The invention claimed is: 1. A patient support apparatus comprising a frame, a plurality of casters having wheels, each of the casters coupled to the frame and supporting the frame above a floor, a brake coupled to at least one caster of the plurality of casters, the brake including first and second portions movable relative to the at least one caster, the brake having a first braking position and a second braking position, a brake pedal coupled to the frame and movable to place the brake in the first braking position, a push handle coupled to the frame and gripable to maneuver the patient support apparatus along the floor, and a brake handle coupled to the push handle and movable to place the brake in the second braking position, wherein (i) the first and second portions of the brake engage a wheel of the at least one caster of the plurality of casters with a first force when placed in the first braking position, (ii) only the first portion of the brake engages a wheel of the at least one caster of the plurality of casters with a second force when placed in the second braking position, and (iii) the second force is less than the first force. 2. The apparatus of claim 1, wherein the frame comprises a lower frame and an upper frame supported above the lower frame and movable relative to the lower frame, the plurality of casters are coupled to the lower frame, and the push handle is coupled to the upper frame. 3. The apparatus of claim 1, wherein the frame has first and second ends with two corner regions at each end and the plurality of casters comprises four casters coupled to the frame near the four corners regions. 4. The apparatus of claim 3, wherein each of the four casters has a brake coupled thereto, the brake pedal is moveable to place at least one of the brakes in the first braking position, and the brake handle is moveable to place at least two of the brakes in the second braking position. 5. The apparatus of claim 4, wherein the push handle has a proximal end coupled to the frame and a distal end, and the brake handle is coupled to the distal end. 6. The apparatus of claim 1, further comprising a cable routed from the brake handle through the push handle. 7. The apparatus of claim 6, wherein the push handle includes a hollow tube portion and the cable is routed through the hollow tube portion. 8. The apparatus of claim 6, wherein the push handle includes a bend at a region defining an intersection of a generally vertically-extending portion and a generally horizontally-extending portion, and the cable is routed through the bend. 9. The apparatus of claim 8, wherein the generally horizontally-extending portion extends generally perpendicular to a longitudinal axis of the frame. 10. The apparatus of claim 6, wherein the push handle includes a bottom portion, and the cable exits the push handle through the bottom portion. 11. The apparatus of claim 6, further comprising a brake rod coupled to the lower frame for rotation about a lateral axis, the brake rod being rotatable to move the brake into the first braking position and the second braking position, the cable being coupled to the brake rod, and the cable causing rotation of the brake rod when the brake handle is moved. 12. The apparatus of claim 1, wherein the frame includes a lower frame and an upper frame supported above the lower frame, the lower frame has a first end, a second end, a first side, and a second side, and further comprising a plurality of brake pedals some of which are situated adjacent the first and second sides of the lower frame and others of which are situated adjacent the first and second ends of the lower frame. 13. The apparatus of claim 12, wherein the brake pedals adjacent the first and second ends of the lower frame are coupled to a shaft extending parallel to a longitudinal axis of the lower frame. 14. The apparatus of claim 13, wherein the brake pedals coupled to the longitudinal shaft comprise butterfly pedals. 15. A patient support apparatus comprising a frame, a plurality of wheels coupled to the frame, a brake coupled to at least one of the plurality of wheels, the brake comprising first and second portions, a brake pedal coupled to the frame, the brake pedal being movable relative to the frame between a braking position in which both the first and second portions of the brake engage the associated wheel with a first force and a release position in which both the first and second portions of the brake are disengaged from the associated wheel, a push handle coupled to the frame, and a brake actuator coupled to the push handle, the brake actuator being movable relative to the push handle between a first position in which only the first portion of the brake engages the associated wheel with a second force that is less than the first force and a second position in which the brake is disengaged from the associated wheel.
