연구배경: 산화스트레스는 당뇨병환자의 혈관 합병증 발생에 중요한 역할을 한다. 그러나 당뇨병성 혈관 합병증의 발생과 진행에 대한 기전은 잘 알려져 있지 않다. 저자들은 산화스트레스가 고혈당 유발성 혈관평활근세포의 이동에 중요한 역할을 하는지 알아보고자 하였다. 방법: Sprague-Dawley쥐의 대동맥평활근세포를 5.5 mM D-glucose (정상 농도 포도당). 30 mM D-glucose (고농도 포도당), 5.5 mM D-glucose + 24.5 mM mannitol (삼투압 대조군) 을 포함하는 배지에 72시간 동안 배양하였다. 혈관평활근세포의 이동과 superoxide 음이온 생성, PKC 활성도를 측정하였다. 결과: 고농도 포도당 조건에서 배양된 군에서 정상 포도당 조건보다 혈관평활근세포 이동이 유의하게 증가하였다. DPI (10 μmol/L)와 SOD (500 U/mL)로 처치하였을 때 고혈당 유발성 혈관평활근세포 이동이 의미있게 감소하였다. 고농도 포도당 조건에서 유의하게 증가된 superoxide 음이온 생성은 DPI 와 SOD 처치 후 현저히 감소하였다. 고농도 포도당은 PKC-δ 활성도도 유의하게 증가시켰다. PKC-δ siRNA 핵내 주입 또는 선택적 PKC-δ 억제제인 rottlerin으로 처치된 혈관평활근세포는 고농도 포도당 조건에서의 NBT 염색과 NAD(P)H oxidase 활성도가 유의하게 감소하였다. 또한 PKC-δ 억제는 고농도 포도당 조건에서 증가되었던 혈관평활근세포 이동을 현저히 감소시켰다. 결론: 고농도 포도당에 의해 증가된 혈관평활근세포의 이동에 활성산소족이 중요하게 관여되어 있으며 고농도 포도당에 의해 유발된 NAD(P)H oxidase의 활성화가 PKC-δ 의존성 경로에 의해 매개됨을 알 수 있었다.

Background: Oxidative stress contributes to vascular diseases in patients with diabetes. As the mechanism of development and progression of diabetic vascular complications is poorly understood, this study was aimed to assess the potential role of hyperglycemia-induced oxidative stress and to determine whether the oxidative stress is a major factor in hyperglycemia-induced migration of vascular smooth muscle cells (VSMCs). Methods: We treated primary cultured rat aortic smooth muscle cells for 72 hours with medium containing 5.5 mM D-glucose (normal glucose), 30 mM D-glucose (high glucose) or 5.5 mM D-glucose plus 24.5 mM mannitol (osmotic control). We measured the migration of VSMCs and superoxide production. Immunoblotting of PKC isozymes using phoshospecific antibodies was performed, and PKC activity was also measured. Results: Migration of VSMCs incubated under high glucose condition were markedly increased compared to normal glucose condition. Treatment with diphenyleneiodonium (DPI, 10 μmol/L) and superoxide dismutase (SOD, 500 U/mL) significantly suppressed high glucose-induced migration of VSMCs. Superoxide production was significantly increased in high glucose condition and was markedly decreased after treatment with DPI and SOD. High glucose also markedly increased activity of PKC-δ isozyme. When VSMCs were treated with rottlerin or transfected with PKC-δ siRNA, nitro blue tetrazolium (NBT) staining and NAD(P)H oxidase activity were significantly attenuated in the high glucose-treated VSMCs. Furthermore, inhibition of PKC-δ markedly decreased VSMC migration by high glucose. Conclusion: These results suggest that high glucose-induced VSMC migration is dependent upon activation of PKC-δ, which may responsible for elevated intracellular ROS production in VSMCs, and this is mediated by NAD(P)H oxidase.
