A Review of Acupuncture Treatment Methods for Polycystic Ovary Syndrome

Objectives: The purpose of this study is to review the acupuncture treatment for Polycystic ovary syndrome (PCOS) in women. Methods: We searched articles in 3 search engines with keywords related to 'Polycystic ovary syndrome', 'PCOS', and 'Stein-leventhal' in February 2024. Clinical researches and case reports that used acupuncture on PCOS were included. Animal studies and non clinical data were excluded. Data on acupuncture treatment such as methods, site, duration, frequency, and period were analyzed. Results: Of 60 selected articles, there were 51 randomized controlled trials, 5 clinical trials and 4 case reports. Studies were conducted using manual acupuncture, electro acupuncture, auricular acupuncture, thread embedding acupuncture, warm needling and laser acupuncture. Most studies used more than one acupoint, and there were 78 acupoints selected for acupuncture treatment for PCOS. The most commonly used acupoint was 三陰交 (SP6)(n=50). By analyzing the network of acupoints, 關元 (CV4), 氣海 (CV6), 中脘 (CV12), 三陰交 (SP6), 血海 (SP10), 天樞 (ST25), 足三里 (ST36) were located in center of the network. The mean treatment time, number of treatments, and duration were 28.63±4.48 minutes, 34.52±29.26 times, and 98.18±38.25 days. Conclusions: The results of this study could be useful in establishing the evidence for performing standardized acupuncture treatment for Polycystic ovary syndrome.

전체 연구의 주요 결과지표를 배란/임신 지표, PCOS 증상지표, 그리고 기타지표로 구분하였다(Table 1, 2). 주요 결과지표가 명확하게 제시되지 않은 논문은 연구 주제와 부합하는 결과를 선정하여 주요 결과지표로 간주하였다.
일반 침 치료를 활용한 40편의 연구에서는 關元(CV4)이 26회, 三陰交(SP6)가 24회, 中極(CV3), 氣海(CV6), 子宮(EX-CA1), 足三里(ST36)가 각각 14회 순으로 활용되었다. 전침 치료를 활용한 36편의 연구에서는 三陰交(SP6)가 22회, 氣海(CV6)가 19회, 足三里(ST36)가 15회, 中極(CV3)이 13회, 子宮(EX-CA1), 關元(CV4)이 각각 12회, 天樞(ST25), 歸來(ST29)가 각각 11회 순으로 활용되었다. 이를 통해 치료법 간 사용된 경혈의 빈도에는 약간의 차이가 있지만, 경혈의 종류는 유사한 것을 알 수 있다.
침 치료 외에 사용된 치료 방법으로는 한약 치료, 양방 치료, 뜸 치료, 섭생치료 등이 있었다(Table 5). 침 치료 방법으로는 일반 침 치료, 전침 치료, 이침 치료, 매선 치료, 온침 치료, 레이저침 치료가 활용되었다.
사용된 경혈을 명시하지 않은 논문, 한국어, 중국어, 영어 외 다른 언어로 작성된 논문, 원문을 구할 수 없는 논문은 제외하였다. 침 치료의 범주는 일반 침치료 이외에 전침, 매선침, 이침, 약침, 레이저 침 등 다양한 형태의 침 치료를 포함하였다.

대상 데이터

최종 대상 문헌 선정 및 제외는 독립된 2명의 연구자가 수행하였다. 검색된 논문 중 침 치료가 치료 중재로 사용되었고, 여성의 PCOS를 대상으로 하는 임상연구 및 증례 보고 논문을 포함하였다. 이 연구는 침 치료의 효과를 평가하기보다는 활용되고 있는 침 치료법, 경혈의 종류 등의 확인을 주목적으로 한다.
비무작위 대조군 및 비대조군 임상연구는 각각 1편, 2편, 2편이 발행되었다. 증례 보고는 2000~2009년 사이에 1편, 2010~2019년 사이에 3편이 발행되었다.
1. 총 60편의 논문이 선정되었으며, 이 중 무작위 대조군 임상연구가 51편, 비무작위 대조군 및 비대조군 임상연구가 5편, 증례 보고가 4편이었다. 또한 국가별로는 중국에서 46편, 이란에서 4편, 스웨덴과 인도네시아에서 각 3편, 미국과 이집트에서 각 2편, 캐나다와 호주에서 각 1편이 발행되었다.
총 60편의 연구가 선별되었고(Fig. 1), 기본 특성은 다음과 같다(Table 2, 3).
최종 대상 문헌 선정 및 제외는 독립된 2명의 연구자가 수행하였다. 검색된 논문 중 침 치료가 치료 중재로 사용되었고, 여성의 PCOS를 대상으로 하는 임상연구 및 증례 보고 논문을 포함하였다.

데이터처리

42편의 논문에서 치료 기간과 빈도를 언급하였다. 총 치료 횟수를 제시하지 않은 논문은 제시된 치료 기간과 빈도를 이용하여 횟수를 추정하였다. 18편의 논문에서는 환자의 월경주기를 치료 단위로 제시하거나 기간, 빈도 중 한 가지만 언급하였기 때문에 총 치료 횟수를 추정할 수 없었다.
42편의 논문에서 치료 기간과 빈도를 언급하였다. 총 치료 횟수를 제시하지 않은 논문은 제시된 치료 기간과 빈도를 이용하여 횟수를 추정하였다. 18편의 논문에서는 환자의 월경주기를 치료 단위로 제시하거나 기간, 빈도 중 한 가지만 언급하였기 때문에 총 치료 횟수를 추정할 수 없었다.

성능/효과

41편의 논문에서 배란/임신 지표를 주요 결과지표로 제시하였으며, 그 예시로는 월경주기, 배란율, 임신율, 유산율, 난소 용적 등이 있었다. 8편의 논문에서 PCOS 증상지표를 주요 결과지표로 제시하였으며, F-G score(Ferriman-Gallwey score), 다모증, 여드름 등이 있었다. 기타 주요결과지표로는 총 임상 유효율, 체질량지수(body mass index, BMI), 혈청 성호르몬 수치, 인슐린저항성(homeostatic model assessment for insulin resistance, HOMA-IR) 등이 있었다.
PCOS의 평가 면에서는 배란/임신 지표, 증상지표, 신체 계측 지표, 당 대사지표, 총 임상 유효율 등이 활용되었고, 1편의 논문을 제외한 모든 연구에서 주요 결과지표의 호전을 확인하였다. 9편의 연구에서 침 치료 후 부작용을 언급하였는데, 모두 근육통, 발적, 혈종 등 침 치료 시 발생할 수 있는 예측 가능한 부작용이 대부분이었다.
PCOS 유형별 치료 시간을 분석한 결과, 기타 특이사항이 없는 PCOS는 평균 27.9±6.61분(최소 20분~최대 30분), 비만형 PCOS는 평균 28.75±3.54분(최소 20분~최대 30분), 난임형 PCOS는 평균 24.29±9.32분(최소 15분~최대 40분), 보조생식술을 동반한 PCOS의 경우 평균 30.71±1.89분(최소 30분~최대 40분)으로 나타났다.
PCOS 유형별 치료 횟수를 분석한 결과, 기타 특이사항이 없는 PCOS는 평균 30.61±25.9 회(최소 6회~최대 120회), 비만형 PCOS는 평균 40.38±15.89회(최소20회~최대 60회), 난임형 PCOS는 평균 19±12.14회(최소 12회~최대 33회)로 나타났다

PCOS 유형별로는 기타 특이사항이 없는 PCOS는 평균 30.61±25.9회, 비만형 PCOS는 평균 40.38±15.89회, 난임형 PCOS는 평균 19±12.14회, 보조생식술을 동반한 PCOS의 경우 평균 5회로 나타나 유형별 치료 횟수의 차이를 확인할 수 있었다.

전침 치료 36회 중 전침 치료만 사용한 것은 10편이었고, 일반 침 치료와 함께 사용한 것이 21편, 이침 치료와 함께 사용한 것이 4편, 온침 치료와 함께 사용한 것이 2편, 매선 치료와 함께 사용한 것이 1편 있었다. 매선 치료 4회 중 매선 치료만 사용한 것은 2편이었고, 일반 침 치료와 함께 사용한 것이 1편, 전침 치료와 함께 사용한 것이 1편 있었다. 이침 치료 6회 중이침 치료만 사용한 것은 1편 있었고, 일반 침 치료와 함께 사용한 것이 1편, 전침 치료와 함께 사용한 것이 4편 있었다.
복수 사용된 경혈들 사이의 관계를 파악하기 위해 네트워크 중심성 분석을 하였을 때, 關元(CV4), 氣海(CV6), 中脘(CV12), 三陰交(SP6), 血海(SP10), 天樞(ST25), 足三里(ST36) 등의 경혈이 중심에 위치하여 여러 경혈과 함께 사용됨을 확인할 수 있었다. PCOS의 침 치료 시에는 빈용 경혈 및 네트워크 연결망에서 중심에 위치한 경혈을 우선적으로 조합하여 활용할 수 있겠다.
이상의 분석 결과를 토대로, PCOS 환자에게 일반 침 치료 및 2 Hz의 저주파 전침 치료를 병행하고, 三陰交(SP6), 關元(CV4), 氣海(CV6), 足三里(ST36), 天樞(ST25), 中脘(CV12) 등의 경혈을 선혈하여 30분 내외에 유침하며, 3개월 동안 주 3회 총 36회를 기준으로 침 치료 프로그램을 설정할 수 있을 것으로 사료된다. 다만 이 연구는 PCOS에 활용되고 있는 침 치료 방법에 대한 분석을 목적으로 하였기에, 연구의 유형을 전향적 임상연구로 제한하지 않고, 단순 증례보고까지 모두 포함하였다.
매선 치료 4회 중 매선 치료만 사용한 것은 2편이었고, 일반 침 치료와 함께 사용한 것이 1편, 전침 치료와 함께 사용한 것이 1편 있었다. 이침 치료 6회 중이침 치료만 사용한 것은 1편 있었고, 일반 침 치료와 함께 사용한 것이 1편, 전침 치료와 함께 사용한 것이 4편 있었다. 온침 치료는 2편의 논문 모두 일반 침치료 및 전침 치료와 함께 사용되었다.

일반 침 치료 40회 중 일반 침 치료만 사용한 것은 17편이 있었다. 전침 치료 36회 중 전침 치료만 사용한 것은 10편이었고, 일반 침 치료와 함께 사용한 것이 21편, 이침 치료와 함께 사용한 것이 4편, 온침 치료와 함께 사용한 것이 2편, 매선 치료와 함께 사용한 것이 1편 있었다. 매선 치료 4회 중 매선 치료만 사용한 것은 2편이었고, 일반 침 치료와 함께 사용한 것이 1편, 전침 치료와 함께 사용한 것이 1편 있었다.
침 치료 방법에 따라서는 일반 침 치료는 평균 100.17±38.91일, 전침 치료는 평균 87.03±3 .25일로 전침 치료가 일반 침 치료보다 치료 기간이 짧게 나타났다.

주 치료 중재법을 살펴보았을 때, 침 치료를 단독으로 사용한 것이 33편, 침치료와 기타치료를 함께 사용한 것이 32편이었다. 침 치료 외에 사용된 치료 방법으로는 한약 치료, 양방 치료, 뜸 치료, 섭생치료 등이 있었다(Table 5). 침 치료 방법으로는 일반 침 치료, 전침 치료, 이침 치료, 매선 치료, 온침 치료, 레이저침 치료가 활용되었다.

60편의 연구 모두 복수의 경혈을 사용하여 치료하였다. 표준경혈명을 사용하지 않은 이침혈을 제외하고, 총 78개의 경혈이 사용되었고, 이 중 가장 많이 사용된 경혈은 三陰交(SP6)였다. 다음으로는 關元(CV4), 氣海(CV6), 足三里(ST36), 中極(CV3), 子宮(EX-CA1), 歸來(ST29) 순으로 빈용되었다.
횟수가 범위로 제시된 경우 중간값을 설정하여 평균을 계산하였을 때, 42편 논문의 평균 침 치료 횟수는 34.52±29.26회였다

후속연구

하지만, 국내에서는 아직 PCOS의 침 치료를 주제로 한 임상 시험이 활발히 진행되고 있지 않다. PCOS 환자가 한의 치료에 신뢰성 있게 접근할 수 있도록 국내에서 활발한 임상연구가 시행될 필요가 있다.
복수 사용된 경혈들 사이의 관계를 파악하기 위해 네트워크 중심성 분석을 하였을 때, 關元(CV4), 氣海(CV6), 中脘(CV12), 三陰交(SP6), 血海(SP10), 天樞(ST25), 足三里(ST36) 등의 경혈이 중심에 위치하여 여러 경혈과 함께 사용됨을 확인할 수 있었다. PCOS의 침 치료 시에는 빈용 경혈 및 네트워크 연결망에서 중심에 위치한 경혈을 우선적으로 조합하여 활용할 수 있겠다.

또한 국외 데이터베이스인 ‘PUBMED’, ‘Cochrane Library Central Register of Controlled Trials’, ‘EMBASE’ 3개의 검색원만으로 검색을 진행하였기 때문에 보다 다양한 문헌을 확인하지 못한 한계가 있다.
한의 치료가 그 방안이 될 수 있으며, 특히 침 치료법은 비용과 부작용 면에서 필수적으로 활용 가능한 치료라고 생각된다. 향후 국내에서 PCOS의 침 치료에 대한 다양한 전향적 임상연구 및 근거의 수준이 높은 고찰 논문을 통해 재현 가능한 최선의 치료법을 도출하는 것이 필요하다고 사료된다.
다만 이 연구는 PCOS에 활용되고 있는 침 치료 방법에 대한 분석을 목적으로 하였기에, 연구의 유형을 전향적 임상연구로 제한하지 않고, 단순 증례보고까지 모두 포함하였다. 향후 침 치료의 유효성 및 안전성의 평가에는 유의할 필요가 있다. 또한 국외 데이터베이스인 ‘PUBMED’, ‘Cochrane Library Central Register of Controlled Trials’, ‘EMBASE’ 3개의 검색원만으로 검색을 진행하였기 때문에 보다 다양한 문헌을 확인하지 못한 한계가 있다.
