A method and device for delivering ventricular resynchronization pacing therapy in conjunction with electrical stimulation of nerves which alter the activity of the autonomic nervous system is disclosed. Such therapies may be delivered by an implantable device and are useful in preventing the deleterious ventricular remodeling which occurs as a result of a heart attack or heart failure. The device may perform an assessment of cardiac function in order to individually modulate the delivery of the two types of therapy.
What is claimed is: 1. An implantable device for delivering cardiac therapy to a patient, comprising: one or more pacing channels for delivering pacing pulses to one or more ventricular sites; a sympathetic inhibition channel for stimulating nerves which inhibit sympathetic nerve activity; a contro
What is claimed is: 1. An implantable device for delivering cardiac therapy to a patient, comprising: one or more pacing channels for delivering pacing pulses to one or more ventricular sites; a sympathetic inhibition channel for stimulating nerves which inhibit sympathetic nerve activity; a controller for controlling the delivery of pacing pulses to one or more ventricular sites during a cardiac cycle in accordance with a programmed pacing mode; wherein the controller is programmed to deliver remodeling control therapy (RCT) by delivering ventricular pacing in a manner which pre-excites a region of the ventricular myocardium so as to mechanically unload that region during systole; wherein the controller is further programmed to delivery anti-remodeling therapy (ART) in conjunction with RCT by inhibiting sympathetic nerve activity via the sympathetic inhibition channel; means for assessing cardiac function and wherein the controller is programmed to deliver RCT and ART as modulated by an assessment of cardiac function; and, wherein ART is modulated in response to the assessment of cardiac function by varying the duty cycle of neural stimulation pulses. 2. The device of claim 1 further comprising a parasympathetic stimulation channel for stimulating parasympathetic nerve activity and wherein the anti-remodeling therapy delivered by the controller further includes stimulation of parasympathetic nerve activity. 3. The device of claim 2 wherein the parasympathetic stimulation channel includes a pulse generator for outputting neural stimulation pulses and a nerve cuff electrode adapted for disposition round a parasympathetic nerve. 4. The device of claim 2 wherein the parasympathetic stimulation channel includes a pulse generator for outputting neural stimulation pulses and an electrode adapted for intravascular disposition in order to transvascularly stimulate a parasympathetic nerve adjacent a blood vessel. 5. The device of claim 2 further wherein the electrode and pulse generator are incorporated into a satellite unit which includes a wireless receiver and further comprising a wireless transmitter interfaced to the controller for controlling the operation of the satellite unit via a wireless link. 6. The device of claim 2 further wherein the electrode and pulse generator are incorporated into a satellite unit which includes an RF receiver and further comprising an RF transmitter interfaced to the controller for controlling the operation of the satellite unit via an RF link. 7. The device of claim 1 wherein the sympathetic inhibition channel includes a pulse generator for outputting neural stimulation pulses and an electrode adapted for disposition near an arterial baroreceptor. 8. The device of claim 7 wherein the sympathetic inhibition channel includes means for applying baroreflex stimulation to a baroreceptor site in a pulmonary artery. 9. The device of claim 7 wherein the sympathetic inhibition channel includes means for applying baroreflex stimulation to nerve endings in a cardiac fat pad. 10. The device of claim 7 wherein the sympathetic inhibition channel includes means for applying baroreflex stimulation to an afferent nerve. 11. The device of claim 1 wherein the controller is programmed to deliver RCT and ART simultaneously. 12. The device of claim 1 wherein the controller is programmed to deliver RCT and ART separately at programmed time intervals. 13. The device of claim 1 wherein the controller is further programmed to separately modulate the delivery of parasympathetic stimulation and sympathetic inhibition in accordance with the assessment of cardiac function. 14. The device of claim 1 wherein the cardiac function assessing means comprises a sensor for measuring cardiac output and wherein the controller is programmed to modulate the delivery of RCT and ART in accordance with the measured cardiac output. 15. The device of claim 14 wherein the cardiac output sensor is a trans-throracic impedance measuring circuit. 16. The device of claim 1 wherein the cardiac function assessing means comprises an arterial blood pressure sensor and wherein the controller is programmed to modulate the delivery of RCT and ART in accordance with the measured blood pressure. 17. The device of claim 1 wherein the cardiac function assessing means comprises an exertion level sensor for measuring the patient's exertion level, and wherein the controller is programmed to modulate the delivery of RCT and ART in accordance with the measured exertion level. 18. The device of claim 17 wherein the exertion level sensor measures minute ventilation. 19. The device of claim 1 wherein the cardiac function assessment means includes a sensing channel for measuring autonomic balance and wherein the cardiac function assessment includes an assessment of the patient's autonomic balance. 20. The device of claim 19 further comprising: circuitry for measuring and collecting time intervals between successive intrinsic beats, referred to as a BB interval, and storing the collected intervals as a discrete BB interval signal, filtering the BB interval signal into defined high and low frequency bands, and determining the signal power of the BB interval signal in each of the low and high frequency bands, referred to LF and HF, respectively; and, circuitry for computing an LF/HF ratio and assessing autonomic balance by comparing the LF/HF ratio to a specified threshold value. 21. The device of claim 19 wherein the sensing channel for measuring autonomic balance measures electrical activity in sympathetic and parasympathetic nerves. 22. The device of claim 1 wherein RCT is modulated in response to the assessment of cardiac function by varying the extent of pre-excitation provided to a ventricular pacing site. 23. The device of claim 1 wherein ART is modulated in response to the assessment of cardiac function by varying the amplitude of neural stimulation pulses. 24. The device of claim 1 wherein ART is modulated in response to the assessment of cardiac function by varying the frequency of neural stimulation pulses. 25. The device of claim 1 further comprising: a sensing channel for detecting intrinsic cardiac activity; a shock channel for delivering a cardioversion/defibriulation shock pulse; and, wherein the controller is programmed to deliver a shock pulse upon detection of a tachyarrhythmia above a specified rate threshold. 26. The device of claim 1 wherein the controller is programmed to deliver RCT as biventricular pacing so as to pre-excite one of the ventricles relative to the other. 27. The device of claim 1 wherein the controller is programmed to deliver RCT as multi-site ventricular pacing where at least one of the ventricles is paced at a plurality of sites so as to pre-excite one or more of the sites relative to the other sites. 28. The device of claim 1 wherein the controller is programmed to deliver RCT as left ventricle-only pacing which pre-excites the left ventricle relative to the right ventricle during systole. 29. The device of claim 1 wherein the assessment of cardiac function is transmitted to a remote monitor via an RF telemetry link. 30. The device of claim 29 wherein the controller is programmed to modify the delivery of RCT and ART in response to communications received from the remote monitor via the RF telemetry link.
