연구배경: 본 연구에서는 한국인 제2형 당뇨병환자에서의 아스피린 저항성의 유병률을 조사하고 그 임상 특징을 알아보고자 하였다. 방법: 최근 3개월 동안 매일 아스피린 100 mg을 복용한 한국인 제2형 당뇨병환자 181명을 대상으로 하였으며, VerifyNow System으로 측정한 아스피린 반응 지수(ARU)가 550 이상인 경우 아스피린 저항성이 있다고 정의하였다. 신체계측 및 콜레스테롤, 중성지방, 고밀도 지단백 콜레스테롤, 저밀도 지단백 콜레스테롤, 당화 혈색소, 심전도, hsCRP, homocysteine, 당뇨병성 망막병증의 유무, 미세알부민뇨 유무 등을 검사하였으며, 말초혈관 동맥경화의 평가를 위해 발목상완동맥혈압비(ABI)와 cardio-ankle vascular index (CABI)를 측정하였다. 결과: 전체 181명의 환자 중 ARU 550 이상으로 아스피린 저항성을 보이는 환자는 17명으로 유병률은 9.4%이었다. 아스피린 저항성 군의 평균연령은 64.6 ± 10.6세이었고 아스피린 반응성 군의 평균 연령은 59.8 ± 8.1세로 두 군 사이에 유의한 차이를 보였다(P = 0.024). 그러나 연령을 제외한 임상 지표에서는 양 군 간의 유의한 차이는 없었다. 결론: 본 연구에서는 아스피린을 복용하고 있는 한국인 제2형 당뇨병환자에서 9.4%의 아스피린 저항성의 유병률을 관찰하였으며 연령을 제외한 다른 임상지표와의 연관성은 관찰할 수 없었다. 따라서 심혈관 합병증을 예방하기 위해서 아스피린을 복용 중인 제2형 당뇨병환자에서 ARU 측정을 통한 평가가 필요할 것으로 사료된다.

Methods: We studied 181 Korean patients with type 2 diabetes mellitus who were taking aspirin (100 mg/day for ≥ 3 months) and no other antiplatelet agents. We measured the cardio-ankle vascular index (CAVI) and ankle-brachial index (ABI) to evaluate arteriosclerosis. The anthropometric parameters, electrocardiogram, blood pressure, fasting plasma glucose, lipid profiles, hemoglobin A1c, highly sensitive C-reactive protein (hsCRP), homocysteine, and microalbuminuria were measured in each patient. Results: The prevalence of aspirin resistance in type 2 diabetic patients was 9.4% (17 of 181). Those who had aspirin resistance were older than those without aspirin resistance (64.6 ± 10.6 vs. 59.8 ± 8.1, P = 0.024). Aspirin resistance was not associated with fasting plasma glucose, total cholesterol, triglyceride, LDL-cholesterol, HDL-cholesterol, hemoglobin A1c, hsCRP, homocysteine, microalbuminuria, ABI, CAVI, and body mass index. Although aspirin resistance was associated with old age, we could not find any good clinical parameter to predict it.
