목적: 본 연구는 자궁내 태아사망의 임상적 특성과 통계학적으로 의의있는 모체측 및 주산기 위험인자들을 알아보아 자궁내 태아사망의 예방 및 관리에 도움이 되고자 한다. 방법: 2000년 1월부터 2007년 12월까지 8년간 본원 산부인과에서 발생한 62예의 자궁내 태아사망을 대상으로 의무기록을 토대로 한 임상적 고찰을 시행하였고 생아출생군 3,887예와 함께 통계학적 분석을 시행하여 모체측 및 주산기 위험인자들을 분석하였으며, 이의 위험도를 산출하였다. 결과: 자궁내 태아사망의 전체 발생빈도는 1.57%이고 연구기간 동안 연도별 발생율의 차이는 없었다. 대상 임신부의 연령은 21~41세이고, 30에서 34세군에서 가장 많은 빈도를 보였다 (1.63%). 미산부가 가장 많았고 (48.78%) 이전 자궁내 태아사망의 기왕력은 5예 (8.06%)였다. 자궁내 태아사망의 임신주수는 임신 20~29주가 48.4%로 가장 많았고 남 녀 성비는 1.03:1이었다. 분만방법은 대부분의 예에서 (80.64%) 질분만을 하였으며 (자연진통으로 인한 분만: 62.90%, 유도분만: 17.74%), 개복수술을 한 경우가 19.35%였다. 자궁내 태아사망의 위험인자는 임신부의 비만, 감염, 태반 이상, 고령 임신부, 원인 미상 순이었고, 50 예에서 두 가지 이상의 위험인자를 가지고 있었다. 전자간증 (OR 2.733; 95% CI 1.408~5.306)과 태반조기박리 (OR 5.190;95% CI 2.165~12.441)가 통계적으로 의의있는 위험도를 보였다. 결론: 자궁내 태아사망의 위험인자들을 비교 분석하여 임신부의 산전관리에 적용하는 것이 앞으로 자궁내 태아사망의 발생과 재발 방지에 도움이 될 것으로 판단되며 각각의 위험인자들의 위험도 분석은 임상에서 실제적인 도움이 될 것으로 판단된다.

The age distribution of mother with IUFD was between 21 to 41, and was highest in the 30 to 34 year old age group (41.94%). There were 29 cases (46.78%) with nulliparous and 5 cases (8.06%) with previous IUFD. IUFD was the most prevalent (48.39%) at 20-29 weeks of gestation and the sex ratio of male versus female fetus was 1.03:1. Most of cases (80.64%) were delivered vaginally (spontaneous labor: 62.90%, labor induction: 17.74%), and laparotomy was 19.35%. Risk factors were maternal obesity, infection, placental abnormality, advanced maternal age (≥35), and unexplained cause in that order. 50 cases included more than two risk factors. The risk analysis showed statistically significant risk in preeclampsia (OR 2.733; 95% CI 1.408-5.306) and placental abruption (OR 5.190; 95% CI 2.165-12.441).
