목적: 부분 위 절제술을 받은 환자를 대상으로 역류성 식도염의 발생률과 그에 영향을 주는 위험인자에 대해 알아보고자 한다. 대상 및 방법: 2003년 6월부터 2004년 12월까지 분당서울대병원에서 부분 위 절제술을 받은 환자 225명 중에서 6개월 이상 추적관찰이 되었고 추적 기간 동안 1회 이상의 내시경을 받은 201명의 환자를 대상으로 2007년 12월까지의 의무 기록을 후향적으로 분석하였다. 역류성 식도염은 Los Angeles (LA)분류에 따라 분류하였으며 미세변화도 역류성 식도염의 범주에 포함시켰다. 결과: 총 201명의 환자 중에 빌로스 I과 빌로스 II로 수술 받은 사람은 각각 173명과 28명이었다. 역류성 식도염의 누적발생률은 40.8%였고 이중 미세변화는 31.3%, LA 분류 A는 6.0%, LA 분류 B는 3.5%였다. 다변량 분석 결과 65세 이상의 고령, 40개월 이상의 장기간 추적 관찰, 담즙 역류성 위염, 수술 후 비만(각각 p=0.04, p=0.03, p<0.01, p=0.02)이 술 후 역류성 식도염의 독립적 위험 인자였다. 미세변화의 경우 4회 이상 내시경을 받았던 경우, 담즙 역류성 위염, 수술 후 비만(p=0.012, p=0.002, p=0.038)이 위험 요인이었고 미세변화를 제외한 역류성 식도염의 경우에는 65세 이상의 고령, 40개월 이상의 추적 관찰, 수술 전 헬리코박터 제균 치료(각각 p=0.03, p=0.01, p<0.01)가 위험 인자였다. 결론: 부분 위 절제술 후 미세 변화를 보이는 역류성 식도염 발생이 증가하며 담즙 역류가 중요한 기전으로 보인다. 수술 전에 헬리코박터 제균 치료를 한 경우는 미세 변화를 제외한 역류성 식도염의 발생과 관련 있었는데 이에 대해서는 전향적인 연구로 확인이 필요할 것으로 생각한다.

Background/Aims: The aims of this study were to evaluate the incidence of reflux esophagitis in patients after a subtotal gastrectomy and to determine risk factors for reflux esophagitis. Methods: Among 225 patients who underwent a subtotal gastrectomy from June 2003 to December 2004 at Seoul National University Bundang Hospital, 201 patients who received follow-up for more than six months and underwent at least one endoscpoic examination were retrospectively reviewed. We used the Los Angeles (LA) classification system and included a minimal change for reflux esophagitis. Results: There were 173 patients who underwent a Billroth I procedure and 28 patients who underwent a Billroth II procedure. The cumulative incidence of reflux esophagitis was 40.8%. A patient age ≥65 years (p=0.04), a follow-up duration ≥40 months (p=0.03), bile reflux gastritis (p<0.01) and postoperative obesity (p=0.02) were significant risk factors for the development of reflux esophagitis including a minimal change. The number of postoperative endoscopies ≥4 (p=0.012), bile reflux gastritis (p=0.002) and postoperative obesity (p=0.038) were risk factors for a minimal change. A patient age ≥65 years (p=0.04), a follow-up duration ≥40 months (p=0.03) and Helicobacter pylori eradication before surgery (p<0.01) were independent risk factors for LA grade A/B reflux esophagitis. Conclusions: Bile reflux gastritis is a risk factor for the development of reflux esophagitis after a subtotal gastrectomy. Preoperative helicobacter eradication is also associated with reflux esophagitis, except for a minimal change, but further studies are needed.
