배경 및 목적 CCU에 입원하여 인공호흡기나 IABP의 사용이 필요한 합병증이 동반된 고위험 급성 심근경색증 환자들의 사망률은 매우 높다고 보고되고 있다. 저자 등은 본 연구를 통하여 이러한 환자들의 원내 사망률과 사망과 관련된 유의한 예측인자를 알아보고자 하였다. 방 법 급성 심근경색증의 합병증으로 심인성 쇼크나 급성 호흡부전으로 관상동맥 집중치료실에서 인공호흡기나 IABP를 사용하여 치료 받은 환자 134명을 후향적으로 분석하여 생존군과 사망군 사이의 임상적 특징, 혈액학적 검사소견, 심초음파 소견, 관상동맥 조영술 소견 및 GRACE score 등을 비교하였다. 결 과 원내 사망률은 56.0%이었고 임상적 특징은 사망군에서 평균 입원일수가 짧고 흡연력과 관상동맥 우회수술의 과거력, 심인성 쇼크 및 IABP 사용빈도가 의미있게 많았다 (각각 p<0.05). 관상동맥 조영술에서 생존군에서 관상동맥 중재술(PCI)을 보다 많이 시행하였고 (p=0.001), 진단의학검사에서 사망군에서 hs-CRP와 NT-proBNP 수치가 높았으며 (각각 p<0.05), GRACE score 역시 사망군에서 의미있게 높았다(p<0.0001). 원내 사망과 관련된 유의한 예측인자를 알아보기 위한 multiple logistic regression test에서 IABP 사용여부 (OR: 3.52, 1.182∼11.541 95% CI: p=0.019), PCI 여부(OR: 0.29, 0.057∼0.843 95% CI: p=0.027)와 GRACE 점수 (OR: 1.03, 1.015∼1.041 95% CI: p=0.043)가 유의한 사망 예측인자였다. 결 론 관상동맥 집중치료실에 입원하여 인공호흡기나 IABP를 사용하여 치료 받은 고위험 급성 심근경색증 환자의 원내 사망률은 56.0%이었으며, 유의한 사망 예측 인자는 IABP 사용여부, PCI 여부와 높은 GRACE 점수 등이었다.

Background and Objectives:The mortality rate of complicated acute myocardial infarction (AMI) patients who require an artificial ventilator and/or an intra-aortic balloon pump (IABP) at a coronary care unit (CCU) has been reported to be very high. The aims of this study were to evaluate the mortality rate and to investigate the predictors of hospital death for these critically ill patients. Subjects and Methods:We retrospectively analyzed a total of 134 complicated AMI patients who were treated with an artificial ventilator and/or an IABP at the CCU of Chonnam National University Hospital between January 2004 and December 2005. We compared the clinical characteristics, the laboratory, echocardiographic and coronary angiographic findings and the Global Registry of Acute Coronary Event (GRACE) score between the survivors and non-survivors. Results:The overall mortality rate was 56.0%. The following variables were more common in non-survivors: smoking and a history of coronary artery bypass grafting, the incidence of cardiogenic shock and the use of an IABP and/or percutaneous coronary intervention (PCI). The non-survivors had higher levels of high sensitivity C-reactive protein (hs-CRP) and N-terminal pro-brain natriuretic peptide (NT-proBNP) and a higher GRACE score (p<0.05, respectively). The predictive factors for hospital mortality according to multiple logistic regression analysis were the use of an IABP (odds ratio: 3.52, 95% CI: 1.182-11.541), PCI (odds ratio: 0.29, 95% CI: 0.057-0.843) and a high GRACE score (odds ratio: 1.03, 95% CI: 1.015-1.041).
