본 연구의 목적은 영국 정신장애인의 탈원화 경험에 관한 연구를 통해 우리나라 정신장애인 보호정책에의 함의를 찾기 위한 것이다. 20세기 중반, 서구 사회에 정신장애인의 탈원화가 새로운 패러다임으로 등장한 이후, 영국은 탈원화가 가장 성공적으로 진행된 것으로 평가되고 있다. 우리나라는 1995년 사회 복귀를 강조한 정신보건법이 제정되었지만 그 후 오히려 정신병원과 병상수가 증가하는 역탈원화 경향을 보이고 있다. 이러한 상황에서 이 연구는 영국의 탈원화 경험을 분석하여 그에 기여한 요인을 살펴봄으로써 우리나라 정신장애인 정책에의 함의를 찾고자 수행되었다.
연구결과, 영국의 탈원화 과정에는 환자의 자기선택권을 강화한 정신보건법, 일관되게 추진된 지역사회보호정책, 강력한 대형 정신병원 폐쇄정책, 입원결정과 의료비지불의 일원화된 비용 억제장치, 그리고 사회과학자, 시민운동가, 정신장애인 보호자 등에 힘입은 정신장애인 인권신장 등이 관여하고 있었다. 이러한 요인들에 비추어 우리나라 상황을 점검해본 결과, 정신보건법에 내재된 강제입원 요소, 민간주도 정신병원의 경제적 이해관계, 중앙정부와 지방정부간의 정신보건 재정조달의 탈원화 역인센티브 구조, 정신장애인에 대한 낮은 인권의식 등이 개선되어야 할 문제점으로 발견되었고, 이에 기초하여 정책적 대안을 제시하였다.

Since the 1950s, deinstitutionalization has been a trend in mental health care provision throughout western world. Many factors contributed to this movement including failure of long-term hospitalization treatment, isolation of resident of in-patients, and poor and inhuman standards of care. Today, many of the psychiatric patients were cared in the community living their own lives due to deinstitutionalization in Britain. In spite of the legislation of the Mental Health Law in 1995, the number of hospitals and in-patients has been even more increasing in Korea. This study tried to figure out what made big difference between Britain and Korea in deinstitutionalization. Based on the research investigation of Britain experiences, this paper concludes that the most important factors of deinstitutionalization in Britain are strict legal prohibition of involuntary admission, ceaseless implementing of community care policy, a closure of asylums, cost-saving financing mechanism, and human right movement for the mental ill. This study draws implications from the experiences of Britain for deinstitutionalization in Korea. First of all, the poor degree to which Korean society is willing to accept the mental ill is an obvious obstacle to deinstitutionalization. Besides, there are several factors impeding deinstitutionalization in Korea such as the involuntary admission mechanism rooted in the Mental Health Law, an incentive to anti-deinstitutionalization in the mental health care financing system between central and local government, and the economic interest of private mental hospital industry.
