Purpose : This study systematically assessed 9 emotion-function programs for schizophrenic individuals that have been implemented over the past 10 years in Korea and abroad, with the objective of providing clinical grounds and data for further programs. Methods : This study identified 2,558 studies from among those registered in the RISS, KISS, EMbase, PubMed and Scopus databases from 2013 to September 2023, using 'schizophrenia' AND 'emotion' AND ('program' OR 'therapy') as keywords. Of the 2,558 studies retrieved from electronic databases, 602 duplicate papers were excluded. After this process, 1,915 papers that did not meet the study's inclusion and exclusion criteria were excluded based on their titles and abstracts. Following a review of the texts of the remaining 41 papers, nine papers were finally selected, excluding 32 papers according to the selection and exclusion criteria. Results : Programs for the emotional function of patients with schizophrenia have continued to be researched. The results of an evaluation on the quality of the selected literature confirmed that a majority of the reviewed items were "uncertain", confirming that there was a risk of bias. The interventions for emotions included art, cognitive-behavioral, and physical programs, of which art programs were frequently applied. Most of the interventions were implemented in hospitals. The dependent variables for evaluating emotions included emotional expression, depression, anxiety, and positive emotions, and among them, emotional expression was used most frequently. Accordingly, the Berkeley expressivity questionnaire was commonly used as an evaluation tool. Conclusion : This study is significant for intending to analyze domestic and overseas programs for the emotions of patients with schizophrenia, thereby proposing the direction for planning and applying interventions in clinical settings. In follow-up studies, it will be necessary to develop a protocol that confirms interventions according to the settings and patient characteristics of programs and expands the scope of literature reviews.

대상 데이터

이후 제목과 초록으로 선정기준과 제외기준에 부합하지 않는 논문 1,915편을 배제하였다. 41편의 전문을 확인하여 실험설계, 대상자, 종속변수, 원문이 열리지 않는 논문 등 기준에 적합하지 않은 문헌 32건을 제외한 최종 9편의 문헌을 선정하였다. 검색 및 선정 과정은 Preferred reporting items for systematic reviews and meta-analysis (PRISMA)를 사용하여 나타내었다(Fig 1).
본 연구는 2013년부터 2023년까지 출간된 국내외 연구를 대상으로 하였다. 검색 데이터베이스로 RISS, KISS, EMbase, PubMed, Scopus를 사용하였다.
본 연구는 최근 10년간 진행된 조현병 환자의 정서를 위한 국내외 프로그램 9편을 아래와 같이 분석하였고, 이에 따른 중재 프로토콜을 제시하여 임상에서의 기초 자료로 활용하고자 하였다. 최종 선정된 9편의 논문은 2013년~2017년 연구 4편, 2019년~2021년 연구 5편이었다.
본 연구는 최근 10년간 진행된 조현병 환자의 정서를 위한 국내외 프로그램 9편을 아래와 같이 분석하였고, 이에 따른 중재 프로토콜을 제시하여 임상에서의 기초 자료로 활용하고자 하였다. 최종 선정된 9편의 논문은 2013년~2017년 연구 4편, 2019년~2021년 연구 5편이었다. 국내외로 살펴보았을 때, 국내 연구는 4편, 해외 연구는 5편으로 국내외 모두 조현병의 정서와 관련된 연구가 지속적으로 이루어지고 있음을 확인할 수 있었다.

이론/모형

41편의 전문을 확인하여 실험설계, 대상자, 종속변수, 원문이 열리지 않는 논문 등 기준에 적합하지 않은 문헌 32건을 제외한 최종 9편의 문헌을 선정하였다. 검색 및 선정 과정은 Preferred reporting items for systematic reviews and meta-analysis (PRISMA)를 사용하여 나타내었다(Fig 1).
선정된 9편의 연구의 질적 수준 평가는 Cochrane group에서 개발한 Risk of bias(ROB)를 사용하였다. ROB 평가는 무작위 배정순서 생성, 배정순서 은폐, 참여자와 연구자 눈가림, 결과 평가자 눈가림, 불충분한 결과 처리, 선택적 결과보고, 다른 잠재적 비뚤림의 위험의 총 7가지 영역을 측정한다.
정서를 평가하기 위한 변수로 정서표현, 우울, 불안, 긍정정서를 사용하였다. 이 중 정서표현이 가장 많이 사용되었으며, 평가도구로는 Berkeley 표현질문지(Berkeley expressivity questionnaire; BEQ)를 많이 사용하였다. 정서표현을 위해 예술 프로그램, 신체 프로그램 순으로 많이 적용되었고, 모두 유의한 효과를 나타내었다.
9편의 문헌 중 정서와 관련된 종속변수를 살펴본 결과, 정서 표현이 4편(44 %)이 가장 많았고, 우울 3편(33 %), 불안과 긍정 정서가 각 1편(11 %)으로 나타났다. 정서 표현은 모두 Berkeley 표현질문지(Berkeley expressivity questionnaire; BEQ)를, 불안은 상태-특성 불안척도(state-trait anxiety inventory; STAI)를, 긍정정서는 긍정정서 및 부정정서 척도(positive affect and negative affect schedule; PANAS)로 측정하였다. 우울은 조현병을 위한 Calary 우울 척도(Calary depression scale for schizophrenia; CDSS), Beck의 우울척도(Beck depression inventory; BDI) 순으로 평가도구 사용이 많았다(Table 4).

성능/효과

우울을 위해서는 인지-행동 프로그램, 예술 프로그램 순으로 많이 적용되었고, 예술 치료프로그램은 유의한 효과를 나타내었으나 인지-행동 프로그램은 일부에서 유의한 효과를 나타내지 않았다. 불안을 위해 신체 프로그램을 적용하였고 유의한 효과를 나타내었다. 긍정정서를 위해서는 인지-행동 프로그램을 적용하였으나 유의한 효과를 나타내지 못하였다.

조현병 환자의 정서를 위한 프로그램은 지속적으로 연구되고 있었으나 분석된 문헌의 질을 평가한 결과 대다수의 항목이 ‘불확실’로 나타나 편향의 위험이 있음을 확인하였다
효과를 살펴본 결과, 예술치료 프로그램과 신체 프로그램은 모두 통계적 유의미한 효과를 나타내었고, 인지행동 프로그램은 3편 중 1편만 통계적으로 유의미한 효과를 나타내었다(Table 3).

후속연구

인지-행동 프로그램은 정서에 대한 효과를 입증하기엔 미흡하여 향후 연구의 제한점을 보완한 추가적 연구를 통해 분석이 필요할 것으로 본다. 또한 추후에도 조현병 환자의 정서를 위한 프로그램을 위해 환경 및 환자 특성에 따른 중재를 확인하고 문헌 고찰의 범위를 확장한 프로토콜의 개발이 이루어져야 할 것이다.
본 연구는 조현병 환자의 정서를 위한 국내외 프로그램을 분석하였지만 국문과 영문으로 작성된 문헌만을 분석하여 일반화하는 것에 어려움이 있다. 또한 중재가 적용된 병원과 지역사회 환경에 있는 환자의 차이를 구분하지 못했다.
긍정정서를 위해서는 인지-행동 프로그램을 적용하였으나 유의한 효과를 나타내지 못하였다. 인지-행동 프로그램은 정서에 대한 효과를 입증하기엔 미흡하여 향후 연구의 제한점을 보완한 추가적 연구를 통해 분석이 필요할 것으로 본다. 또한 추후에도 조현병 환자의 정서를 위한 프로그램을 위해 환경 및 환자 특성에 따른 중재를 확인하고 문헌 고찰의 범위를 확장한 프로토콜의 개발이 이루어져야 할 것이다.
이에 유효성 결과를 일반화시키는데 제한이 있을 것으로 보인다. 추후 무작위 대조군 설계의 보고지침인 CONSORT(consolidated standards of reporting trials)를 통한 연구를 진행하여 프로그램의 효과성에 대한 근거를 충분히 제시하여 연구의 질을 높이는 것이 사료된다.
그러나 본 연구는 조현병 환자의 정서를 위한 국내외 프로그램들을 분석하여 임상 현장에서의 중재 계획 및 적용에 대한 방향을 제시하고자 한 것에 의의가 있다. 후속연구에서는 프로그램의 환경 및 환자 특성에 따른 중재를 확인하고 문헌 고찰의 범위를 확장한 프로토콜의 개발이 필요할 것으로 본다.
