목적: 회절형 다초점 인공수정체인 AcrySof® ReSTOR®의 근거리 및 원거리 시력교정 효과와 환자의 만족도를 조사 하고, 술 후 만족도가 떨어지는 원인이 무엇인지 알아보고자 하였다. 대상과 방법: 한 술자에 의해 백내장 초음파 유화술 및 AcrySof® ReSTOR® 삽입술을 받은 환자 19명 23안을 대상으로 3개월간의 임상결과를 후향적으로 조사하였다. 술 후 1일, 1주, 1개월, 3개월에 각각 추적 관찰하여 나안 근거리 및 원거리 시력, 근거리 및 원거리 최대 교정시력 및 현성 굴절검사상의 굴절오차를 시행하였고, 술 후 2개월째 환자의 주관적인 시각증상(달무리, 눈부심, 야간시력장애) 및 만족도에 대해 조사하였다. 결과: 술 후 3개월째 평균 나안 근거리 및 원거리 시력은 0.59±0.24 (0.25±0.22 LogMAR unit), 0.78±0.27 (0.13±0.10 LogMAR unit)이었고, 최대 교정 근거리 및 원거리 시력은 0.73±0.16 (0.15±0.09 LogMAR unit), 0.91±0.21 (0.06±0.13 LogMAR unit)이었다. 술 후 3개월째 만족도에 대한 조사에서 야간 운전시 이외 나안 근거리, 중간거리, 원거리 시력 및 전반적인 만족도는 비교적 만족스러운 결과를 보였으며, 술 후 심한 눈부심 및 심한 달무리 현상을 호소하는 경우가 19명 중 2명(10.2%) 및 19명 중 1명(5.3%)이었다. 술 후 3개월째 결과가 불만족스러웠던 경우는 7안(30.4%)이었고, 술 전 안병증의 빈도가 높을수록, 술 전 및 술 후 각막 난시가 1.0디옵터보다 클수록 만족스러운 군에 비해 통계적으로 유의한 차이를 보였다. 결론: AcrySof® ReSTOR® 다초점 인공수정체는 비교적 만족스런 나안 원거리 및 근거리 시력 교정, 술 후 환자의 만족도를 보였지만, 술 후 환자의 만족도를 높이기 위해서는 술 전 환자 선정시 안병증의 기왕력이 없으면서 각막난시 가 1.0 디옵터 이내인 환자를 대상으로 해야하고 환자의 생활 패턴 및 개인적인 성향을 고려해야 할 것으로 사료된다.

The main outcome measures, taken at postoperative 1 day, 1 week, 1 month, and 3 months, were uncorrected and corrected near and distant visual acuity, refractory errors, subjective visual symptoms (glare, halo, and night vision), and satisfaction. Results: At the 3-month postoperative visit, the mean uncorrected near and distant visual acuities were 0.59±0.24 (0.25±0.22 LogMAR unit) and 0.78±0.27 (0.13±0.10 LogMAR unit), respectively. In addition, patients’ satisfaction with uncorrected near vision, intermediate vision, far vision, and general visual performance were better than their satisfaction with night vision. Glare and halos were reported as severe by only 10.2% and 5.3% of patients, respectively. The seven eyes with poor patient satisfaction included eyes with a high incidence of preoperative ocular diseases or preoperative and postoperative high corneal astigmatisms of more than 1.0 diopter.
