Objectives: Since infertility has been big social issue in Korea, this study aims to analyze the domestic study trends in Korean Medicine (KM) for female infertility treatment. Methods: Case studies applying KM treatment on female infertility were selected through 5 domestic journal databases. General characteristics, interventions, outcomes and results and the quality of the reports were analyzed. Especially, the quality assessments of studies were made using CAse REport (CARE) guideline and Joanna Briggs Institute (JBI) critical appraisal checklist. Results: 14 studies (22 cases) were finally selected for the analyzation. The mean age of the participants was 35.1 and the most common factor of female infertility in this study was Uterine factor. Herbal medicine was applied in all studies, and acupuncture was also used frequently. Most cases reported pregnancy of the participants. According to quality assessment, 'Diagnostic challenges', 'Intervention adherence and tolerability', 'Adverse and unanticipated events', 'Patient perspective' and 'Informed consent' were showed low reporting rates. Conclusions: Korean Medicine treatment for female infertility is expected to be effective. In quality evaluation, there were some items with low reporting rates. Further clinical studies have to be conducted to establish the evidence for the treatment.

대상 데이터

검색결과 총 302편의 연구가 검색되었고, 165개의 중복된 연구를 제거하여 총137편의 결과를 얻었다. 137편 연구의 제목과 초록을 검토하여, 난임과 연관이없는 연구 50편, 한의학적 치료와 관련없는 연구 31편, 실험 연구 1편, 여성을대상으로 하지 않은 연구 3편, 연구 디자인이 증례보고가 아닌 경우 37편, 연구결과에 대한 설명 및 치료 방법에 대한인과성 설명이 부족한 연구 1편을 제외하고 총 14편의 연구를 선정하였다(Fig. 1).
14편 연구의 연구 대상 환자는 총 22명으로, 연령은 최소 27세에서 최대 41세로 평균 35.1세였다. 난임력은 최소 6개월 미만, 최대 6년 이상으로 6개월 미만이 4명, 1년 미만이 1명, 1년 이상이 6명, 2년 이상이 6명, 3년 이상이 3명, 6년 이상이 2명이었다.
14편의 연구에 포함된 연구 대상 환자는 총 22명으로, 이전에 출산을 하여 자녀가 있는 경우가 3례였고 그 외 19례는 모두 자녀가 없었다. 연구 대상자의 평균 연령은 35.
국내 데이터베이스 검색을 통해 난임에 한의학적 치료를 시행한 임상 연구를 검색하였고, 선정기준과 제외기준에 따라 최종적으로 10편의 증례 연구와 4편의 증례군 연구, 총 14편의 연구가 분석 대상으로 포함되었다.
여성 난임 치료를 위해 한의학적 중재를 사용한 국내 임상 논문을 대상으로 하였다. 중복 게재된 경우를 제외하고, 검색된 논문의 제목 및 초록을 확인하여 난임의 한의학적 치료와 무관한 연구, 여성만을 대상으로 하지 않은 연구, 프로토콜 연구, 문헌 분석 연구, 증례보고가 아닌 경우를 제외하였다.
게재된 학회지는 대한한방비만학회지에 1편, 그 외 13편은 모두 대한한방부인과학회지에 발표되었다. 연구에 포함된 총 14편의 연구 중 증례 보고가 10편, 증례군 연구가 4편이었고, 증례군 연구의 증례 수는 4례가 2편, 2례가 2편이었다(Table 1).
1. 최종적으로 선정된 연구는 총 14편으로, 대한한방비만학회지에 보고된 1편의 연구를 제외하면 모두 대한한방부인과학회지에 게재되었다. 연구에 포함된 환자 수는 총 22명으로, 연령은 최소 27세에서 최대 41세로 평균 35.
평가지표를 분석한 결과, 임신을 확인한 경우가 22례 중 22례로 모든 연구에서 활용되었다. 그 다음으로 출산 확인이 11례, 제반 증상에 대한 시각적 상사척도(Visual Analogue Scale, VAS)를 사용한 증례가 5례였다.

데이터처리

CARE 지침은 본래 연구의 질 평가 도구가 아닌, 증례 보고 지침이라는 한계가 있기 때문에, Joanna Briggs Institute(JBI) critical appraisal checklist를 활용하여 추가적인 질 평가를 시행하였다. JBI critical appraisal checklist는 전문가 합의를 통해 개발된 질 평가 도구로, 증례 보고(case report)의 경우 8개 항목, 증례군 연구(case series)의 경우 10개 항목으로 구성된 도구이다13).
JBI critical appraisal checklist를 활용하여 증례 보고 10편과 증례군 연구 4편에 대하여 추가적인 질 평가를 시행하였다(Table 6, 7). 증례보고 10편에 대한 질적 평가 및 세부항목별 분석 상, 7번 ‘이상 반응 및 예상치 못한 사건에 대해 보고하였는가?'(100%)의 항목에서 50% 이상의 연구들이 해당 항목을 분석하지 않았다.
5. 문헌의 질 평가는 CARE 점검표 와 JBI critical appraisal checklist를 활용하였다. CARE 점검표 기반의 평가 결과, ‘진단적 한계’, ‘중재 순응도 및 내약성’, ‘이상 반응 및 예상치 못한 사건’, ‘적절한 시점에 중재에 대한 환자의 의견을 공유', ‘환자의 동의’ 등의 항목에서 낮은 보고율을 보여 추후 증례보고 작성 시 해당 항목들을 특히 주의하여 보고해야 할 것으로 보인다.
최종 선정된 14편의 연구에 대해, CARE 점검표를 활용하여 문헌의 질을 평가하였다(Table 5). 세부 항목별로 분석한 결과, 8b번 ‘진단적 한계’(100%), 10c번 ‘중재 순응도 및 내약성’(78.

성능/효과

VAS의 경우, 월경통이나 불면, 불안, 소화불량 등의 증상에 대하여 평가를 하였고 VAS를 활용한 모든 연구에서, 치료에 따라 증상이 호전되었음을 확인하였다. AMH의 경우, 난소기능저하가 난임의 원인으로 진단받은 증례에서 활용되었으며 AMH를 활용한 3례 모두에서 수치 증가를 확인하였다.

결과 평가 지표로는 모든 연구에서 임신 확인을 하였으며, 치료가 진행 중인 1례를 제외하고 모든 경우에서 임신이 되었음을 확인하였다. 21례 중 10례는 임신확인 방법에 대해 서술하지 않았으며, 그 외 8례에서는 초음파를 통한 임신낭 확인이 이루어졌다.
4. 결과 평가지표로는 치료를 진행 중인 1례를 제외한 모든 증례에서 임신을 확인하였고, 출산확인(50%), 제반 증상에 대한 VAS 척도(22.7%), AMH 검사(13.6%)를 활용하였다. 월경통이나 불안, 불면 등에 대한 VAS 척도는 대부분의 연구에서 호전 양상을 보였으며 AMH 검사 상 수치의 상승을 보고하였다.
난임 치료에 가장 많이 활용된 한의학적 치료 방법은 한약치료와 침치료로, 한약 치료는 총 22례 중 22례에서 사용되었고, 침치료는 총 22례 중 15례에서 사용되었다(Table 2).
3. 난임 치료에 가장 많이 활용된 한의학적 치료법은 한약치료(100%)와 침치료(62.6%)였고 다음으로 뜸치료(59.1%)가 자주 활용되었다. 한약치료에 자주 사용된 처방은 勝金丹이었고 大營煎加減, 逍遙散加減, 淸胞逐瘀湯, 加味補血湯 등이 그 다음으로 많이 활용되었다.
자궁 요인과 난관 요인이 결합된 경우가 2례로 파악되었다. 난임의 서양의학적 치료력 분석 결과, 난임 치료력이 없는 경우가 7례로 가장 많았고, 인공수정(Intrauterine Insemination, IUI)를 시행한 경우가 6례, 체외수정(In Vitro Fertilization, IVF)를시행한 경우가 4례, 배란유도제를 복용한 경우가 3례였으며 IUI와 IVF를 다수복합적으로 시행했던 경우가 2례였다(Table 1).
난임력은 최소 6개월 미만, 최대 6년 이상으로 6개월 미만이 4명, 1년 미만이 1명, 1년 이상이 6명, 2년 이상이 6명, 3년 이상이 3명, 6년 이상이 2명이었다. 난임의 원인에 대한 서양의학적 진단으로는 자궁 요인이 7례로 가장 많았으며, 난소기능저하가 6례, 원인불명이 5례, 배란 요인이 2례였다. 자궁 요인과 난관 요인이 결합된 경우가 2례로 파악되었다.
난임력은 1년 미만이 5례, 3년 이상이 5례였으며 그 외는 모두 1년 이상이었으며 1년 이상 3년 미만의 경우가 12례로 가장 많았다. 난임의 원인으로 자궁요인이 7례로 가장 많았으며, 원인불명이 6례, 난소기능저하가 5례였고 배란요인이 2례, 자궁 요인과 난관 요인이 결합된 경우가 2례였다.
본 연구는 난임 환자들에 한의학적 치료를 시행한 다양한 국내 임상 증례 연구들을 분석하였고, 그 결과 총 14편이 연구에서 대체로 임신에 성공한 것을 확인하였다. 비록 본 연구에 포함된 연구들이 모두 증례 연구이며 비교군이 없다는 한계가 있으나, 실제 임상에서 접하는 환자들과 활용되는 치료법의 그 경향성을 파악하였다는 의의가 있다.
5%)로 가장 많았다. 서양의학적으로 진단받은 난임의 원인으로 자궁 요인이 7례(31.8%)로 가장 많았으며, 난소기능저하가 6례(27.3%), 원인불명이 5례(22.7%)로 많이 나타났다. 서양의학적 치료력으로는 배란유도제 복용이나 IUI, IVF 시행 등이 있었다.
약침 치료는 총 22례 중 6례에서 사용되었으며, 신양허 약침이 4례로 가장 많이 활용되었고, 자하거 약침이 3례에서 사용되었다. 그 외 삼정 약침, 중만어혈약침, 중성어혈 약침, 팔강약침이 사용되었다(Table 3).
이 외에 JBI checklist를 활용한 질 평가 결과, 증례 보고 연구의 경우 CARE 점검표를 활용한 평가와 동일하게 ‘이상반응 및 예상치 못한 사건에 대해 보고하였는가?'의 항목에서 낮은 보고율을 보였고, 그 밖에 ‘중재 또는 치료 절차에 대해 명확히 기술하였는가?'의 항목에서도 50% 이상에서 보고하지 않았음을 확인하였다

후속연구

CARE 점검표 기반의 평가 결과, ‘진단적 한계’, ‘중재 순응도 및 내약성’, ‘이상 반응 및 예상치 못한 사건’, ‘적절한 시점에 중재에 대한 환자의 의견을 공유', ‘환자의 동의’ 등의 항목에서 낮은 보고율을 보여 추후 증례보고 작성 시 해당 항목들을 특히 주의하여 보고해야 할 것으로 보인다
비록 본 연구에 포함된 연구들이 모두 증례 연구이며 비교군이 없다는 한계가 있으나, 실제 임상에서 접하는 환자들과 활용되는 치료법의 그 경향성을 파악하였다는 의의가 있다. 난임의 한의학적 치료에 대한 효과와 안전성 평가를 위해 향후 질적 수준이 높은 증례 연구와 대규모 임상 연구가 필요할 것으로 생각된다.
증례 연구에 대한 JBI checklist 평가결과 ‘이상 반응 및 예상치 못한 사건에 대한 보고’, ‘중재 또는 치료 절차에 대한 명확한 기술’의 항목에서 낮은 보고율을 보였고, 증례군 연구에 대한 평가 결과 상 참가자 포함 및 평가와 관련된 항목에서 불충분하게 보고한 것으로 나타나 추후 주의가 필요하다.
