A system and method for predicting the effect of patient self-care actions on a disease control parameter. A future disease control parameter value X(tj) at time tj is determined from a prior disease control parameter value X(ti) at time ti based on an optimal control parameter value R(tj) at time tj, the difference between the prior disease control parameter value X(ti) and an optimal control parameter value R(ti) at time ti, and a set of differentials between patient self-care parameters having patient self-care values SM(ti) at time ti and optimal self-care parameters having optimal self-care values OM(ti) at time ti. The differentials are multiplied by corresponding scaling factors KM. The system includes an input device for entering the patient self-care values SM(ti). A memory stores the optimal control parameter values R(ti) and R(tj), the prior disease control parameter value X(ti), the optimal self-care values OM(ti), and the scaling factors KM. A processor in communication with the input device and memory calculates the future disease control parameter value X(tj). A display is connected to the processor to display the future disease control parameter value X(tj) to a patient.

The invention claimed is: 1. An electronic device for outputting a signal according to at least one control parameter value of a patient, comprising: a data recording device configured to receive a first disease control parameter value; a memory comprising one or more optimal disease control parameter values, one or more self-care values of the patient, one or more optimal self-care values and one or more scaling factors; a microprocessor, in communication with the data recording device and the memory, programmed to calculate a value based upon the one or more self-care values, the one or more optimal disease control parameter values and the scaling factors; and a display configured to display information according to the calculated value, wherein the information displayed enables the patient to select appropriate self care actions and the disease control parameter values are related to the patient self-care of a chronic or persistent condition being managed by the patient. 2. The electronic device according to claim 1, further comprising a housing configured to hold the microprocessor, the memory and the display, wherein the housing is sufficiently compact to be held in the patient's hand. 3. The electronic device according to claim 1, further comprising an output port configured to couple the electronic device to an external device. 4. The electronic device according to claim 3, wherein the external device comprises one or more devices selected from the group consisting of a healthcare provider computer, a server, and a multi-media system. 5. The electronic device according to claim 3, wherein said output port is configured to connect the electronic device to the external device using one or more communication media selected from the group consisting of telephone networks, cellular networks, wireless networks, cable networks and computer networks. 6. The electronic device according to claim 1, further comprising a modem for establishing a communication link through a communication medium. 7. The electronic device according to claim 1, wherein the processor is programmed to calculate a plurality of future disease control parameter values representative of a corresponding plurality of expected actual disease control parameter measurements of the patient. 8. The electronic device according to claim 7, wherein the display is configured for displaying the plurality of calculated disease control parameter values in a graphical format. 9. The electronic device according to claim 1, wherein the data recording device comprises a measuring device selected from the group consisting of a peak flow meter, a cholesterol meter, a blood pressure cuff, electrocardiograph (ECG or EKG), weight scale and pulse rate monitor. 10. The electronic device according to claim 1, wherein the one or more disease control parameter values comprise one or more values selected from the group consisting of respiratory flow, peak flow rate, weight, blood pressure, cholesterol, T-cell or viral count, and number, timing or frequency of disease-related episodes. 11. The electronic device according to claim 1, wherein the chronic or persistent condition being managed by the patient comprises one or more diseases selected from the group consisting of asthma, obesity, hypertension, coronary artery disease, heart disease, chronic obstructive pulmonary disease, panic disorder and nicotine addiction. 12. The electronic device according to claim 1, wherein the electronic device comprises a device selected from the group consisting of a device configured to communicate with a server via a wireless network, a device configured to communicate with a multi-media processor unit via a wireless network, a cellular phone, a mobile telephone, and a personal digital assistant (PDA). 13. The electronic device according to claim 1, wherein the microprocessor is configured to determine, adjust or determine and adjust the one or more scaling factors. 14. A simulation system comprising: a processor unit; a server remotely situated from the processor unit, wherein the server is configured to communicate with the processor unit through at least one communication network; a hand-held apparatus remotely situated from the server; and a simulation program stored in a computer readable medium, wherein (i) the simulation program is configured to generate graphs of simulated disease control parameter values and (ii) the disease control parameter values are related to the patient self-care of a chronic or persistent condition being managed by the patient. 15. The simulation system according to claim 14, wherein the simulation program is run on the server. 16. The simulation system according to claim 14, wherein the simulation program is run on the processor unit. 17. The simulation system according to claim 14, wherein the simulation program is run on the hand-held apparatus. 18. The simulation system according to claim 14, wherein the simulation program is configured to generate one or more tables of values comprising a time column, an optimal control parameter value column, a simulated control parameter value column, one or more self-care value differential columns indicating differentials between patient self-care parameter values and optimal self-care parameter values, and one or more corresponding scaling factor columns for weighting the corresponding self-care value differentials. 19. The simulation system according to claim 14, wherein the simulation program is configured to calculate future disease control parameter values from prior calculated disease control parameter values, actual disease control parameter values or a combination of calculated and actual disease control parameter values. 20. The simulation system according to claim 14, wherein the processor unit comprises a webTV compliant processor unit. 21. The simulation system according to claim 14, wherein the processor unit is connected to a display unit. 22. The simulation system according to claim 21, wherein the display unit comprises a television. 23. A simulation system comprising: a processor unit configured to run a simulation program stored in a computer readable medium, wherein (i) the simulation program is configured to generate graphs of simulated disease control parameter values and (ii) the disease control parameter values are related to the patient self-care of a chronic or persistent condition being managed by the patient; a recording device configured to communicate with the processor unit; and a healthcare provider computer remotely situated from the processor unit and the recording device, wherein the healthcare provider computer is configured to communicate with the processor unit and the recording device. 24. The simulation system according to claim 23, wherein the recording device is further configured to communicate with the healthcare provider computer via at least one communication network. 25. The simulation system according to claim 23, wherein the recording device is further configured to communicate with the healthcare provider computer via a standard connection cable.
