Methods, systems and devices are provided for monitoring respiratory disorders based on monitored factors of a photoplethysmography (PPG) signal that is representative of peripheral blood volume. The monitored factors can be respiratory effort as well as respiratory rate and/or blood oxygen saturation level. The systems and devices may or may not be implanted in a patient.

A system for monitoring respiration, comprising: a light source and a light detector to produce a photoplethysmography (PPG) signal that is representative of peripheral blood volume; and a monitor configured to monitor respiratory rate based on the PPG signal, wherein the monitor is configured to determine an average of the PPG signal; add an offset to the average to produce a first threshold; subtract an offset from the average to produce a second threshold; and monitor respiratory rate by determining a number of transitions from one of the first and second thresholds to the other threshold. 2. The system of claim 1, wherein the monitor is also configured to monitor respiratory effort by determining amplitudes of the PPG signal. 3. The system of claim 1, wherein the monitor is configured to detect an exacerbation of a respiratory disorder based on the monitored respiratory effort and respiratory rate. 4. The system of claim 3, wherein the monitor is further configured to detect an exacerbation of the respiratory disorder based on the monitored respiratory effort and respiratory rate. 5. The system of claim 1, wherein the monitor is further configured to monitor blood oxygen saturation level based on the PPG signal. 6. The system of claim 5, wherein the monitor is further configured to: detect an exacerbation based on the monitored respiratory effort, respiratory rate and blood oxygen saturation level. 7. The system of claim 1, wherein the light source and the light detector can be used to produce the PPG signal while a patient, whose respiratory disorder is being monitored, is sleeping. 8. The system of claim 7, wherein the monitor is configured to detect an episode of sleep apnea based on the monitored respiratory effort. 9. The system of claim 7, wherein the monitor is further configured to: detect an episode of sleep apnea based on the monitored respiratory rate and respiratory effort. 10. The system of claim 9, further comprising: a stimulator configured to stimulate the patient in response to the monitor detecting an episode of sleep apnea. 11. The system of claim 10, wherein the stimulator is configured to stimulate the patient by producing at least one of an audible stimuli, a vibrating stimuli and an electrical stimuli. 12. The system of claim 9, further comprising: an alert indicator that is triggered in response to the monitor detecting an episode of sleep apnea. 13. The system of claim 7, wherein the monitor is further configured to monitor blood oxygen saturation level based on the PPG signal. 14. The system of claim 13, wherein the monitor is further configured to detect an episode of sleep apnea based on the monitored respiratory effort, respiratory rate and blood oxygen saturation level. 15. The system of claim 14, further comprising: a stimulator configured to stimulate the patient in response to the monitor detecting an episode of sleep apnea. 16. The system of claim 15, wherein the stimulator is configured to produce at least one of an audible stimuli, a vibrating stimuli and an electrical stimuli. 17. The system of claim 14, further comprising: an alert indicator that is triggered in response to the monitor detecting an episode of sleep apnea. 18. The system of claim 13, wherein the monitor is further configured to: monitor respiratory effort based on the PPG signal; and detect an episode of sleep apnea based on the monitored respiratory rate, respiratory effort and blood oxygen saturation level. 19. The system of claim 18, further comprising: a stimulator configured to stimulate the patient in response to the monitor detecting an episode of sleep apnea. 20. The system of claim 19, wherein the stimulator is configured to produce at least one of an audible stimuli, a vibrating stimuli and an electrical stimuli. 21. The system of claim 18, further comprising: an alert indicator that is triggered in response to the monitor detecting an episode of sleep apnea. 22. The system of claim 1, wherein the light source and the light detector are configured to be implanted in a patient whose respiration is being monitored. 23. A system for monitoring respiration, the system comprising: a light source and a light detector to produce a photoplethysmography (PPG) signal that is representative of peripheral blood volume; and a monitor configured to monitor respiratory rate based on the PPG signal by analyzing a frequency spectrum of the PPG signal and detecting a peak frequency. 24. A system for detecting episodes of sleep apnea, and distinguishing between hypopnea, obstructive sleep apnea (OSA) and central sleep apnea (CSA), comprising: a light source and a light detector configured to produce a photoplethysmography (PPG) signal that is representative of the patient's blood volume; and a monitor configured to: monitor respiratory effort and respiratory rate based on the PPG signal; detect an episode of sleep apnea based on at least one of the monitored respiratory effort and respiratory rate; and distinguish between hypopnea, OSA and CSA, for a detected episode of sleep apnea, based on the monitored respiratory effort and respiratory rate. 25. The system of claim 24, wherein the monitor is further configured to monitor blood oxygen saturation level of the patient based on the PPG signal. 26. The system of claim 25, wherein the monitor is configured to detect an episode of sleep apnea based at least two of the monitored respiratory effort, respiratory rate and blood oxygen saturation level. 27. The system of claim 24, wherein the monitor is adapted to distinguish between hypopnea, OSA and CSA, for a detected episode of sleep apnea, based on the monitored respiratory effort, respiratory rate and blood oxygen saturation level. 28. The method system of claim 24, wherein the monitor is configured to monitor: determine an average of the PPG signal; add an offset to the average to produce a first threshold; subtract an offset from the average to produce a second threshold; and monitor respiratory rate by determining a number of transitions from one of the first and second thresholds to the other threshold.
