Purpose : This study aimed to present evidence by analyzing the characteristics and effectiveness of group art therapy interventions through an examination of domestic studies on group art therapy for older people with dementia. Methods : The database used DBpia, Riss, and Google Scholar, and the research period was from 2016 to November 2021. For the selected studies, the level of evidence was analyzed, bias evaluation was performed, and patient, intervention, comparison, and outcome were analyzed. For the evaluation of bias, the risk of bias assessment tool for non-randomized study (RoBANS) and Cochrane's risk of bias (RoB) were used. Results : As for the level of evidence of the included studies, level I consisted of five studies, and levels II and III each had one article. As a result of the bias evaluation of five studies through RoB, a "low risk of bias" was found for incomplete result data, selective result reporting, and others, except for four unclear evaluation areas. The "low risk of bias" ratio was 0~25 % in the evaluation of bias in two studies through RoBANS. For the evaluation tool, cognitive evaluation tool was used the most while mini-mental state examination-Korea was used the most frequently. For the intervention method, the most frequently used was group art therapy that employed recall in three studies, while collage, Korean painting, use of paper media, and procedural memory were used in each of the other studies. Each intervention was found to be significantly effective overall. Conclusion : This study provided clinical evidence by systematically reporting research on group art therapy for older people with dementia. In the future, it is necessary to check the effect of group art therapy on various areas other than cognition for older people with dementia. Moreover, the study should be conducted with the risk of bias sufficiently taken into consideration.

제안 방법

본 연구는 치매 노인을 위한 비약물중재방법으로 많이 사용되고 있는 집단미술치료 연구를 체계적 고찰하였다. 국내 치매 노인을 대상으로 집단미술치료 중재를 실시한 연구를 7편 포함하여, 근거수준, 비뚤림평가 등을 분석하였고, PICO에 따라 분류하고 분석하였다. 이를 통해 치매 노인을 위한 집단미술치료에 대한 임상적 근거를 마련하고 추후 연구방향을 제시하기 위한 기초를 마련하고자 하였다.

본 연구는 치매 노인을 위한 집단미술치료에 대한 국내 연구를 고찰하여 집단미술치료 중재들의 특성과 효과성을 분석하여 근거를 제시하기 위하여 총 7편의 논문을 중심으로 체계적 고찰을 실시하였다. 포함된 문헌의 질적 수준은 level Ⅰ이 5편으로 가장 많았다.
집단미술치료의 효과를 분석하기 위한 평가도구를 분석한 결과, 인지평가, 정신건강평가, 신체 평가, 삶의 질 평가를 사용하였다. 선행연구에서는 중재의 효과크기를 본 결과 삶의 질에서 제일 큰 효과가 보였다(Han & Moon, 2019).

대상 데이터

검색용어는 ‘치매 AND 집단미술’, ‘파킨슨 AND 집단미술’, ‘루이소체 AND 집단미술’, ‘혈관성 AND 집단미술’, ‘전측두엽 AND 집단미술‘을 사용하였다. 각 데이터베이스에서 검색된 총 874개의 문헌 중 3명의 저자가 제목과 초록을 검토하여 아래의 포함기준 및 배제기준에 따라 문헌을 선정하였다. 데이터베이스 간 중복된 논문은 제외시켰으며 총 7개의 연구가 최종적으로 선정되었다(Fig 1).
각 데이터베이스에서 검색된 총 874개의 문헌 중 3명의 저자가 제목과 초록을 검토하여 아래의 포함기준 및 배제기준에 따라 문헌을 선정하였다. 데이터베이스 간 중복된 논문은 제외시켰으며 총 7개의 연구가 최종적으로 선정되었다(Fig 1).
체계적 고찰을 실시하기 위하여 2021년 11월까지 DB pia, Google scholar, RISS 의 온라인 데이터베이스에 등록된 논문을 검색하였다. 검색용어는 ‘치매 AND 집단미술’, ‘파킨슨 AND 집단미술’, ‘루이소체 AND 집단미술’, ‘혈관성 AND 집단미술’, ‘전측두엽 AND 집단미술‘을 사용하였다.
최종 분석에 포함된 연구는 총 7편이었다. 연구형태별로는 Ⅰ수준의 무작위 대조연구 5편(71 %), Ⅱ 수준의 코호트 연구 1편(14 %), Ⅲ 수준의 전-후연구 1편(14 %)이었다(Table 1, 2).

이론/모형

본 연구에서 분석한 총 7개의 연구 중 사용된 평가도구는 총 22개이었으며 한 연구에 두 가지 이상 사용된 경우도 각각의 평가도구로 포함하였다. 이 중 가장 많이 사용된 평가도구는 인지기능을 평가하는 MMSE-K(minimental state examination-Korea)로 총 4편의 연구에서 사용하였다. 다음으로는 기억력을 평가하기 위한 도구로 ECR(enhanced cued recall), 언어적 기억력을 평가하는 서울 언어학습검사(Seoul verbal learning test), 시각적 기억력을 검사하는 레이 복합 도형검사(rey complex figure test)이었다.

성능/효과

회상을 활용한 집단미술치료, 종이매체를 활용한 집단미술치료, 콜라주 프로그램을 이용한 집단미술치료, 한국화 매체를 활용한 집단미술치료, 절차 기억을 통한 집단미술치료를 적용한 치매 노인은 대조군에 비해 인지기능, 우울, 자기표현, 자아통합감이 유의미하게 향상되었다. 전-후 연구인 세시풍속 집단미술치료 연구는 대조군은 없었지만 치매 노인들의 인지기능, 우울, 표현능력이 유의하게 향상된 것으로 나타났다(Table 4).
회상을 활용한 집단미술치료, 종이매체를 활용한 집단미술치료, 콜라주 프로그램을 이용한 집단미술치료, 한국화 매체를 활용한 집단미술치료, 절차 기억을 통한 집단미술치료를 적용한 치매 노인은 대조군에 비해 인지기능, 우울, 자기표현, 자아통합감이 유의미하게 향상되었다. 전-후 연구인 세시풍속 집단미술치료 연구는 대조군은 없었지만 치매 노인들의 인지기능, 우울, 표현능력이 유의하게 향상된 것으로 나타났다(Table 4).

후속연구

대상자는 경증치매를 대상으로 한 연구가 가장 많았고, 가장 많이 사용한 중재 방법은 회상 중심 중재, 평가도구는 인지 평가도구가 가장 많았다. 각 중재의 효과는 전반적으로 유의미하게 나타났으나, 추후 연구에서는 다각적인 측면으로 인지 외 다양한 영역에서 집단미술치료의 효과를 검증할 필요가 있다.

RoB와 RoBANS로 비뚤림 평가를 실시하였으나, 본문에 확실하게 나와 있지 않아 확인할 수 없는 평가 요소가 많았다. 추후 연구에서는 편향 위험을 충분히 고려하여 연구를 진행하고, 이에 대한 정보를 충분히 제시할 필요가 있다. 대상자는 경증치매를 대상으로 한 연구가 가장 많았고, 가장 많이 사용한 중재 방법은 회상 중심 중재, 평가도구는 인지 평가도구가 가장 많았다.
