일로 사용 승인을 받아 사용하였다. PSQI는 최근 한 달 동안의 수면의 질 및 수면장애를 평가하는 자가 보고식 설문도구로 총 19문항으로 구성되었다.

성능/효과
