Abstract

ZusammenfassungIn dieser Arbeit wurde die Aussprache von zahnlosen Patienten untersucht, die festsitzenden oder abnehmbaren Zahnersatz mit und ohne Implantate tragen. Insgesamt nahmen bei dieser Untersuchung 138 Patienten teil. Die experimentelle Gruppe (n=113) wurde in 4 Untergruppen aufgeteilt. Die Gruppe FD/FFPi hatte 22 Patienten mit einer Oberkiefertotalprothese (=FD) und einer auf Implantaten befestigten Unterkieferrekonstruktion (=FFPi). Die Gruppe FFPi/ND hatte 27 Patienten mit einer auf Implantaten fixierten Oberkiefer‐rekonstruktion und Eigenbezahnung im Unterkiefer (=ND). Die Gruppe FD/ODi hatte 49 Patienten mit einer Oberkiefertotalprothese und einer Unterkieferhybridprothese auf 2 Implantaten (=ODi). Die Gruppe FFPi/FFPi hatte 15 Patienten mit einer auf Implantaten befestigten Rekonstruktion in beiden Kiefern. Die Ergebnisse der logopädischen Untersuchung in der experimentellen Gruppe wurde mit derjenigen einer Kontrollgruppe verglichen, in welcher alle Patienten eigenbezahnt waren (ND/ND; n=25). Ein ausgebildeter Pathologe für Logopädie führte ein standartisiertes klinisches Verfahren zur Untersuchung der Sprache durch. Daneben untersuchte man auch bei allen Patienten spezifische orale und prothetische Parameter. Man stellte fest, dass in 84% aller Patienten der experimentellen Gruppen ein oder mehrere Aussprachefehler auftraten. Das war signifikant häufiger als in der Kontrolgruppe, wo etwa die Hälfte aller Patienten leichte Sprachschwierigkeiten hatte. Man konnte keine klaren Einflüsse von spezifischen oralen und prothetischen Parametern identifizieren. Von diesen Resultaten schloss man, dass bei Patienten mit implantatgetragenen Rekonstruktionen Sprachschwierigkeiten häufiger auftreten als bei Patienten mit Eigenbezahnung.

Abstract

RésuméLa phonation a étéévaluée chez des patients édentés portant des prothèses dentaires fixes ou amovibles ancrées ou non sur des implants. Cent trente‐huit patients ont participéà cette étude. Le groupe expérimental (n=113) a été divisé en quatre sous‐groupes: FD/FFPi comprenant 22 patients avec une prothèse supérieure totale (=FD) et une prothèse inférieure totale fixée sur des implants (=FFPi), FPi/ND comprenant 27 patients avec une prothèse supérieure totalte fixée sur des implants et une dentition naturelle dans la mandibule (=ND), le sous‐groupe FD/ODI de 49 patients avec une prothèse totale supérieure et une prothèse inférieure sur deux implants (=ODi) et le sous‐groupe FFPi/FFPi de quinze patients ayant deux bridges sur implants. L’évaluation logopédique du groupe expérimental a été comparée avec celle d’un groupe contrôle de sujets ayant une dentition normale (ND/ND; n=25). Un processus clinique standardisé pour l’évaluation de la phonation a été effectué par un pathologiste de la phonation. De plus, des paramètres prothétiques et buccaux spécifiques ont été enregistrés chez tous les patients. Une ou plusieurs difficultés de prononciation apparaissaient chez 84% des patients du groupe expérimental. C’était significativement plus élevé que dans le groupe contrôle où la moitié des sujets avaient des déficiences de phonation. Aucune influence claire de paramètres prothétiques ou buccaux spécifiques n’ont pûêtre mis en évidence. Chez les patients ayant reçu des prothèses ancrées sur des implants le problème de phonation est plus souvent observé que chez les sujets ayant une dentition naturelle.
