목적: 네비게이션을 이용한 슬관절 전치환술과 고식적 방법에 의한 슬관절 전치환술에서 내반변형의 정도가 하지 및 치환물의 정렬에 미치는 영향을 분석하였다.
대상 및 방법: 2004년 1월부터 2005년 1월까지 술 전 하지의 역학적 축을 중심으로 내반 변형이 10˚ 이상인 환자 중 CT-free navigation system (Vector Vision?/SUP>, BrainLAB)을 이용하여 슬관절 전치환술을 시행한 49예(Ⅰ군)와 고식적 방법으로 슬관절 전치환술을 시행한 24예(Ⅱ군)를 대상으로 하였다. 하지의 역학적 축을 중심으로 한 내반 변형 정도에 따라 내반 변형이 10-20˚인 군(A군)과 20-30˚인 군(B군)으로 분류하였다. 모든 예에서 PFC Sigma삽입물을 사용하였다. 수술 전 하지의 역학적 축을 측정하였고 수술 후 두 명의 관측자가 각각 역학적 축과 미국 슬관절 학회의 방사선학적 평가 방법에 따른 삽입물의 위치를 측정하였다.
결과: Ⅰ군과 Ⅱ군 간의 역학적 축, 대퇴골 치환물의 관상면 정렬 각(α각), 경골 치환물의 관상면 정렬 각(β각), 경골 치환물의 시상면 정렬 각(δ각)은 유의한 차이가 없었으나, 대퇴골 치환물의 시상면 정렬 각(γ각)은 통계적으로 유의한 차이가 있었다(p<0.05). IA군과 IB군 간 역학적 축, α각, β각은 통계적으로 유의한 차이가 있었다(p<0.05). ⅡA 군과 ⅡB 군간 역학적 축, α각, β각은 통계적으로 유의한 차이가 있었다(p<0.05). 두 관측자간의 측정치는 강한 양적 선형 관계를 보였고 통계적으로 유의하였다(p<0.01).
결론: Ⅱ군 보다 Ⅰ군에서 시상면상 대퇴골 삽입물을 정확히 위치시킬 수 있었으며, Ⅰ군과 Ⅱ군 모두 술 전 내반변형이 10-20˚인 군 보다 20-30˚인 군에서 통계적으로 유의하게 술 후 역학적 축이 내반되는 경향이 있었다. 자동항법 장치를 이용할 경우 내반 변형이 심한 환자에서 대퇴골의 역학적 축에 대한 등록(registration)에 더 세심한 주의가 필요하며, 고식적 방법으로 할 경우 대퇴골의 내반 변형 정도에 따라 대퇴골 원위부의 외반 절제각의 증가가 필요하며, 정확한 위치에 골수강내 지침자를 삽입해야 할 것으로 사료된다.

Purpose: To compare the radiologic measurements of the mechanical axis and the implant position of Total Knee Arthroplasty (TKA) using a computer-assisted navigation system with those using conventional TKA in varus deformity. Materials and Methods: From January 2004 to January 2005, 49 TKAs using a CT-free navigation system (Vector Vision?/SUP>, BrainLab, Heirnstetten, Germany) (Group Ⅰ) and 24 TKAs using the conventional technique (Group Ⅱ) were performed on patients who had a preoperative varus deformity>10°. The patients were also subdivided into two groups, patients with a varus deformity <20° (group A) and patients with varus deformity>20° (group B). The PFC Sigma implants were used in both groups. The mechanical axis and implant position were measured by 2 observers according to the reontgenographic evaluation system of the American Knee Society. Results: There was no significant difference in α, β, δ angle and mechanical axis between group Ⅰ and Ⅱ. There was a significant difference in the γ angle between group Ⅰ and Ⅱ (p < 0.05). There was a significant difference in the α and β angle and mechanical axis between group IA and IB (p<0.05). There was a significant difference in the a angle and mechanical axis between group IIA and IIB (p<0.05). There was α positive correlation between the measured angle by the respective observers in all groups (p < 0.05). Conclusion: Patients with a preoperative varus deformity>20° tended to have more postoperative varus mechanical alignment than those with a preoperative varus deformity between 10° and 20° after TKA. More careful attention during the registration of the femoral mechanical axis should be paid in patients with a larger varus deformity in TKA using a computer-assisted navigation system. On the other hand, a reasonable mechanical valgus angle should be considered in femoral bone cutting for a varus deformity of the distal femur in conventional TKA. In addition, inadequate positioning of intramedullary rod should be recognized in conventional TKA.
