목적: 만성 후방십자인대 손상에서 변형된 tibial inlay 술식을 이용하여 늘어난 상태로 남아있는 후방십자인대를 긴장시키고 후방십자인대의 전외측 다발을 재건하는 새로운 술식을 소개하고 이 방법으로 치료하였던 환자의 결과를 보고하고자 한다.
대상 및 방법: 1998년 1월부터 2003년 8월까지 86명의 환자가 만성 후방십자인대 손상으로 늘어난 상태로 치유되어 남아있는 후방십자인대에 대해 긴장술 및 전외측 다발 재건술을 시행 받았으며 이 중 2년 이상 추시관찰이 가능하였던 52명의 환자를 대상으로 연구하였다. 후방십자인대의 긴장술은 앙와위에서 후내측 도달법을 통해 남아있는 후방십자인대의 경골부착부의 골편을 떼어내어 원위부로 이동시켜 긴장술을 시행하였고, 4다발의 자가슬괵근 또는 동종이식건을 이용하여 전외측다발의 재건술을 시행하였다. Telos 기구를 이용한 스트레스 방사선 사진 및 KT-1000 관절계측기를 이용한 최대 도수 전위 검사를 통해 객관적인 안정성을 평가하였고 IKDC 및 OAK score를 이용하여 임상적인 결과를 분석하였다. 이학적 검사로는 후방 견인 검사, 내반 긴장검사, 후외측 견인 검사와 30도와 90도 무릎관절 굴곡 상태에서 경골의 외회전 검사(Dial test)를 측정하였다.
결과: 스트레스 방사선 사진 및 KT 1,000 관절계측기를 이용한 최대 도수 전위 검사에서 건측과의 차이가 각각 10.4±2.0 ㎜에서 2.2±1.0 ㎜, 8.2±1.5 ㎜에서 1.9±1.0 ㎜로 감소하였다. 최종 IKDC 점수는 A가 11명(21.2%), B가 35명(67.3%), C가 6명(11.5%)이었다. 평균 OAK점수는 64.3±8.9에서 90.8±7.2점으로 향상되었다.
결론: 만성 후방십자인대 손상 시 늘어난 상태로 남아있는 후방십자인대의 긴장술 및 전외측 다발의 재건술을 통해 만족할 만한 임상 결과 및 아주 좋은 후방 안정성을 얻을 수 있었다.

Purpose : This report introduces a new method for tensioning the remnant PCL with a reconstruction of the anterolateral (AL) bundle of the PCL using a modified tibial inlay technique with an assessment of the outcome of this method in chronic PCL injury. Materials and Methods : From January 1998 to August 2003, eighty six patients was underwent tensioning of a laxed remnant PCL with a reconstruction of the anterolateral bundle of the PCL. Of these, fifty two patients who were followed up for more than 2 years were evaluated. Tensioning was performed using a distal transfer of the tibial attachment with the posteromedial approach in the supine position. The AL bundle of the PCL was reconstructed with 4 bundles of a hamstring autograft or tibialis anterior tendon allograft. The stability was assessed objectively using stress radiographs with the TelosⓇ device and the maximal manual test with the KT-1000 arthrometer. The clinical results were assessed by the IKDC (International Knee Documentation Committee) and OAK (Orthopadische Arbeitsgruppe Knie) scores. The posterior drawer test, varus stress test, posterolateral drawer test and dial test in 30° and 90° flexion were performed for a physical examination. Results : The average side to side difference of the posterior tibial translation in stress radiographs with the TelosⓇ device decreased from 10.4±2.0 ㎜ to 2.2±1.0 ㎜. The average side to side difference in the maximal manual test with the KT-1000 arthrometer also decreased from 8.2±1.5 ㎜ to 1.9±1.0 ㎜.
